Abstract: Introdução: A presença de boa circulação colateral pial (CCP) no doente com AVC isquémico (AVCi), submetido a terapêutica de revascularização arterial, associa-se a melhor prognóstico. Desconhece-se a totalidade de fatores que explicam a variabilidade interindividual da CCP. A sua identificação poderá contribuir para melhorar o tratamento do doente com AVCi agudo.Materiais e Métodos: Estudo observacional, de coorte histórica, que incluiu doentes internados no serviço de Neurologia do Centro Hospitalar e Universitário de Coimbra (CHUC), no período compreendido entre janeiro e dezembro de 2015, avaliados no contexto de Via Verde do AVC, com diagnóstico de AVCi da circulação anterior e oclusão de artéria de grande calibre da circulação anterior, que realizaram angiografia por tomografia computorizada (Angio-TC) nas primeiras 12 horas de evolução dos sintomas. A CCP foi classificada em três graus, de acordo com o sistema proposto por Miteff et all, e dicotomizada em boa CCP e CCP reduzida. Resultados: Dos 101 doentes incluídos, 64 (63.37 %) apresentaram CCP reduzida e 37 (36.63%) boa CCP. A população, onde 45.50% eram do sexo masculino, apresenta mediana de idades de 75 anos (Aiq de 15) e de NIHSS 18 (Aiq de 10). A análise univariada identificou associação entre a CCP e o valor de ASPECTS (p<0.001) e história prévia de AVC (p=0.043). Na análise multivariada apenas o valor de ASPECTS (OR, 1.435; IC, 1.058 a 1.946, p=0.020) se revelou preditor independente da CCP. Não se encontrou associação entre CCP e terapêutica prévia com estatinas ou diferentes classes de anti-hipertensores.Conclusão: Não foram encontrados fatores, passíveis de serem modificados, capazes de promover uma boa CCP no doente com AVCi agudo secundário a oclusão de artéria de grande calibre. Neste grupo de doentes, o valor de ASPECTS correlaciona-se com a CCP.
Introduction: Good pial collateral circulation (PCC) in patients undergoing arterial revascularization therapy after an acute ischemic stroke is associated with a better outcome. The totality of factors that explain PCC interindividual variability are unknown. Their identification may contribute to the improvement of acute stroke management.Materials and Methods: This observational historic cohort study included the patients admitted to Neurology department at Centro Hospitalar e Universitário de Coimbra, between January and December 2015, evaluated with the diagnose of anterior circulation acute ischemic stroke and occlusion of large artery from the anterior circulation, and submitted to CT angiography within the first 12 hours of symptoms. PCC was classified in tree degrees, according to the system proposed by Miteff et all, and dichotomized into good and reduced PCC.Results: From the 101 patients included, 64 (63.37 %) presented reduced PCC and 37 (36.63%) good PCC. The population, where 45.50% were males, presents a median age of 75 years (interquartile range of 15) and NIHSS 18 (interquartile range of 10). Univariate analysis identified association between PCC and ASPECTS (p<0.001) and history of previous stroke (p=0.043). Multivariate analysis shows ASPECTS (OR, 1.435; IC, 1.058 to 1.946, p=0.020) as an independent predictor of PCC. Association between PCC and previous treatment with statins or antihypertensives was not demonstrated.Conclusion: No modifiable factors were found capable of promoting good PCC in the patient with acute stroke, secondary to large caliber artery occlusion. In this group of patients, ASPECTS score correlates with PCC.
