Title: Avaliação clínica da displasia de desenvolvimento da anca e sua validade para referenciação ecográfica

Abstract: Introdução: A displasia do desenvolvimento da anca engloba um amplo espectro de alterações da articulação coxo-femoral em crescimento. Em Portugal não há dados rigorosos da incidência desta patologia. Estima-se que seja 5 por 1000 nascimentos, aumentando para 20 a 25 por 1000 nascimentos quando contabilizados os casos de instabilidade articular. O diagnóstico precoce é fator prognóstico e decisivo no resultado final. As alterações no exame clínico neonatal e as observações sistemáticas nos primeiros meses de vida, enquadradas em fatores de risco conhecidos, constituem indicação para a realização do exame ecográfico basilar no diagnóstico precoce desta doença. O Serviço de Ortopedia Infantil do Hospital Pediátrico de Coimbra tem uma vasta experiência no tratamento desta patologia. Assim torna-se pertinente correlacionar dados da avaliação clínica e imagiológica, de forma a inferir dados específicos desta patologia que permitam uma melhor articulação entre Pediatras, Médicos de Família e Ortopedistas. Objetivos: Avaliação comparativa dos elementos do exame clínico e ecográfico. Análise da especificidade e sensibilidade dos dados do exame físico, de forma a estabelecer a sua validade enquanto critérios de referenciação e indicação para exame ecográfico. Materiais e Métodos: Análise prospetiva de uma amostra de crianças enviadas à Consulta de Ortopedia Infantil do Hospital Pediátrico de Coimbra por suspeita de Displasia do Desenvolvimento da Anca. Correlação entre motivo de referenciação hospitalar, alterações no exame físico, fatores de risco e a avaliação ecográfica objetivada pela classificação de Graf. Análise estatística de forma a determinar a significância e importância relativa destes elementos no espectro da Doença Displásica da Anca. A revisão da literatura foi feita pelo pubmed e o banco de dados inclui os seguintes termos de pesquisa: displasia do desenvolvimento da anca, fatores de risco, avaliação ecográfico e classificação de Graf. Inclui ainda livros de texto e referências online. Resultados: No grupo estudado, a amostra é constituída por 28 crianças com idades cronológicas entre 1 mês e 7 meses de idade. Há um predomínio do sexo feminino (n=19 crianças). Os motivos de referenciação para consulta de Ortopedia Infantil foram variados, entre os quais destaca-se a instabilidade articular (n=10) e a limitação da abdução (n=6). Os fatores de risco identificados nesta amostra de crianças foram: primiparidade, apresentação pélvica, instabilidade neonatal, história familiar e outras condições associadas como torcicolo congénito. Ao exame físico as manobras realizadas foram: Ortolani/Barlow, Abdução, Galeazzi, Telescopagem, Sinal das Pregas Inguinais e das Pregas das Coxas. A correlação das manobras do exame físico com o resultado ecográfico revelou que há uma relação estatisticamente significativa entre a manobra de abdução e o resultado ecográfico com p=0,004, assim como entre o sinal de pregas inguinais e o resultado ecográfico com p=0,001. As manobras de abdução e o sinal de pregas inguinais permitem uma redução na incerteza da classificação do resultado ecográfico de 67,0%. O modelo apresenta elevada sensibilidade e boa especificidade. Conclusões: Não há um modelo ideal no rastreio clínico e ecográfico, devendo-se dar especial atenção à manobra de abdução e à avaliação das pregas inguinais na criança dado o seu valor de sensibilidade e especificidade. Pode-se afirmar que as manobras de limitação da abdução e o sinal de assimetria das pregas inguinais quando conjugadas apresentam validade como critérios para diagnóstico de DDA.
Introduction: The developmental dysplasia of the hip involves a wide spectrum of alterations of the growing hip joint. In Portugal there are no rigorous data about the incidence of this pathology. We can estimate that developmental dysplasia of the hip affects 5 per 1000 births, increasing to 20/25 per 1000 births when counting the cases of joint instability. The early diagnosis is a decisive prognostic factor in the final result. The alterations in the neonatal exam and the systematic observations during the first months of life, framed in well-known risk factors, are a strong indication to a basilar ultrasonography exam in the precocious diagnosis of this disease. The Department of Orthopaedics of the Hospital Pediátrico de Coimbra has a wide experience in the treatment of this pathology. So it is relevant to correlate data from the clinical and ultrasound evaluation as a way to infer specific data of this pathology in order to allow a better articulation between Pediatricians, General Practitioners and Orthopedists. Objectives: Comparative evaluation of the elements from the clinical exam and ultrasonography. Analysis of the specificity and sensibility of the data from the physical exam, in order to establish its validity as a referral criteria and indication to the ultrasonography. Materials and methods: The prospective analysis of the sample of children studied sent to an appointment in the Infant Orthopedics Service from the Hospital Pediátrico de Coimbra with suspect of developmental dysplasia of the hip. Correlation to the reason of hospital referral, alterations on the physical exam, risk factors and ultrasound evaluation as per Graf classification. Statistical analysis in order to determine its significance and relative importance of these elements in the spectrum of the dysplastic hip disease. The literature review was made in pubmed and the data bank includes the following research words: developmental dysplasia of the hip, risk factors, ultrasonography evaluation and Graf classification. Also adds textbooks and online references. Results: In the studied group, the sample is composed by 28 children between the ages of 1 month to 7 months. The predominant sex is the female (n=19 children). The motives for the clinical referral to a medical appointment in Orthopedics were varied. Among the motives we can point the articular instability (n=10) and the limitation of the abduction (n=6). The risk factors identified in this sample of children were: primiparity, pelvic presentation, neonatal instability, family history, among other conditions like congenital torticollis. During the physical exam the maneuvers were: Ortolani/Barlow, Abduction, Galeazzi, Telescoping, Sign of Inguinal folds and Thigh folds. The correlation of the maneuvers from the physical exam and the ultrasound result revealed that there is a statistical significant relation between the abduction maneuver and the ultrasound result with p=0.004, as well as between the sign of inguinal folds and the ultrasound result with p=0,001. The abduction maneuvers and the sign of inguinal folds allow the reduction of the uncertainty of the classification of the ultrasound exam of 67,0%. The model presents high sensibility and good specificity. Conclusions: There isn´t an ideal model in the clinical and ultrasound screening and we should mind special attention to the abduction maneuver and to the evaluation of the inguinal folds in the child due to its value of sensibility and specificity. We can assert that the maneuvers of the limitation of abduction and the asymmetry of the sigh of inguinal folds when combined present validity as diagnostic criteria of Development Dysplasia of the Hip.
