dc.description.abstractIntrodução: A OMS define a obesidade como a epidemia do século XXI. O seu aumento na população infantil é preocupante, uma vez que 60% das crianças obesas serão adultos obesos. A hipótese de que o aleitamento materno tem um efeito protetor contra a obesidade infantil não é nova, porém, foram resultados controversos têm sido publicados. Os Açores registaram a menor taxa de aleitamento materno em Portugal e uma elevada prevalência de sobrepeso e obesidade infantil. Assim, torna-se importante perceber se esses fatos estão relacionados ou não.Objetivo: investigar a relação entre aleitamento materno e obesidade infantil numa população de crianças açorianas em idade escolar.Métodos: foi realizado um estudo observacional transversal com 183 crianças açorianas entre os 5 e os 10 anos de idade, que acederam às unidades de cuidados primários de saúde entre Setembro e Dezembro de 2016. O peso e a altura da criança foram medidos na consulta. Outras variáveis, como o tempo de gestação, diabetes gestacional, tabagismo durante a gravidez, peso ao nascer, amamentação, hábitos da criança, escolaridade e estado nutricional da mãe, foram investigadas através de um questionário. As curvas de percentil do IMC da OMS foram utilizadas para definir excesso de peso (85≤P<95) e obesidade infantil (P≥95). A associação entre o aleitamento materno e a obesidade infantil foi testada utilizando modelos de regressão logística.Resultados: A prevalência de excesso de peso foi de 15,9% e obesidade infantil foi de 23,5%. 74,3% das crianças foram amamentadas. A maioria das mães (61,2%) apresentou excesso de peso ou obesidade. A taxa de aleitamento materno exclusivo aos 3 meses foi de 44,3% e aos 6 meses 3,3%. O aleitamento materno complementar esteve presente em 39,3% aos 6 meses e 7,1% aos 2 anos. Em média, as crianças receberam leite materno em exclusivo por 9,2±10,54 semanas e amamentação complementar por 7,38±10,53 meses. As crianças obesas foram amamentadas menos tempo do que as crianças não obesas, sugerindo uma relação dose-efeito (p=0,037). Após análise multivariada, verificou-se uma relação significativa e independente entre obesidade infantil e aleitamento materno (rr=0,930; p=0,011), atividade física (rr=0,924; p=0,028) e estado nutricional materno (rr=2,937; p=0,009).Conclusão: A amamentação e a atividade física comportaram-se como fatores protetores da obesidade infantil, enquanto que o estado nutricional da mãe agiu como fator de risco. A obesidade infantil é um problema atual nos Açores e a amamentação pode ser uma ferramenta eficaz, simples, acessível e barata para reduzir a obesidade infantil.pt
dc.description.abstractIntroduction: WHO defines obesity as the 21st century epidemic. Its increase in the child population is worrisome, since 60% of obese children will be obese adults. The hypothesis that breastfeeding has a protective effect against childhood obesity is not new, however, controversial results have been published. The Azores reported the lowest rate of breastfeeding in Portugal and a high prevalence of overweight and childhood obesity. So, it becomes important to understand whether these facts are related or not.Objective: To investigate the relationship between breastfeeding and childhood obesity in a population of Azorean children of school age.Methods: A cross-sectional observational study was carried out with 183 Azorean children between 5 and 10 years old, who access primary health care units between September and December 2016. The weight and height of the child were measured at the consultation. Other variables (gestation time, gestational diabetes, smoking habits during pregnancy, birth weight, breastfeeding, child's habits, schooling and nutritional status of the mother) were investigated through a questionnaire. The WHO BMI percentile curves were used to define overweight (85≤P<95) and childhood obesity (P≥95). The association between breastfeeding and childhood obesity was tested using logistic regression models.Results: The prevalence of overweight was 15,9% and childhood obesity was 23,5%. 74,3% of the children were breastfed. The majority of mothers (61,2%) were overweight or obese. The exclusive breastfeeding rate at 3 months was 44,3% and at 6 months 3,3%. Complementary breastfeeding was present in 39,3% at 6 months and 7,1% at 2 years. On average, the children received exclusively breast milk for 9,2±10,54 weeks, and complementary breastfeeding for 7,38±10,53 months. Obese children were breastfed less time than non-obese children, suggesting a dose-effect relationship (p=0,037). After multivariate analysis, there was a significant and independent relationship between infant obesity and breastfeeding (relative risk=0,930; p=0,011), physical activity (relative risk=0,924; p=0,028) and maternal nutritional status (relative risk= 2,937; p=0,009). Conclusion: Breastfeeding and physical activity behaved as protective factors for childhood obesity, while the nutritional status of the mother acted like a risk factor. Childhood obesity is a current problem in the Azores and breastfeeding can be an effective, simple, affordable and inexpensive tool to reduce childhood obesity.pt
