Saúde sexual: conhecimentos e atitudes do estudante de medicina da Faculdade de Medicina da Universidade de Coimbra

Abstract

Introdução: De acordo com a Organização Mundial de Saúde, a saúde sexual é um estado de bem-estar físico, emocional, mental e social em relação à sexualidade, e não meramente a ausência de doença, disfunção ou enfermidade. Em Portugal, apesar de elevadas prevalências de problemas sexuais estes assuntos são muito pouco abordados entre o doente e o seu médico. Como o futuro dos cuidados de saúde, o enfoque na saúde sexual na formação médica é imprescindível para a criação de sociedades e indivíduos sexualmente saudáveis, sendo que o mesmo não acontece nas escolas médicas portuguesas.Objetivo: Avaliou-se a formação no âmbito da sexologia humana disponível aos alunos do 2º ciclo do Mestrado Integrado em Medicina da Faculdade de Medicina da Universidade de Coimbra (FMUC-MIM), avaliando os seus conhecimentos, perceções, atitudes, crenças e autoconfiança na abordagem da saúde sexual. Metodologia: Realizou-se um estudo observacional e descritivo com aplicação de um questionário simples, auto preenchido e anónimo, a estudantes do 2º Ciclo (4º, 5º e 6º anos) do MIM da FMUC, inquirindo sobre os dados do participante, participação prévia em atividades relacionadas com a sexologia humana, atitudes e crenças sobre a sexualidade humana (baseado no SABS desenvolvido por Reynolds e Magnan em 2005, traduzido e validado por Dias e Sim-Sim em 2011, conhecimentos da sexologia aplicada à medicina, contato com a sexologia durante o curso e a importância da mesma. Resultados: 186 respostas foram obtidas no total, revelando o nível de conhecimento presumido de 6.83 em 10 e 57.5% participou em atividades sobre sexualidade e saúde sexual. As atitudes e crenças sobre a sexualidade humana mostram-se favoráveis à boa prática médica. A compreensão presumida da disfunção sexual é de 5.52 em 10, tendo o inquérito de conhecimentos uma cotação de 70.5% de respostas corretas. 78% de alunos abordou a saúde sexual de um doente ao longo do curso, sendo os sentimentos de constrangimento e insegurança, assim como responsabilidade presentes. 84.9 % consideram importante a inclusão da Sexologia no currículo medico, sendo o grau de preparação para o futuro nesta área de 57%. Discussão e Conclusão: Os estudantes de medicina da FMUC referem estar subpreparados e desconfortáveis para atender adequadamente as necessidades de saúde sexual, insatisfeitos com a quantidade de educação nesta área, mesmo sendo as pontuações de conhecimento positivas. Existe uma necessidade de aumentar o entendimento da ligação entre a saúde e a sexualidade, melhorando a qualidade de vida dos doentes. Introduction: According to the World Health Organization, sexual health is a state of physical, emotional, mental and social well-being in terms of sexuality, and not merely the absence of disease, dysfunction or illness. In Portugal, despite high prevalence of sexual problems, these subjects are rarely approached between the patient and his doctor. As the future of health care, the focus on sexual health in medical training is essential for the creation of sexually healthy societies and individuals, and this does not happen in Portuguese medical schools.Aim: The human sexology education available to 2nd cycle students of the Integrated Master's Degree in Medicine at the Faculty of Medicine of the University of Coimbra (FMUC-MIM) was evaluated, assessing their knowledge, perceptions, attitudes, beliefs and self-confidence in the approach of sexual health.Methods: An observational and descriptive study was carried out with the application of a simple, self-completed and anonymous questionnaire to students of the 2nd Cycle (4th, 5th and 6th years) of the FMUC-MIM, inquiring about participant data, previous participation in human sexuality, attitudes and beliefs about human sexuality (based on the SABS developed by Reynolds and Magnan in 2005, translated and validated by Dias and Sim-Sim in 2011, knowledge of sexology applied to medicine, contact with sexology during the course and its importance in the medical curriculum.Results: 186 responses were obtained in total, revealing the presumed knowledge level of 6.83 in 10 and 57.5% participated in activities on sexual health. Attitudes and beliefs about human sexuality are favorable to a good medical practice. The presumed understanding of sexual dysfunction is 5.52 out of 10, being that there were 70.5% correct answers in the knowledge survey. 78% of students addressed the sexual health of a patient throughout the course, displaying feelings of embarrassment and insecurity, as well as responsibility. 84.9% consider important to include Sexology in the medical curriculum, and the degree of preparation for the future in this area is 57%.Discussion and Conclusion: FMUC medical students report being underprepared and uncomfortable to adequately address sexual health needs and are dissatisfied with the amount of education in this area, even though the knowledge scores are positive. There is a need to increase understanding of the link between health and sexuality, improving the quality of life of patients.Key-words: Sexual Health; Medical Education; Medical Students; Attitudes; Knowledge.
