Traumatologia orofacial na criança com perturbação de hiperatividade e défice de Atenção (PHDA)

Abstract

Introdução: A Perturbação de Hiperatividade e Défice de Atenção é um distúrbio do neurodesenvolvimento que se carateriza por um conjunto de comportamentos disruptivos como a desatenção, impulsividade e agitação motora. Esta perturbação surge descrita na literatura enquanto fator predisponente para a ocorrência de traumatismos orofaciais, nomeadamente traumatismos dentários, uma vez que associado à sintomatologia do distúrbio estes indivíduos tendem a colocar-se em situações perigosas, assumindo comportamentos irrefletidos e impulsivos. Objetivo: Caraterizar a ocorrência de traumatismos orofaciais numa amostra de crianças e jovens seguidos na consulta de hiperatividade do Centro de Desenvolvimento Luís Borges do Hospital Pediátrico de Coimbra – CHUC, EPE, complementada com uma revisão da literatura atual que retrata esta problemática, possibilitando deste modo, amplificar o conhecimento existente sobre a saúde oral destas crianças. Materiais e métodos: Foram observados 50 crianças e jovens seguidos na Consulta de Hiperatividade do Centro de Desenvolvimento Luís Borges do Hospital Pediátrico de Coimbra – Centro Hospitalar e Universitário de Coimbra, EPE, entre os meses de dezembro de 2015 e maio de 2016, procurando estudar a prevalência de traumatologia orofacial, segundo os critérios de diagnóstico da World Health Organization, cumprindo os princípios e requisitos éticos. Complementarmente, efetuou-se uma pesquisa na PubMed/MEDLINE e EBSCOhost, limitada aos últimos 10 anos, com os termos “Attention Deficit Disorder with Hyperactivity”, Child, “Tooth injuries”, “Oral health”, “Oral manifestations”, “Orofacial trauma”, “Dental trauma”, “Dental traumatology”, “Dental injuries”, conjugados parcelarmente através do conetor booleano “AND”. Resultados: Na revisão bibliográfica obtiveram-se 131 referências, selecionando-se 26, às quais se adicionaram 6 referências cruzadas, perfazendo um total de 32 referências bibliográficas. No estudo observacional registaram-se traumatismos em mais de metade da amostra, sendo as fraturas não complicadas as mais observadas, afetando principalmente o setor antero-superior. Em mais de 30% das crianças com traumatismos, observaram-se traumatismos em mais de um dente. Conclusão: A literatura sugere que as crianças com este distúrbio de neurodesenvolvimento constituem um grupo de risco para os traumatismos orofaciais, devido a uma maior propensão a acidentes. Os resultados deste trabalho parecem corroborar esta informação, sendo ainda necessários mais estudos que investiguem esta associação. Introduction: Attention Deficit and Hyperactivity Disorder is a neurodevelopmental disorder that is characterized by a set of disruptive behaviors, such as inattention, impulsivity and restlessness. This disorder is described in the literature as a predisposing factor for the occurrence of orofacial injuries, including dental trauma, since the symptoms of the disorder lead these children to put themselves into dangerous situations, assuming thoughtless and impulsive behaviors. Aim: The aim of this study is to characterize the orofacial trauma in a pediatric population sample with Attention Deficit Hyperactivity Disorder, followed in Disorder Hyperactivity consultation at Luis Borges Development Center in Children's Hospital - Coimbra Hospital and University Centre, EPE. Additionally, a literature review was performed, aiming to systematize the existent knowledge referring to this subject. Methods: Fifty children and adolescents, who attended to the Disorder Hyperactivity consultation at Luis Borges Development Center in Children's Hospital - CHUC, EPE, were observed between December 2015 and May 2016, in order to study the prevalence of orofacial trauma, according to the diagnostic criteria of the World Health Organization, fulfilling the principles and ethical requirements. Additionally, a literature search was done in PubMed/Medline AND EBSCOhost with the keywords “Attention Deficit Disorder with Hyperactivity”, Child, “Tooth injuries”, “Oral health”, “Oral Manifestations”, “Orofacial Trauma”, “Dental Trauma”, “Dental traumatology”, “Dental Injuries”, individually combined through the Boolean connector "AND", covering the last 10 years of English and Portuguese language publications with an indexed abstract. Results: The literature review yielded 131 references, of which 26 were selected and 6 publications were added through across-reference process, for a total of 32 references. In the observational study, trauma injuries were observed in more than half of the sample, being the uncomplicated fractures the most common, mainly affecting the anterosuperior sector. More than 30% of the children with trauma, had injuries in more than one tooth. Conclusion: The literature suggests that children with this neurodevelopmental disorder constitute a risk group for orofacial injuries due to a greater propensity to accidents. The results of this study seem to corroborate this information, although more studies are required to investigate this association.
