dc.description.abstract | Introduction: With the implementation of high-sensitivity cardiac troponins (Tn-HS) the diagnosis of unstable angina (UA) has declined markedly and the prognosis of these patients is still unknown. Methods: In a retrospective study, 103 patients admitted in the cardiac intensive care unit (CICU) diagnosed with UA with undetectable high-sensitivity troponins (< 0.017 µg/L) were followed approximately during 517 days, regarding the endpoints: mortality rate by all causes, acute myocardial infraction (AMI), cerebrovascular accident (CVA) and visits to the emergency department. Results: The medium age was 65±1 years, 68% were men, 65 were submitted to cardiac catheterization and of those 52% had significant lesions in the coronariography. Regarding the outcomes 3 (2.91%) died from non-cardiovascular or unknown causes, 3 (2.91%) had an AMI and there were no CVA. The event-free survival rate was 100% in 30 days, 98% in 180, 97% in 365 and 94% in 800 days. In the TIMI low-risk group the event-free survival rate was 95%, in the intermediate-risk 97% and in the high-risk 78%. We compared the groups: A (n=25, 24%) – without previous coronary heart disease and B (n=78, 76%) – with previous coronary heart disease. There were no differences in the medium age, Killip-Kimball classification and TIMI score between the groups. The survival and the event-free survival rates were 92 and 92% in the A group, and 99 and 95% in the B group.Conclusion: Patients with UA diagnosed with undetectable Tn-HS have an excellent prognosis. However the patients with high-risk TIMI score still have a higher mortality and AMI rate. Even in patients with previous coronary heart disease, negative Tn-HS provide a similar prognosis to patients without previous coronary disease. | eng |
dc.description.abstract | Introdução: Com a implementação das troponinas ultra-sensíveis (Tn-HS) na prática clínica, o diagnóstico de angina instável (AI) diminuiu consideravelmente e o prognóstico destes doentes ainda é pouco conhecido. Métodos: Conduzimos um estudo de coorte retrospectivo, incluindo 103 doentes consecutivamente internados numa Unidade de Cuidados Intensivos Cardíacos (UCIC) diagnosticados com AI com Tn-HS indetectáveis (< 0.017 µg/L). Foram analisados os seguintes desfechos, com um tempo de seguimento mediano de 517 dias: mortalidade por todas as causas, incidência de EAM, de AVC e recorrência ao SU por causas cardiovasculares. Resultados: A idade média dos doentes foi de 65±1 anos, 68% eram homens, 65 realizaram cateterismo e desses 52% apresentavam lesões significativas na coronariografia. Relativamente aos desfechos, 3 (2.91%) faleceram de causas não CV ou desconhecidas, 3 (2.91%) sofreram um EAM e não se registaram AVCs. A taxa de sobrevivência livre de eventos CV adversos foi de 100% em 30 dias, 98% em 180, 97% em 365 e 94% em 800 dias. No grupo com score TIMI de risco baixo a taxa de sobrevivência livre de eventos CV adversos foi de 95%, no de risco intermédio 97% e no de risco elevado 78%. Comparámos ainda doentes sem antecedentes de doença coronária (n=25, 24%) e com antecedentes (n=78, 76%). Não se encontraram diferenças significativas na idade média de cada grupo, na classificação Killip-Kimball ou no score TIMI. A taxa de sobrevivência e sobrevivência livre de eventos CV adversos foi de 92 e 92% no grupo sem doença coronária prévia e 99 e 95% no grupo com antecedentes de doença coronária, respectivamente.Conclusão: Os doentes com AI diagnosticada pela negatividade da Tn-HS têm um excelente prognóstico. Mesmo assim, os doentes com score TIMI de risco elevado têm maior taxa de mortalidade e EAM. Mesmo em doentes com antecedentes de doença coronária, a negatividade da Tn-HS confere um prognóstico semelhante aos doentes sem antecedentes de doença coronária. | por |
