é uma importante causa de mortalidade e morbilidade em todo o Mundo sendo, actualmente, a
angioplastia primária o tratamento preferencial.
Objectivos: Fazer uma revisão bibliográfica relativa ao papel da Angioplastia Primária
no tratamento do Enfarte Agudo do Miocárdio com Supra ST.
Desenvolvimento: O enfarte agudo do miocárdio foi em 2004 a terceira principal causa
de morte em Portugal. É maioritariamente causado pela oclusão súbita de uma artéria
coronária major após disrupção de uma placa aterosclerótica. A reperfusão pode salvar o
tecido miocárdico em risco de necrose, com melhoria da função ventricular esquerda e
sobrevivência. Pode ser efectuada com recurso a fibrinólise, angioplastia primária ou a
cirurgia de bypass coronário. A dilatação da artéria coronária através da angioplastia provoca
alterações morfológicas no vaso e, sendo um procedimento invasivo, tem várias complicações
associadas. Ao longo do tempo, este procedimento tem evoluído consideravelmente, com o
desenvolvimento de novos aparelhos para melhorar os resultados obtidos. Dentro destes
destacam-se os stents, que demonstraram estar associados a diminuição da taxa de
mortalidade e de complicações a longo termo. A única contra-indicação absoluta da
angioplastia primária é a recusa por parte do doente. Muitos estudos demonstraram a
superioridade da angioplastia primária face à fibrinólise com redução da mortalidade,
complicações e melhoria da sobrevivência. A eficácia da reperfusão relaciona-se com a
rapidez com que é realizada. Assim, tem sido debatido qual o tempo porta-balão aceitável,
hoje considerado duas horas pela Sociedade Europeia de Cardiologia, bem como com o tempo
de evolução, sendo consensuais os benefícios da reperfusão até às doze horas. Actualmente, a
angioplastia primária é considerada o tratamento preferencial nestes doentes se realizada em
tempo útil, sendo recomendada a implementação de sistemas de emergência que permitam a
optimização do tratamento do enfarte agudo do miocárdio com supradesnivelamento do
segmento ST.
Conclusão: Cerca de três décadas de progresso baseado na evidência têm demonstrado
a eficácia da angioplastia primária, pelo que hoje constitui a terapêutica de reperfusão
preferencial no enfarte agudo do miocárdio com supradesnivelamento do segmento ST. Introduction: Acute myocardial infarction with ST-segment elevation is a major cause of death and disability worldwide and primary angioplasty is the main treatment in these patients nowadays. Objectives: Make a bibliographical review of the primary angioplasty’s role in the treatment of acute myocardial infarction with ST-segment elevation. Development: In 2004, myocardial infarction was the third major cause of death in Portugal. It is mainly caused by sudden thrombotic occlusion of a major coronary artery caused by disruption of an atherosclerotic plaque. Reperfusion therapy may salvage myocardium at risk of necrosis, with improvement of left ventricular function and survival. It can be done by fibrinolytic therapy, primary angioplasty or coronary artery bypass graft. The enlargement of the coronary artery lumen by angioplasty causes morphological changes of the vessel and, since it is an invasive procedure, it has several complications associated. This procedure has undergone major advances with development of new devices to improve the outcomes. Among them, coronary stents stand out, with decreased rate of mortality and complications in long term. Denial of the patient is the only absolute contraindication to primary angioplasty. Many studies have demonstrated the superiority of primary angioplasty over thrombolysis, with reduced mortality, complications and improved survival. The beneficial effects of reperfusion therapy are greater when performed soon after presentation. Therefore, it has been discussed the acceptable door-to-balloon time, which is now considered two hours by the European Society of Cardiology, as well the duration of symptoms, with recognition of reperfusion benefits until twelve hours. Today, primary angioplasty is considered the reperfusion therapy of choice in most patients if performed in timely fashion and the implementation of a system of care network is recommended to allow the optimization of acute myocardial infarction with ST-segment elevation treatment. Conclusion: Almost three decades of progress based on evidence have demonstrated the efficiency of primary angioplasty making it, nowadays, the preferable reperfusion therapy in acute myocardial infarction with ST-segment elevation. |
