do punho, e é uma das mais frequentes neuropatias compressivas. Esta síndrome está
associada a uma elavada taxa de morbilidade e também de custos para os serviços de
saúde. É caracterizada por dor e parestesias no território do nervo mediano e em casos
avançados, atrofia muscular da mão. Apesar das extensas pesquisas existentes, subsiste
ainda controvérsia sobre a correcta etiologia, diagnóstico e tratamento.
O tratamento da STC inclui o tratamento conservador, que pode ser indicado em
pacientes com sintomas moderados, e o tratamento cirúrgico, quer por cirurgia aberta ou
endoscópica, que constitui o único tratamento curativo.
Existem diversos métodos de diagnóstico, tal como testes clínicos, estudos
imagiológicos e electrodiagnósticos, mas ainda não foi definido um “gold standard”
para o diagnóstico desta condição. Actualmente, o diagnóstico da STC envolve uma
combinação de uma história clínica detalhada, exame físico pormenorizado, estudos
electrodiagnósticos e raramente imagiológicos.
Um diagnóstico correcto é a base para um correcto e eficaz tratamento da STC.
Neste artigo de revisão, o objectivo principal é sumarizar os conceitos actuais
sobre esta patologia, incluindo a epidemiologia, apresentação clínica, etiologia,
fisiopatologia e tratamento. As múltiplas opções de diagnóstico existentes e a
controvérsia sobre qual o melhor método para o mesmo serão também abordadas de
forma detalhada. The carpal tunnel syndrome (CTS) is a compression neuropathy of the median nerve at the wrist level and accounts for the majority of compression neuropathies. This disorder is responsible for considerable patient suffering and healthcare-related costs. It is characterized by pain, paresthesia and, in advanced cases, muscle wasting of the hand but despite extensive research, controversies still exist regarding its precise etiology, diagnosis and treatment. Treatment options for CTS include conservative approaches, which can be helpful for the patient with mild symptoms, but the surgical release of the median nerve, through open or endoscopic techniques, remains the only curative method available. Several diagnostic tools, comprising clinical tests, imaging and electrodiagnostic studies are currently considered but there is no diagnostic “gold-standard” for the condition. In brief, the diagnosis of CTS involves a combination of a detailed clinical history, accurate examination and electrodiagnostic studies; when appropriate, other diagnostic modalities can be implied. An accurate diagnosis supports the correct and successful treatment of CTS, leading to significant social, economical and healthcare-related benefits. With this review, we intend to summarize the current concepts regarding CTS, addressing its epidemiology, clinical presentation, etiology, pathophysiology and treatment. Also highlighted are the multiple diagnostic options and the controversies regarding which one of them is best. |
