Title: Refluxo vesico-uretérico na criança : clínica, diagnóstico e tratamento

Abstract: O refluxo vesico-uretérico consiste no fluxo retrógrado de urina da bexiga para o tracto urinário superior. Está presente em 1-2% das crianças, ocorrendo em 25-40% daquelas com infecção do tracto urinário superior e em 17-38% das que são diagnosticas com hidronefrose no período pré-natal. Enquanto o refluxo vesico-uretérico primário é causado por uma anomalia congénita da junção vesico-uretérica com uma base genética, o secundário é causado por obstrução vesical anatómica ou funcional com subsequente elevação da pressão intravesical. É classificado em 5 graus, segundo o International Reflux Study Committee, de acordo com a extensão do fluxo retrógrado e as alterações morfológicas do tracto urinário superior detectadas por cistouretrografia miccional. Um dos maiores desafios à abordagem do refluxo vesico-uretérico prende-se com a definição de crianças susceptíveis de beneficiar do diagnóstico e tratamento. Não existe consenso quanto ao algoritmo de investigação após a infecção urinária nas crianças e a utilidade de rastrear lactentes com hidronefrose pré-natal de baixo grau e de irmãos e descendentes directos assintomáticos permanece sob questão. Os principais objectivos do tratamento são prevenir a pielonefrite e a nefropatia de refluxo. As alternativas são a observação evolutiva, profilaxia antibiótica e correcção cirúrgica (aberta, endoscópica, laparoscópica ou robótica). Este artigo de revisão pretende abordar sumariamente a etiologia, fisiopatologia, classificação e espectro clínico do refluxo vesico-uretérico, e apresentar as recomendações actuais na avaliação inicial, seguimento e tratamento do refluxo vesico uretérico primário, comparando a eficácia e eventuais complicações da cirurgia aberta, endoscópica, laparoscópica e robótica
Vesicoureteral Reflux consists in the back-flow of urine from the bladder into the upper urinary tract. It is present in 1-2% of all children, while it occurs in 25-40% of children with urinary tract infections and is responsible for 17-38% of those prenatally diagnosed with hydronephrosis. While primary vesicoureteral reflux is a congenital anomaly of the ureterovesical junction with genetic basis, secondary reflux is caused by an anatomical or functional bladder outflow obstruction with subsequently elevated pressure or inflammatory disease. It is classified into 5 degrees by the International Reflux Study Committee, based on the extension of retrograde flow and morphological alterations of the upper urinary tract detected on voiding cystourethrography. The clinical presentation depends on the age, associated complications and underlying diseases. One of the most challenges to the advancement of vesicoureteral reflux management is the definition of children likely to benefit from diagnosis and treatment. There is no consensus about the investigation algorithm after a urinary tract infection in children and the utility of screening infants with low-grade prenatal hydronephrosis and asymptomatic sibling and offspring for reflux remains questionable. The main treatment goals are the prevention of pyelonephritis and reflux nephropathy. Treatment alternatives include observation, antibiotic prophylaxis and surgical correction (open, endoscopic, laparoscopic or robotic). This review aims to summarily address the etiology, pathophysiology, classification and clinical spectrum of vesicoureteral reflux, and present actual recommendations on the initial evaluation, followup and treatment of primary vesicoureteral reflux, comparing the efficacy and eventual complications of open surgery, endoscopic injection and, more recently, laparoscopic and robotic surgery
