A área da “Traumatologia Renal” reveste-se de especial interesse na sociedade actual,
na qual se tem registado um elevado número de vítimas por traumatismo, comprometendo o
rim em 1.13 - 2.8% do total dos casos no adulto.
Pretende-se fazer uma revisão bibliográfica dos artigos científicos publicados que
tratam do tema “Traumatologia Renal” e abordar algoritmos de reposta a uma situação de
traumatismo renal no adulto. Objectiva-se ainda, apresentar uma perspectiva da casuística do
Serviço de Urologia e Transplantação Renal dos Hospitais da Universidade de Coimbra nesta
área.
Material e Métodos:
Identificou-se e reviu-se a literatura em língua Inglesa, acerca do tema “Traumatologia
Renal”, através do motor de busca da PubMed.
O estudo da casuística do Serviço de Urologia e Transplantação Renal teve por base a
pesquisa e análise de processos clínicos, referentes a 114 doentes, admitidos no período de
15.12.1999 – 05.12.2009. Os dados recolhidos foram trabalhados com o software PASW
Statistics 18.0.
Resultados:
Os traumatismos renais classificam-se segundo o mecanismo de lesão em abertos ou
fechados e ocorrem em aproximadamente 10% e 90% dos casos, respectivamente. A escala de
classificação imagiológica mais utilizada é Escala de Severidade Lesional para os
Traumatismos Renais da American Association for the Surgery of Trauma; apresenta 5 graus
lesionais de gravidade crescente que se correlacionam com a necessidade de exploração
cirúrgica e prognóstico. O diagnóstico deve ser sugerido pela anamnese e exame objectivo,
dependendo a conduta terapêutica primária do estado geral do doente.
A terapêutica não cirúrgica é largamente a mais utilizada, apresentando crescente
adopção. Contudo, a terapêutica cirúrgica ainda tem indicação absoluta em casos de avulsão
do pedículo renal, hemorragia persistente com choque hipovolémico e hematoma
retroperitoneal incontido ou pulsátil.
A análise da série dos Hospitais da Universidade de Coimbra revelou uma percentagem
de explorações renais de 2%, 0%, 12.5%, 52.6% e 83.3%, e de nefrectomias de 0%, 0%, 0%,
47.4% e 83.3%, do grau I ao grau V respectivamente.
Conclusões:
Apesar de existirem guidlines incidentes na “Traumatologia Renal”, o seu seguimento
não é unânime, tendo cada centro hospitalar protocolos específicos, dependentes da logística
disponível e da experiência e competências da equipa multidisciplinar. Certa é a capacidade
crescente em tratar estes doentes de modo conservador, poupando-os às complicações
inerentes à cirurgia e diminuindo custos hospitalares.
Publicações futuras serão necessárias para redefinir a Escala de Severidade Lesional
para os Traumatismos Renais da American Association for the Surgery of Trauma, para
comparar prognósticos entre terapêutica cirúrgica, conservadora e angiográfica dos graus IV e
V, e para definir com maior certeza as indicações para seguimento imagiológico dos rins pós
traumáticos. Por estudar, está ainda o papel da antibioterapia profilática durante a terapêutica
conservadora Introduction and Objectives: Renal Trauma is an area of particular interest in nowadays’ society, where there has been a high number of trauma victims, compromising the kidney in 1.13 – 2.8% of all adult cases. It’s intend to do a literature review of published scientific papers that address the theme "Renal Trauma", and mentioning the management algorithms to a situation of renal trauma in adults. It’s also an objective of this work, to present a perspective of the “Serviço de Urologia e Transplantação Renal dos Hospitais da Universidade de Coimbra” case series in the area. Material and Methods It was identified and reviewed the English-language literature, on the topic "Kidney Trauma", through the PubMed search engine. The study of the case records of “Serviço de Urologia e Transplantação Renal” was based on the research and analysis of clinical processes, relating to 114 patients admitted between 15.12.1999 - 05.12.2009. The data collected were processed with SPSS Statistics 18.0 software. Results According to the mechanism of injury, renal injuries are classified in penetrating or blunt and occur in approximately 10% and 90% of cases, respectively. The most used rating imaging scale is the American Association for the Surgery of Trauma Organ Injury Severity Scale for the Kidney; it features 5 lesional degrees of increasing severity that are correlated with the need of exploratory surgery and prognosis. The diagnosis should be suggested by clinical history and physical examination, depending, the primary therapy, of patient’s global conditions. The non-surgical management is used most widely, showing increasing adoption. However, surgical management still is absolutely indicated in cases of avulsion of the renal pedicle, persistent bleeding with shock and pulsatile or uncontained retroperitoneal haematoma. The analysis of the series of the “Hospitais da Universidade de Coimbra” revealed a renal exploration’s percentage of 2%, 0%, 12.5%, 52.6% and 83.3%, and a nephrectomies’ percentage of 0%, 0%, 0%, 47.4% and 83.3% of grade I to grade V respectively. Conclusions Despite there are guidlines incidents in the "Renal Trauma", his following is not unanimous, using each hospital specific protocols, dependents on available logistics and the experience and expertise of multidisciplinary team. Is certain the increasing capacity to treat these patients conservatively, saving them to the complications inherent to surgery and reducing hospital costs. Future publications are required to redefine the American Association for the Surgery of Trauma staging classification, to compare the outcome between operative management, conservative management and angiographic management of grade IV and V, and to set indications for kidney injuries’ imaging follow-up. The value of antibiotic administration during conservative management is still in need of further investigation. |
