Abstract: A Disfunção sexual feminina é a perturbação de uma das fases do ciclo da resposta sexual feminina, seja do desejo, da excitação ou do orgasmo ou a existência de dor associada à relação sexual, que causa mal-estar à mulher. A DSM-IV-TR classifica a Disfunção sexual feminina em seis categorias: Perturbações do desejo sexual (Desejo sexual hipoactivo e Aversão sexual), Perturbação da excitação sexual, Perturbação do orgasmo e Perturbações por Dor sexual. Este artigo pretende fazer uma revisão da literatura sobre a disfunção sexual feminina, que, apesar de lhe ser já atribuída elevada prevalência, se pensa estar sub-diagnosticada. Pretende-se fazer uma reflexão mais profunda sobre o Desejo sexual hipoactivo na mulher depois da menopausa e aferir se a reposição de androgénios pode ajudar no tratamento desta perturbação. O Desejo sexual hipoactivo é definido pela ausência ou diminuição do desejo sexual, pensamentos e /ou fantasias sexuais, de forma persistente ou recorrente, que causa mal-estar pessoal. Esta perturbação do desejo sexual é multifactorial, envolvendo uma complexa interacção entre componentes psicológicos, biológicos, socioeconómicos e interpessoais, sendo os factores psicossociais os que detém maior relevo. O papel dos androgénios na resposta sexual feminina ainda está em estudo. Parece que a sua acção a nível central - no núcleo pré-optico do hipotálamo, participa na libido e na manutenção do desejo sexual. Partindo de duas condições que cursam com hipoandrogenismo - a Doença de Addison e a Menopausa cirúrgica em mulheres jovens - é possível evidenciar o papel dos androgénios na Disfunção sexual feminina. Parece haver um grupo minoritário de mulheres com Desejo sexual hipoactivo nas quais não se consegue identificar uma causa psicossocial para esta disfunção. O défice de androgénios é aqui uma causa possível. Com a menopausa, devido a factores hormonais e relacionais, a disfunção sexual tem elevada prevalência, tendo também aqui o desejo sexual hipoactivo relevância epidemiológica. Vários estudos relacionam o desejo sexual e a testosterona na mulher pós-menopausica. Apesar da relação entre níveis de testosterona endógena e função sexual feminina não ser clara, a relação entre testosterona exógena e melhoria do desejo sexual parece ser evidente. Se é verdade que são necessários mais estudos nesta área, também é verdade que os estudos clínicos suportam a decisão de que o uso de testosterona em grupos criteriosamente seleccionados de mulheres pós-menopausicas, com Desejo sexual hipoactivo, após excluir todas as possíveis causas não atribuíveis ao défice de androgénios, é clinicamente razoável. Female sexual dysfunction is the disorder of one phase of the female sexual response, including the disorder related with the desire, orgasm, or the existence of painful intercourse that causes distress to women. DSM-IV-TR classifies Female sexual dysfunction into six categories: Sexual desire disorders (Hypoactive sexual desire disorder and Sexual Aversion disorder); Sexual arousal disorder; Orgasmic disorder and Pain disorders. This article is a review of the current scientific data regarding female sexual dysfunction. Despite its high prevalence, some suggest that this disorder is under diagnosed. The goal of this paper is to address specifically the Hypoactive sexual desire disorder after menopause. Furthermore, we wish to evaluate the role of androgen reposition on the treatment of this disorder. The Hypoactive sexual desire disorder is absence or deficiency of sexual fantasies, thoughts and/or desire for sexual activity, which causes personal distress. The causes vary considerably, and there´s a complex interplay among biological, psychological, socioeconomic and interpersonal components. Psychosocial factors play a major role in this disorder etiology. The role of the androgens on the sexual response is still in study. It seems that their action on a central level (preoptic hypothalamic nucleus) have a role on libido and maintenance of the sexual desire. Considering two medical conditions that cause hypoandrogenism (Addison disease and surgically menopausal young women) it’s possible to identify the role of androgens on Female sexual dysfunction It seems there is a minority group of women with Hypoactive sexual desire disorder which we are unable to indentify psychosocial components as causing factors. Androgen deficit is a plausible cause. In menopause, due to hormonal and relationship factors, sexual dysfunction has high prevalence and Hypoactive sexual desire disorder as also epidemiological relevance. Several trials show a connection between sexual desire and testosterone in postmenopause women. Even though the relation between endogenous testosterone levels and female sexual function is not that clear, it seems obvious that there is an improvement of sexual desire using exogenous testosterone. Despite the fact there is a need for more trials in this area, the scientific evidence supports the use of testosterone. It is clinically reasonable to do so only after excluding causes not attributed to androgen deficit disorders and after choosing carefully the groups of post-menopause women with Hypoactive sexual desire disorder. The current knowledge in this area is insufficient. Therefore sexual dysfunction research should continue regarding both safety and efficacy of androgen replacement.
