Abstract: A insuficiência cardíaca (IC) com função sistólica preservada (ICFSP) representa uma síndrome clínica caracterizada por sinais e/ou sintomas de IC e uma fracção de ejecção do ventrículo esquerdo (FEVE) preservada (> 40% ou 50%), que no presente é responsável por cerca de metade dos casos de IC. Este artigo pretende fazer uma revisão da literatura sobre a epidemiologia, fisiopatologia, diagnóstico, tratamento e prognóstico da ICFSP. A ICFSP está associada a idades mais avançadas, género feminino, obesidade, hipertensão arterial e fibrilhação auricular. A atenção por parte da comunidade médica para este tipo de IC é relativamente recente, daí que o conhecimento sobre a fisiopatologia seja ainda escasso. Atribuí-se o mecanismo principal à disfunção diastólica, que significa um conjunto de alterações ao nível do relaxamento e da elastância do ventrículo esquerdo (VE). Existem também alguns estudos que apontam para a existência de alterações ao nível da função sistólica e do acoplamento ventrículo-arterial. Durante algum tempo os critérios de diagnóstico da ICFSP foram alvo de discussão. Em 2007, foi proposto um algoritmo de diagnóstico pela Sociedade Europeia de Cardiologia (SEC), segundo o qual seria necessário que se verificassem três condições para o diagnóstico de ICFSP: sinais e/ou sintomas de IC, função sistólica do VE normal ou ligeiramente deprimida e evidência de disfunção diastólica do VE. A investigação diagnóstica de um doente com suspeita de ICFSP implica, por isso, o recurso a métodos como ecocardiografia com doppler, doppler tecidular, doseamento de biomarcadores cardíacos ou mesmo cateterismo cardíaco. Assim como para a IC com fracção de ejecção diminuída (ICFED), o tratamento da ICFSP deve contemplar uma intervenção no estilo de vida a par de medidas farmacológicas. A este nível, existe ainda pouca evidência sobre os fármacos com maior beneficio em doentes com ICFSP. Segundo os ensaios mais recentes, o antagonista do receptor da angiotensina II (ARAII) candesartan mostrou algum benefício quando usado em doentes com ICFSP. A utilidade de bloqueadores adrenérgicos β e de digitálicos permanece pouco clara. Actualmente, a única orientação classe I nível de evidência A para o tratamento da ICFSP é o controlo da pressão arterial sistólica e diastólica. O prognóstico da ICFSP e da ICFED parece ser semelhante, com taxas de morbilidade e mortalidade comparáveis, segundo os estudos mais actuais. No entanto, ao contrário da ICFED, na ICFSP, registam-se mais mortes por causas não cardiovasculares. O conhecimento actual sobre a fisiopatologia e o tratamento da ICFSP é insuficiente. A investigação deverá continuar, procurando identificar os seus mecanismos principais e possíveis alvos terapêuticos.
Heart failure (HF) with preserved systolic function (HFPSF) is a syndrome defined as the presence of symptoms and/or signs of HF and a preserved left ventricle ejection fraction (LVEF) (> 40%) and occurs in half of the patients with HF. The aim of the present review is to summarize available data on epidemiology, pathophysiology, diagnosis, treatment and prognosis of HFPSF. Patients with HFPSF are usually older, more often women and more likely to have a history of obesity, hypertension and atrial fibrillation. There is still debate regarding the pathophysiology of the syndrome. Diastolic dysfunction refers to abnormalities in both LV relaxation and LV stiffness and is thought to be the key. Several studies found some abnormalities concerning the systolic function and ventriculovascular coupling in HFPSF. The diagnosis of HSPSF has been a matter of discussion for many years. In 2007, the European Society of Cardiology (ESC), proposed a new diagnostic algorithm. According to ESC, three conditions must be fulfilled for the diagnosis of HFPSF: signs or symptoms of HF, normal or mildly abnormal LV systolic function and evidence of diastolic LV dysfunction. Thus, the management of a patient with suspected HFPSF usually require exams like doppler echocardiography, tissue doppler, HF biomarkers or even cardiac catheterization. The treatment of HFPSF, as for the HF with reduced ejection fraction (HFREF), includes both life style interventions and pharmacological strategies. There are still few trials performed in patients with HFPSF. According to recent studies, the angiotensin receptor blocker candesartan showed favourable effects in HFPSF. The benefits of β blockers and digitalis in patients with HFPSF have not been convincingly demonstrated. The only recommendation for the treatment of HFPSF class I level of evidence A is blood pressure control. The prognosis of HSPSF with respect to morbidity and mortality appears to be similar to that of HFREF although cardiovascular mortality is more frequent in this last group of patients, as patients with HFPSF have a higher noncardiovascular mortality. Current knowledge about pathophysiology and treatment of HFPSF is sparse. Further investigation in this field should proceed, so that possible future treatment strategies can be developed, in order to improve patients prognosis
