Abstract: Background: Alpha 1-antitrypsin deficiency (A1ATD) is a genetic disease associated with the presence of pulmonary emphysema. Currently, computed tomography (CT) scan densitometry is recognized as the most sensitive method for detecting and quantifying emphysema. However, it has some limitations that preclude its use in the follow-up of patients.Aims:The aim of this study was to evaluate whether the extension of the emphysema determined by CT scan densitometry could be evaluated by the lung function tests (LFT) that have fewer limitations, and which lung function tests parameters best correlate with it. Methods: In this retrospective study, patients with A1ATD followed in the AIATD appointment of the CHUC-HG Pulmonology B Department that performed CT scan densitometry and LFT concomitantly were included. Correlations between the LFT parameters and total emphysema índex (EIt) assessed by CT scan densitometry were evaluated. A simple linear regression analysis between EIt and independent variables from LFT was performed as well as a multiple regression analysis between the same variables selected by a stepwise procedure. Data were analyzed using IBM SPSS Statistics version 24.Results: 65 patients were included (58.5% male; average age of 53.78 years 15.99) with multiple A1ATD phenotypes. The EIt ranged between 0 and 59.27%. From all variables of the LFT, FEV1/FVC had the highest correlation with the EIt ( r = - 0.797; p <0.001). The variables of the LFT which predicted the EIt selected by a stepwise procedure were DLCO and FEV1/FVC, explaining 75% of the variance (R2 = 0; p < 0,001).Conclusion: Several variables from the LFT strongly correlate with EIt assessed by CT scan densitometry. A considerable proportion of the EIt assessed by CT scan densitometry is explained by the multiple regression model reached in this study, that includes the LFT variables DLCO and FEV1/CVF.
Introdução:O défice de alfa 1- antitripsina (DA1AT) é uma doença genética que se associa à presença de enfisema pulmonar. A densitometria por tomografia computorizada (TC) é hoje em dia reconhecida como o método mais sensível para detetar e quantificar o enfisema. Tem, no entanto, algumas limitações que impedem o seu uso regular no seguimento dos doentes.Objetivos:O objetivo deste estudo foi avaliar se a extensão do enfisema determinada pela densitometria por TC poderá ser avaliada pelo estudo funcional respiratório (EFR) que tem menos limitações, e quais as variáveis do EFR que melhor se correlacionam com ele.Material e Métodos: Neste estudo retrospetivo, foram incluídos doentes com DA1AT seguidos na Consulta de DA1AT do Serviço de Pneumologia B do CHUC-HG que tivessem realizado densitometria por TC e EFR concomitantemente. Foram avaliadas as correlações entre vários parâmetros do EFR e o índice de enfisema total (IEt) determinado por densitometria por TC. Foi realizada uma análise de regressão linear simples entre o IEt e variáveis independentes do EFR, assim como, uma análise de regressão múltipla entre as mesmas variáveis selecionadas pelo procedimento stepwise. Os dados foram analisados usando IBM SPSS Statistics versão 24.Resultados: Incluíram-se 65 doentes (58.5% do sexo masculino; com média de idades de 53.78 anos 15.99) com vários fenótipos de DA1AT. O IEt variou entre 0 e 59.27%. De todas as variáveis do EFR analisadas, a FEV1/FVC foi a que mais se correlacionou com o IEt (r = - 0.797; p <0.001). As variáveis do EFR que predizem o IEt selecionadas pelo procedimento de stepwise foram a DLCO e o FEV1/FVC, explicando 75% da variância (R2 = 0; p < 0,001).Conclusão: Várias variáveis do EFR correlacionam-se fortemente com o IEt medido por densitometria por TC. Uma considerável proporção do IEt determinado por densitometria por TC é explicado pelo modelo de regressão múltipla encontrado neste estudo, que inclui as variáveis DLCO e FEV1/FVC do EFR.
