Abstract: Introdução: Um aspeto significativo das perturbações psicóticas é o seu impacto no funcionamento global do indivíduo. Os sintomas negativos, tais como diminuição da expressão emocional ou avolição, têm sido descritos como os que mais contribuem para a incapacidade consequente a este tipo de perturbações. Adicionalmente, existe um forte estigma social envolvido, o qual, combinado com experiências precoces aversivas e/ou elevadas fontes de ameaça internas (por exemplo, os sintomas) e externas, podem conduzir a sentimentos de desvalorização e dificuldades em sentir-se seguro no mundo social. Tal facto pode ser influenciado pela emergência de vergonha (uma emoção com elevada importância social, crucial para a auto-identidade). Este estudo pretende investigar as associações entre os Sintomas Negativos nas perturbações psicóticas, a Vergonha Externa e a Social Safeness (perceção do mundo social como seguro e tranquilizador); bem como, explorar o papel mediador da Vergonha Externa na relação entre os Sintomas Negativos e o Social Safeness.Método: Uma amostra de 43 participantes diagnosticados com perturbações do espectro da psicose (sobretudo esquizofrenia – 72%) foram recrutados, avaliados com a Positive and Negative Symdrome Scale (PANSS) e preencheram questionários que permitem avaliar a Vergonha Externa (Other as Shamer Scale) e a Social Safeness (Social Safeness and Pleasure Scale).Resultados: Os resultados mostraram que os Sintomas Negativos estão associados com a Vergonha Externa e que ambos estão negativamente associados com a Social Safeness. A Vergonha Externa e a Social Safeness estão forte e significativamente associados; no entanto, estão apenas significativamente associados a quatro sintomas negativos avaliados pela PANSS (isolamento social, dificuldades no pensamento abstrato, reduzido fluxo de conversação e pensamento estereotipado). A mediação simples mostrou que a Vergonha Externa é um mediador significativo na relação entre os Sintomas Negativos e a Social Safeness.Discussão e Conclusões: Os resultados obtidos parecem indicar que, em indivíduos com psicose, a ideia de existir negativamente na mente dos outros é um caminho através do qual os sintomas negativos têm impacto nas interações e sentimento de segurança no mundo social. A vergonha pode ser um alvo terapêutico útil para doentes com sintomas negativos nas perturbações psicóticas.
Background: A significant feature of psychotic disorders is its impact on the individual’s global functioning. Negative symptoms, such as diminished emotional expression or avolition, have been reported as one of the major contributors to disability in this context. Additionally, there is a strong social stigma involved, which combined with early aversive experiences and/or high sources of internal (e.g. symptoms) or external threats, may lead to a sense of devaluation and problems in feeling safe in the social world. This may be influenced by the emergence of shame (an emotion with high social importance, crucial to the self-identity). This study aims to investigate the associations between negative symptoms in psychotic disorders, external shame and Social Safeness (perceiving social world as safe and soothing); and explore the mediator role of external shame in the relationship between negative symptoms and Social Safeness.Method: A sample of 43 patients diagnosed with a psychosis-spectrum disorder (mostly Schizophrenia – 72%) were recruited, assessed with the Positive and Negative Syndrome Scale and completed questionnaires that evaluate external shame (Other as Shamer Scale) and Social Safeness (Social Safeness and Pleasure Scale).Results: Results showed that negative symptoms were correlated with external shame and both were negatively associated with Social Safeness. External shame and Social Safeness were strongly and significantly associated with each other; however, they were only significantly associated with four negative symptoms assed by the PANSS (social withdrawal, difficulty in abstract thinking, lack of flow of conversation and stereotyped thinking). Simple mediation revealed that external shame was a significant mediator in the relationship between negative symptoms total and Social Safeness.Discussion and Conclusions: These results appear to indicate that in individuals with psychosis, the thought of being held negatively in the mind of others is a pathway from which negative symptoms impact on social interactions and safeness. Shame can be a useful therapeutic target for patients with negative symptoms in psychotic disorders.
