dc.description.abstract | O padrão de repolarização precoce é uma entidade eletrocardiográfica recentemente redefinida pela presença de uma elevação do ponto J ≥ 0,1 milivolts em pelo menos duas derivações contíguas inferiores e/ou laterais, apresentando-se como um entalhe ou empastamento na parte terminal do complexo QRS.Ainda que a definição utilizada até 2008 fosse diferente, existia algum consenso na literatura sobre o facto do padrão de repolarização precoce ser uma variante clinicamente benigna, pelo que aquando da sua identificação, não seria necessário qualquer investigação adicional. No entanto, dados experimentais sugestivos de um potencial arritmogénico foram confirmados recentemente por diversos autores, que demonstraram uma ligação entre a presença deste achado eletrocardiográfico e morte súbita de origem cardíaca, aumentando assim o debate sobre o seu verdadeiro significado, tendo em conta que a base fisiopatológica para esta relação não esta completamente esclarecida. Por outro lado, vários estudos demonstraram que perante esta alteração relativamente comum, algumas características eletrocardiográficas podem ter um papel importante na estratificação do risco, para identificar os indivíduos que verdadeiramente devem ser acompanhados.A presente tese tem por objetivo primário elaborar uma revisão sistemática da literatura sobre a associação entre o padrão de repolarização precoce e a morte súbita cardíaca, abordando também aspetos relacionados com a sua fisiopatologia, demografia, semiologia, critérios de diagnóstico, prognóstico e implicações terapêuticas. | por |
dc.description.abstract | The early repolarization pattern is an electrocardiographic entity recently redefined by the presence of J point elevation ≥ 0,1 milivolts in at least two contiguous leads in the inferior and/or lateral distributions, either as a QRS notching or slurring at the end of QRS complex.Despite the definition used until 2008 was different, there was a consensus in the literature about its clinical benignity. Therefore when the early repolarization pattern was identified would not be necessary any additional screening. However, some experimental data suggesting an arrhythmogenic potential were recently confirmed by several authors. They have shown a link between early repolarization pattern and sudden cardiac death, increasing the debate about the true meaning of this finding, since its physiopathological basis is presently incompletely understood. On the other hand, knowing that this pattern is relatively common, other studies have revealed that some electrocardiographic features may have a role in the risk stratification, which would be used to identify the ones who really need follow-up.The aim of the present paper is to develop a systematic review of the literature about the association between the early repolarization pattern and sudden cardiac death, while analyzing its pathophysiology, demography, semiology, diagnostic workup, prognosis and therapeutic approach. The assessment of the quality of the major studies which focus on the principal matter of this document, the association between the early repolarization pattern and sudden cardiac death, was carried out with the Newcastle-Ottawa scale for case-control and cohort studies. | eng |
