Abstract: São consideradas doenças raras, aquelas que afetam um número muito restrito depessoas face à população em geral. Apesar dos avanços das últimas décadas na investigaçãocientífica, para muitas destas doenças, ainda continuam a existir inúmeras necessidadesterapêuticas insatisfeitas.Os medicamentos órfãos são destinados ao tratamento de doenças raras. Contudo,estes medicamentos são difíceis de descobrir e comercializar e, para tal, foi criada legislaçãoque estimulasse a indústria farmacêutica a empreender o seu desenvolvimento.Esta monografia estabelece uma comparação entre os procedimentos regulamentares daEuropa, América e Japão, que estão na base da atribuição da designação órfã a um fármaco.Identifica, também, os principais incentivos disponibilizados pelas Agências Regulamentaresdestes países que apoiam os promotores na investigação e desenvolvimento, viabilizandoassim, a colocação no mercado destes medicamentos.São ainda referidos dois casos de estudo envolvendo doenças raras em Portugal,designadamente, o Cancro do Trato Biliar (BTC) e a Polineuropatia Amiloidótica Familiar(PAF). São descritos os mecanismos fisiopatológicos subjacentes a estas doenças eapresentadas as respetivas inovações terapêuticas em curso e já disponíveis no mercado.Por fim, é abordada a Estratégia Integrada para as doenças raras, instituída pelaComissão Europeia (CE), como uma prioridade comunitária a este nível, bem como asiniciativas tomadas por Portugal, para a implementação da sua Estratégia Integrada para asdoenças raras 2015-2020.
Rare diseases are those that affect a very small number of people, compared with the general population. Despite the advances in scientific research over the last decades, many of these diseases still have unmet therapeutic needs. Orphan drugs are intended for the treatment of rare diseases. However, these drugs are difficult to discover and market, and legislation has been created to stimulate the pharmaceutical industry to develop them . This monograph establishes a comparison between the regulatory procedures in Europe, America and Japan, which are on the basis of the attribution of the orphan designation to a drug. It also identifies the main incentives provided by the Regulatory Agencies of these countries that support the sponsors in the research and development,thus enabling the marketing of these drugs.Two case studies encompassing rare diseases in Portugal, specifically the Biliary Tract Cancer (BTC) and the Familial Amyloid Polyneuropathy (PAF), are also mentioned. The physiopathological mechanisms underlying these diseases are described, as well as the respective therapeutics innovations that are ongoing and those already in the market.Finally, it addresses the Integrated Strategy for rare diseases, established by the European Commission (CE), as a community priority at this level. It also refers the initiatives taken by Portugal, to implement its Integrated Strategy for rare diseases 2015-2020.
