anticoagulante crónica, no entanto, esta terapêutica está em mudança. Face às suas
limitações, decorrentes do início de ação, da janela terapêutica estreita, das múltiplas
interações com medicamentos e com alimentos, dos seus efeitos adversos e da
necessidade de monitorização, tornou-se emergente a pesquisa de fármacos que
superem estas dificuldades. Os novos anticoagulantes orais demonstram ser uma
alternativa eficaz e segura, sem necessidade de monitorização e com poucas interações.
Demonstrando-se igualmente eficazes em termos terapêuticos, os novos anticoagulantes
orais são cada vez mais recomendados pelos profissionais de saúde na prevenção e
tratamento de eventos tromboembólicos. No presente trabalho faz-se uma breve
análise farmacocinética e farmacodinâmica dos anticoagulantes orais, interacções e
reacções adversas, bem como da aplicabilidade de cada um. Faz-se ainda uma análise da
terapêutica anticoagulante instituída na região servida pela Farmácia Carlos Pereira
Lucas no período de Janeiro a Junho de 2015, assim como do papel do Farmacêutico no
acompanhamento e aconselhamento do doente sujeito a terapêutica anticoagulante. For over six decades warfarin has been the first choice in chronic anticoagulation treatment, however, this treatment is changing. Due to its limitations, from action initiation, narrow therapeutic window, multiple drug and food interactions, its adverse effects and monitoring need, there has been an emergent research of new drugs that overcome those problems. New oral anticoagulants have shown to be a more effective and safer alternative because of the lack of need for therapeutical monitoring and the fewer interactions. Showing the same therapeutical efficiency, the new oral anticoagulant drugs are being used by health professionals in preventing and treating thromboembolic events. This paper provides analysis of the pharmacokinetics and pharmacodynamics of oral anticoagulants, interactions and adverse effects and their applicability. It also considers the established anticoagulant therapies in the Carlos Pereira Lucas Pharmacy area between January to June of 2015, as well as the Pharmacist’s role in the monitoring and counseling of the patient during therapy. |
