Tratamento Regenerativo da Peri-implantite com L-PRF – Revisão Sistemática e Casos Clínicos

Abstract

Introdução: A reabilitação oral com implantes não é isenta de riscos e por vezes está associada a complicações biológicas, sendo a mais destrutiva a peri-implantite. Ainda não há um tratamento regenerativo da peri-implantite previsível e consensual entre clínicos. A fibrina rica em plaquetas e leucócitos (L-PRF – Leucocyte and platelet rich fibrin) apresenta resultados favoráveis na correção de defeitos infra-ósseos. Objetivo: Neste trabalho foi realizada uma revisão sistemática sobre a utilização de L-PRF, sozinha ou associada a um xenoenxerto, no tratamento da peri-implantite. Em simultâneo apresentou-se uma série de casos onde foi utilizado L-PRF no tratamento desta patologia. Metodologia: Foi definida uma questão PICOT: “Em pacientes reabilitados com implantes, com diagnóstico de peri-implantite, qual a efetividade do tratamento cirúrgico regenerativo com L-PRF, sozinho ou combinado com xenoenxerto comparado com tratamento cirúrgico com xenoenxerto, com follow-up mínimo de 6 meses?”. Realizou-se uma pesquisa nas bases de dados PubMed, Cochrane e EMBASE com os termos “Platelet rich plasma [MeSH Terms]”, “Platelet rich fibrin”, “Plasma rich growth factors”, “Platelet concentrates”, “Leucocyte Platelet Rich Fibrin”, “Heterografts[MeSH Terms]”, “Xenografts”, “Peri-implantitis”, “Peri-implant” associados com os conectores boleanos “OR” e “AND”. Efetuou-se também uma pesquisa manual no “Journal of Clinical Periodontology” e no “Journal of Periodontology”. Para além disso, realizaram-se 5 casos clínicos para avaliar a aplicabilidade dos L-PRF no tratamento regenerativo da peri-implantite. Resultados: Foram incluídos 9 estudos clínicos onde foram avaliados os parâmetros Profundidade de sondagem (PS) e / ou Nível Clínico de inserção (NCI) e / ou Hemorragia à sondagem (HS) e / ou Recessão gengival (RG) e/ou Rx. Discussão/Conclusão: A utilização de L-PRF na cirurgia regenerativa em defeitos peri-implantares resultantes da peri-implantite ainda não está definida. Dos casos clínicos apresentados, devido ao reduzido tempo de follow-up, ainda não foi possível avaliar todos os parâmetros clínicos, no entanto, em 3 dos casos foi possível verificar um preenchimento ósseo radiográfico considerável, e em todos os casos uma ótima cicatrização dos tecidos moles. São, por isso, necessários mais estudos randomizados, com tempo de follow-up mais longos de forma a avaliar o potencial desta abordagem cirúrgica regenerativa.

Introduction: Oral rehabilitation with implants isn’t risk free and sometimes it is associated with biological complications, the most destructive being peri-implantitis. There is still no predictive and consensual regenerative treatment of peri-implantitis among clinicians. Leukocyte and platelet rich fibrin (L-PRF) presents favorable results in the correction of infra-bony defects. Objective: A systematic review of the use of L-PRF alone or associated with a xenograft in the treatment of peri-implantitis was performed. A cases series was presented where L-PRF was used in the treatment of this pathology.Methodology: A PICOT question was defined: "In implant-rehabilitated patients diagnosed with peri-implantitis, what is the effectiveness of regenerative surgical treatment with L-PRF alone or combined with xenografts compared to xenograft surgical treatment with minimum follow-up of 6 months?". An electronic search was accomplished in PubMed, Cochrane and EMBASE databases under the terms "Platelet rich plasma [MeSH Terms]", "Platelet rich fibrin", "Plasma rich growth factors", "Platelet concentrates", "Leucocyte Platelet Rich Fibrin", "Heterografts [MeSH Terms]", "Xenografts", "Peri-implantitis" and "Peri-implant" associated with the "OR" and "AND" boolean connectors. A manual survey was also conducted in the “Journal of Clinical Periodontology” and in the “Journal of Periodontology”. In addition, 5 clinical cases were performed to evaluate the applicability of L-PRF in the regenerative treatment of Peri-implantitis. Results: We included 9 clinical studies in which the parameters probing depth (PD) and / or Clinical attachment level (CAL) and / or Bleeding on probing (BOP) and / or Gingival Recession (GR) and / or Rx were evaluated. Discussion/Conclusion: The use of L-PRF in regenerative surgery of peri-implantitis resulting defects is not yet defined. From the clinical cases presented, and due to the short follow-up time, it was not possible to evaluate all the clinical parameters. However, in 3 of the cases it was possible to verify a considerable radiographic bone filling, and in all cases an excellent healing of the soft tissues. Therefore, more randomized trials with longer follow-up are needed to evaluate the potential of this regenerative surgical approach.
