Abstract: Resumo:Objetivos: Descrever a prevalência de proliferação epirretiniana (PER) nos buracos maculares lamelares (BML), e correlacionar esta alteração com a morfologia do BML e prognóstico anatómico e funcional após abordagem cirúrgica ou conservadora. Métodos: Série de casos retrospetiva, multicêntrica, que incluiu doentes consecutivos diagnosticados com BML e seguidos por ≥ 6 meses. Observaram-se todas as tomografias de coerência ótica (OCT), onde se avaliaram: presença de membrana epirretiniana (MER) e PER, espessura do pavimento foveal, maior diâmetro e maior diâmetro da porção interna do BML. Nos doentes operados efetuou-se vitrectomia via pars plana com pelagem de MER e membrana limitante interna e tamponamento interno com SF6. Resultados: Foram incluídos 62 olhos (57 doentes), seguidos durante 27,1±19,8 meses, sendo objetivada PER em 33 (53,2%) deles. Comparativamente aos BML sem PER, naqueles com PER objetivou-se um diâmetro significativamente maior (p=0,001), menor espessura do leito (p=0,018), e uma menor razão do diâmetro das camadas internas pelo maior diâmetro (p=0,002). Vinte e sete (81,8%) dos BML com PER apresentaram conformação em tenda, com aproximação dos bordos internos, comparativamente a 23,3% dos BML sem PER. No subgrupo de 13 doentes operados, não se notaram diferenças na performance visual (p=0,687) ou taxa de encerramento (p=0,569) entre os olhos com e sem PER. Conclusões: As PER são entidades descritas recentemente que alteram a fisionomia do BML, com maiores diâmetros máximos nas camadas intermédias e menor espessura do leito originando uma conformação em tenda. Não se encontraram diferenças na performance visual ou taxa de encerramento do BML entre os doentes com e sem PER operados.
Abstract:Objectives: To describe the prevalence of epiretinal proliferations (PER) in lamellar macular holes (BML), to correlate this alteration with BML morphology as well as anatomical and functional prognosis after surgical or conservative approach. Methods: Retrospective, multicentre case series including consecutive patients diagnosed with BML and followed for ≥ 6 months. All of the optical coherence tomographies (OCT) regarding this period were observed and evaluated for: presence of epiretinal membrane (MER) and PER, BML bed thickness, greatest diameter and greatest inner diameter. Surgery involved via pars plana vitrectomy with MER and internal limiting membrane peeling followed by SF6 internal tamponade. Results: Sixty-two eyes from 57 patients were included, with a mean follow-up of 27,1±19,8 months. Thirty-three presented PER, which conferred a greater BML diameter (p=0,001), thinner bed thickness (p=0,018), and smaller internal diameter / greatest diameter ratio (p=0,002). Twenty-seven (81,8%) BML with PER presented a tent configuration with an approach of its boundaries, comparing to 23,3% of those without PER. No differences were noted in visual outcomes (p=0,687) or BML closure (p=0,569) between eyes with and without PER. Conclusions: PER are recently described entities, which predict different BML configuration, with higher greatest diameters and thinner bed thickness. We did not find any difference on visual performance or closure rate between eyes with and without PER submitted to surgery.
