importante problema de saúde, por apresentar risco significativo de morbilidade e mortalidade
sendo uma das principais causas de morte prevenível em doentes hospitalizados.
Objectivo: Fazer uma revisão bibliográfica relativa à estratificação de risco e
terapêutica da tromboembolia pulmonar.
Desenvolvimento: A tromboembolia pulmonar é uma apresentação grave do
tromboembolismo venoso com taxas de mortalidade elevada nos doentes não tratados. A
estratificação do risco é fulcral como auxiliar no acompanhamento dos doentes com
tromboembolismo pulmonar. Baseia-se inicialmente no equilíbrio hemodinâmico do doente
(estável ou instável), e depois na clínica, marcadores de disfunção ventricular direita e
marcadores de sofrimento miocárdico. Existem ainda outros índices baseados na clínica para
estratificar os doentes com tromboembolia pulmonar – o índice de gravidade do embolismo
pulmonar e a estratificação por oximetria de pulso. A terapêutica e os exames
complementares realizados a cada doente dependem da classe de risco que lhe foi atribuída. A
instituição precoce do tratamento adequado diminui consideravelmente a taxa de mortalidade
nestes doentes.
Conclusão: A estratificação do risco na tromboembolia pulmonar deve ser feita
precocemente, sendo fundamental na escolha da terapêutica mais adequada e atempada, o que
melhora a sobrevida dos pacientes. Actualmente, o tratamento da tromboembolia pulmonar
consiste essencialmente na administração de heparina, heparina de baixo peso molecular ou
fondaparinux, concomitantemente ou seguida por antagonistas orais da vitamina K por um
mínimo de 3 meses. Introduction: Pulmonary embolism is a medical emergency and an important health problem by presenting a high risk of morbidity and mortality, being a major cause of preventable death in hospitalized patients. Objectivo: To review the literature on risk stratification and treatment of pulmonary embolism. Development: Pulmonary embolism is a serious presentation of venous thromboembolism with high mortality rates in untreated patients. Risk stratification is crucial as an aid in the management of patients with pulmonary embolism. It is based initially on the patient's hemodynamic equilibrium (stable or unstable), and then in clinical markers of right ventricular dysfunction and markers of myocardial injury. There are other indexes based on the clinic to stratify patients with pulmonary embolism - the index of severity of pulmonary embolism and stratification by pulse oximetry. Therapy and tests performed for each patient depend on the risk class assigned to them. Early initiation of appropriate treatment significantly reduced the mortality rate in these patients. Conclusion: Risk stratification in pulmonary embolism should be done early, and is crucial when choosing the most appropriate and timely therapy, which improves patient survival. Currently, the treatment of pulmonary thromboembolism essentially consists in the administration of heparin, low molecular weight or fondaparinux simultaneously or subsequently followed by oral vitamin K antagonists for at least 3 months. |
