dc.description.abstract | As complicações biliares (CB), nomeadamente as estenoses das vias biliares (EVB), constituem uma causa comum de morbilidade após transplante hepático. Este estudo retrospetivo tem como objetivo analisar diversos fatores que poderão estar envolvidos na etiologia das EVB.Realizado estudo a amostra de 126 transplantes, agrupada em casos (n=48) e controlos (n=78). O grupo de casos (CA) inclui todos os doentes com registo de estenose em relatórios clínicos, distinguida em estenose anastomótica (EA) e estenose não anastomótica (ENA). As ENA foram subdivididas em estenose exclusivamente não anastomótica (EENA) e estenose mista (EM). Os doentes que não desenvolveram estenoses foram incluídos no grupo de controlos (CO). Os dados dos grupos e variáveis referentes ao recetor, procedimento cirúrgico, dador e enxerto foram analisados através do software IBM SPSS Statistics 22. O estudo revelou uma incidência de EVB de 38,1%, uma incidência de 21,4% de EA (n=27) e 16,7% de ENA (n=21). As EENA (n=12) mostram incidência de 9,5% e EM (n=9) de 7,1% na amostra. O cirurgião C, responsável pela anastomose biliar, demonstrou um predomínio no grupo de CO (p=0,004). O tempo de isquémia quente (TIQ) 1 revelou-se superior nas EM (p=0,017). Verificou-se um predomínio de enxertos histologicamente normais nas EENA (p=0,024). As anastomoses biliodigestivas apenas se observaram no CO (p=0,023). Colangiografia percutânea transhepática (CPT) foi maioritariamente realizada nas EENA (p=0,007).Não se retiram conclusões sobre a influência das variáveis em estudo na etiologia das EVB após transplante hepático, mas obtiveram-se resultados de interesse relativamente à prevalência de carcinoma hepatocelular, estabilidade hemodinâmica, hemossiderose do enxerto e o grupo sanguíneo A na análise dos grupos e diferentes estenoses. Os resultados conseguidos analisam e expõem a experiência deste centro, permitindo uma avaliação retrospetiva com possíveis aplicações prospetivas. | por |

This retrospective study aims to analyze several factors that may be involved in the etiology of BS.A study was carried out on the sample of 126 transplants, grouped into cases (n=48) and controls (n=78). The case group (CA) included all patients with stenosis recorded in clinical reports, distinguished in anastomotic stenosis (AS) and non-anastomotic stenosis (NAS). NAS were subdivided into exclusively non-anastomotic stenosis (ENAS) and mixed stenosis (MS). Patients who did not develop stenoses were included in the control group (CO). Data from the groups and variables related to the recipient, surgical procedure, donor and graft were analyzed using the IBM SPSS Statistics 22 software.The study revealed an incidence of BS of 38.1%, an incidence of AS of 21.4% (n=27) and 16.7% of NAS (n=21). ENAS (n=12) showed a incidence of 9.5% and MS (n=9) of 7.1% in the sample. The surgeon C, responsible for the biliary anastomosis, demonstrated a predominance in the CO group (p=0.004). The warm ischemia time 1 (WIT) was higher in the MS (p=0.017). There was a predominance of histologically normal grafts in the ENAS (p=0.024). Bilodigestive anastomoses were only observed in the CO (p=0.023). Percutaneous Transhepatic Cholangiography (PTC) was mostly executed in ENAS (p=0.007).No conclusions were drawn about the influence of the variables of this study on the etiology of BS after liver transplantation, but results of interest were obtained regarding the prevalence of hepatocellular carcinoma, hemodynamic stability, hemosiderosis of the graft and type A blood in the analysis of groups and different stenoses. The results obtained analyze and expose the experience of this center, allowing a retrospective evaluation with possible prospective applications. | eng |
