Abstract: Introdução: Em Cuidados Paliativos (CP) o doente e a família são o foco do cuidado e devem ser integrados pela equipana planificação dos cuidados. Tal, impele a necessidade de apoio, de informação,de educação e de gestão de expetativas e emoçõespara que, possam compreender, aceitar e colaborar no processo de doença. Neste âmbito, surge a Conferência Familiar (CF) enquanto reunião estruturada, que promove a comunicação entre equipa, doente e família. Este estudo tem por objetivos:caracterizar a estrutura e o processo da CF nos diversos contextos de CP; identificar a satisfação que a CF produz nos seus intervenientes e descrever quais as variáveis que interferem nessa satisfação. Procura ainda responder às seguintes questões de investigação: quão eficaz é a CF na resolução dos objetivos para ela estipulados? E qual a satisfação que a CF promove nos seus intervenientes?Material e Métodos: Estudo observacional, analítico e transversal inserido no paradigma quantitativo. Fez uso de uma amostra não probabilística acidental de Doentes, Familiares de Referência e Profissionais de Saúde Gestores da CF. O estudo decorreu de 6-02-2017 a 06-04-2017 em unidades de CP, equipas Intra-hospitalar e domiciliária de CP. Foram aplicados como instrumentos a folha de “Caracterização da CF” e o questionário de “Avaliação da Satisfação com a CF", ambos elaborados pela investigadora, para o estudo.Resultados:Foram realizadas e analisadas 94 CF. No estudo participaram ativamente 83 Familiares de Referência, 93 profissionais e 24 doentes. Os doentes estiveram presentes em 36,2% das CFs e os cuidadores em 88%, os Familiares de Referência estiveram em 100% das CFs. Em 99% das CFs estiveram 2 ou mais profissionais de saúde, sendo que o Enfermeiro (98,9%) e o Médico (61,7%) foram os mais presentes. 97,2% das CFs foram planeadas e 90% demorou de 18 a 54 minutos. 98,2% dos objetivos traçados foram avaliados em satisfeitos e muito satisfeitos. As variáveis planeamento, tipologia de CP, categoria do profissional Gestor da CF, local se cuidados, grau de parentesco do Familiar de Referência e presença do doente tiveram influência significativano nível de satisfação dos participantes com a CF. O número de profissionais na CF, a duração da CF e o espaço onde se realizou a CF não mostraram essa influência.Discussão:As CF são altamente eficazes e satisfatórias na execução dos objetivos traçados. A estrutura e o processo da CF promovem satisfação elevada nos seus intervenientes, em todas as tipologias de CP. A família e o doente demonstram maior satisfação que os profissionais de saúde. A CF na equipa de suporte comunitário em CP mostrou promover maior satisfação que nas restantes tipologias. Os Enfermeirosforam os profissionais que estiveram mais presentes na CF e, enquanto Gestores de CF, os que mais satisfação promoveram nos restantes intervenientes, o que lhes legitima potencial nessa função. Conclusão: A Conferência Familiar como método de trabalho e de apoio a todos os intervenientes na prestação e na definição de cuidados a prestar, em todas as tipologias de Cuidados Paliativos mostrou sereficazquanto à clarificação e resolução dos objetivos definidos e opção de cuidados e muito satisfatória para doente, família e equipa de saúde quanto à comunicação e partilha de informação, na promoção da satisfação pessoal e profissional. Consideramos que a Conferência Familiar é um método potenciador da relação profissional de saúde/familiar/doente que poderá contribuir para otimizar a qualidade dos cuidados prestados, pelo que recomendamos a sua integração sempre que necessário mas de forma ordinária associada ao processo de admissão. IntroductionIn Palliative Care (PC) the patient and his/her family are the focus of care and should be integrated in the team to plan ministration. That impels the support needs, the information, the education and the expectation and emotion management, so that they can understand, accept and collaborate in the disease process. It is in this sphere that Familiar Conference (FC) arouses as a structured session, which promotes the team, patient and family communication. This study has as its goals characterizing the structure and the process of FC in the diverse contexts of PC; identifying the satisfaction that FC produces in its interveners and describing the variables that interfere in that satisfaction. It also expects to answer the following questions: How effective is FC in the resolution of its own goals? And what is the satisfaction that FC promotes in its interveners?Material and MethodsObservational, analytic and transversal study inserted in quantitative paradigm. It was used a non probabilistic random sample of Patients, Reference Relatives and Health Professionals that manage FC. This study occurred from 6-02-2017 to 06-04-2017 in PC units, intra-hospital teams and domiciliary care in PC.Results94 FCs were made and analyzed. In this study 83 Reference Relatives, 93 Professionals and 24 Patients were actively involved. The Patients were present in 36,2 % of FCs, Caregivers in 88% and the Reference Relatives in 100 % of FCs. In 99% of FCs 2 or more health professionals were present, being the nurse (98,9%) and the doctor (61,7%) the more usual. 97,2 of FCs were planned and 90% took between 18 to 54 minutes. 98,2% drawn objectives were evaluated as satisfactory and very satisfactory. The planning variable types of FCs, the rank of Health Professionals that manage FCs, the place of care, the affinity of the reference relative and the presence of the patient significantly influenced the level of satisfaction of participants with FCs. The number of professionals in FCs, the length of FCs and the space where it occurred did not show that influence.DiscussionThe FCs are extremely effective and satisfactory in the prosecution of drawn objectives. The structure and the process of FCs promote high satisfaction in its interveners in all types of FCs. The Family and the Patient demonstrate more satisfaction than Health Professionals. The FC in the team of community support revealed more satisfaction than in the remaining typologies. The nurses were the professionals who were more frequent in FCs and, while FC managers, the ones who promoted higher levels of satisfaction in the remaining interveners, which legitimizes them potential for this role.ConclusionThe Familiar Conference as a work and support method to all interveners in the provision and definition of care to be granted, in all typologies of Palliative Care revealed effectiveness in the clarification and resolution of traced objectives and the options of care. It was also felt as highly satisfactory for the patient, family and health team especially concerning communication and sharing of information and in the promotion of self and professional fulfillment. We consider the Familiar Conference an enhancer method of the professional relationship between health professionals, relatives and patients which can contribute to optimizing the quality of the care given, so we recommend its integration whenever necessary in an ordinary method associated to the admission process.Key words: Familiar Conference, Palliative Care, Satisfaction, Effectiveness
