Title: Caracterização dos doentes e resultados obtidos na consulta de acupunctura no período de 2010 a 2015

Abstract: Introdução: A Acupunctura é uma terapia que faz parte da Medicina Tradicional Chinesa, cuja prática remonta há mais de 3000 anos. Nesta técnica ocorre a inserção de agulhas finas e sólidas no corpo com finalidade terapêutica, que produzem reacções locais nos tecidos como a estimulação de nervos periféricos, induzindo uma neuro-modulação em vários sistemas. Na medicina ocidental é utilizada como uma terapia complementar desde a década de 50 e a sua utilização tem vindo a aumentar. Apresenta bons resultados em todas as patologias, nomeadamente quando existem lesões músculo-esqueléticas, patologia do sistema nervoso e patologia dolorosa. Objetivos: Realização de um estudo retrospetivo para caracterização dos doentes e patologias tratadas na consulta de Acupunctura Médica realizada no Centro Hospitalar e Universitário de Coimbra no período compreendido entre Janeiro de 2010 a Dezembro de 2015. Metodologia: Foi realizada uma consulta dos processos clínicos preenchidos no período indicado para levantamento dos seguintes dados: género, idade, faixa etária, patologia tratada, número de agulhas utilizadas, profissional que reencaminha, tipo de tratamento (eletroacupunctura ou punctura manual), resultados obtidos no final do tratamento e número de sessões. A amostra é representativa de 500 doentes, escolhidos de forma aleatória. Os dados obtidos foram sujeitos a tratamento estatístico adequado. Foi igualmente realizada uma pesquisa bibliográfica através dos motores de busca PubMed/MEDLINE e EBSCO, complementada com consulta manual, respeitando os critérios de inclusão: publicações entre Janeiro de 2004 a Março de 2016, em língua inglesa e portuguesa, com resumo disponível e em espécie humana. Foram utilizadas combinações das palavras-chave: ‘‘acupuncture’’, ‘‘complementary and integrative medicine’’, ‘‘temporomandibular joint dysfunction’’, ‘‘back pain’’, ‘‘pain’’, ‘‘chinese medicine’’ através do conector boleano AND. Resultados: A mediana da idade dos doentes é de 51 anos. Destes 71,3% são do sexo feminino e 28,7% do sexo masculino. O grupo etário que mais frequentemente recorreu à consulta foi o grupo etário compreendido entre 40-59 anos (40,7%). As patologias mais frequentemente tratadas pertencem ao grupo de patologias do aparelho locomotor (60,4%). O número de agulhas utilizado por consulta foi de 12, segundo a mediana calculada. Os profissionais de saúde que mais frequentemente reencaminharam o doente para a consulta de Acupunctura foram os médicos com especialidade em Medicina Física e de Reabilitação e os Médicos Dentistas. Em 85.2% dos casos os doentes referiram melhorias, sendo a mediana das consultas realizadas de 6. Discussão: O número de doentes incluídos no estudo representa um valor intermédio entre os referidos na literatura mas que se considera representativo da população. Os valores obtidos para idade média dos doentes e grupo de patologias tratadas apresenta neste estudo uma elevação e uma grande incidência de patologia do aparelho-locomotor e orofacial respectivamente, que se pode explicar pelo facto de uma percentagem significativa de doentes ser reencaminhado por médicos com a especialidade de Fisiatria e Medicina Dentária. Também a alta incidência de patologias normalmente associadas a idades mais avançadas como lombalgias e artroses pode contribuir para os resultados obtidos, justificando ainda o elevado número de consultas realizadas com eletroacupunctura. Conclusão: O estudo realizado representa o primeiro do seu género realizado em Portugal. De um modo geral, os resultados obtidos neste estudo estão em linha com os referidos pela literatura, embora sejam considerados dois novos parâmetros ainda não descritos: utilização ou não de electroacupunctura e número de sessões. A taxa de sucesso é elevada, sendo que os doentes apresentam melhoria em mais de 85% dos casos, validando a utilização da acupunctura como terapia complementar eficaz. Introduction: Acupuncture is a part of the Chinese Traditional Medicine and has been practised for more than 3000 years. Acupuncture consists in a technique that uses fine and solid needles to stimulate peripheral nerves as well to promote a neuro-modulation of various systems. In Western medicine it is used as a complementary therapy from the 1950s and has been increasing since then. It shows good results in various treated pathologies such as musculoskeletal injuries, diseases of the nervous system and other painful diseases. Aim: To perform a retrospective study with the main goal of characterizing the patients and treated injuries during the acupuncture consult occurring in the Coimbra University Hospital during the period of January 2010 to December 2015. Methodology: A consultation of clinical information was conducted and the fol-lowing data was recorded: gender, age, age group, pathology treated, number of needles used in each treatment, professional who referred the patient, use of eletroacupuncture, results obtained at the end of the treatment and the amount of treatment sessions. To-gether, this data represents a sample of patients treated at acupuncture consultation over the last five years (2010-2015). The sample is representative of 500 patients ran-domly selected. A statistical analysis was performed over the data. An Academic literature search was conducted in PubMed/MEDLINE and EBSCO databases, with the following inclusion criteria: publications between 2004 and 2016, written in English or Portuguese, with available abstract and human studies only. Several combinations of the following keywords were used: ‘‘acupuncture’’; ‘‘complementary and integrative medicine’’; ‘‘temporomandibular joint dysfunction’’; ‘‘back pain’’; ‘‘pain’’ and ‘‘Chinese medicine’’ through the use of the boolean connector AND. Results: It was found that the median age of patients was 51 years. In total, 71.3% were female, while 28.7% were male. The age group most represented was the age group between 40-59 years (40.7%). Overall, the most commonly treated patholo-gies were of the locomotor system with 60.4% of all patients. The average number of needles used per treatment was 12. The professionals who most often referred the pa-tient for acupuncture consultation were the physicians with physical medicine specialty and the dentists. In 85.2% of cases the patients reported improvements and on average, each patient received the treatment on 6 occasions. Discussion: The number of patients included in this study is average between those in the literature and it is considered representative of the population. The values obtained for mean age of patients and group treated pathologies in this study shows a high incidence of pathology from locomotor and orofacial systems, which can be explained by the fact that a significant percentage of patients being referred by physicians with physical medicine specialty and dentists. Also the high incidence of diseases usually associated with older ages as back pain and arthritis can contribute to the results obtained, further justifying the high number of consultations with eletroacu-puncture. Conclusion: The study is the first of its kind held in Portugal. In general, the results obtained in this study are in line with those in the literature, although this study considers two new parameters not described yet in the other studies: use or not electro-acupuncture and number of sessions. The success rate is high and patients show im-provement in 85% of the cases, validating the use of acupuncture as an effective com-plementar therapy.
