Atualmente, a principal indicação da artrodese é o tratamento da falência séptica de artroplastia de joelho.No presente trabalho foram aplicadas alterações ao fixador externo estudado por (Adão 2016), mudando o fixador bilateral monoplanar por um fixador bilateral biplanar (biarticulado), e efetuado um estudo comparativo de modo a perceber qual dos dois modelos proporcionará uma fusão óssea mais eficaz entre os ossos intervenientes. Posteriormente foi avaliado o comportamento do modelo após efetuar o aperto do fixador.Para a modelação de todo o procedimento cirúrgico, foi necessário recorrer aos programas Solidworks®, para a modelação geométrica, e ADINA® para modelação numérica e respetiva simulação numérica utilizando o método dos elementos finitos. Permitindo recriar numericamente o procedimento cirúrgico da artrodese de joelho. Através da análise das tensões de Von Mises nos ossos e nas superfícies de fusão, assim como, a homogeneidade das pressões de contacto no foco da artrodese, a força resultante e os deslocamentos globais e locais, foi possível avaliar a influência na artrodese de cada decisão tomada no desenvolvimento do modelo numérico.Dos resultados apresentados, conclui-se que o fixador biarticulado proporciona menores deslocamentos relativos entre as superfícies de fusão e maiores tensões no foco da artrodese, causando uma melhor distribuição das pressões de contacto. Com o aperto do fixador, conclui-se que após o aperto e descarga das forças de carregamento existe um relaxamento de tensões surgindo uma distribuição mais uniforme por todo o modelo, que até ao momento ainda não tinha sido verificada. Since the beginning of the 20th century, knee arthrodesis has been used to treat knee pain and instability. Currently, the main indication of arthrodesis is the treatment of septic failure of knee arthroplasty.In the present study, the external fixator studied by (Adão 2016) was applied, changing the bilateral fixator to a biplanar fixator (bi-articulated), and carried out a comparative study in order to understand which of the two models will provide a more effective bone fusion between the intervening bones. Afterwards, the behavior of the model was evaluated after tightening the fixator.For the modeling of the entire surgical procedure, it was necessary to use the Solidworks® software for geometric modeling and ADINA® for numerical modeling and its numerical simulation using the finite element method. Allowing to recreate numerically the surgical procedure of knee arthrodesis. Through the analysis of Von Mises tensions in bone and fusion surfaces, as well as the homogeneity of the contact pressures at the focus of the arthrodesis, the resulting force and the global and local displacements, it was possible to evaluate the influence on the arthrodesis of each decision taken in the development of the numerical model.From the results presented, it is concluded that the bi-articulated fixator provides less relative displacements between the fusion surfaces and greater stresses at the focus of the arthrodesis, causing a better distribution of the contact pressures. With the tightening of the fixator, it is concluded that after the tightening and discharge of the loading forces there is a relaxation of stresses resulting in a more uniform distribution throughout the model, which until now had not yet been verified. |
