Title: Avaliação das alterações dimensionais do rebordo alveolar anterior após a extração dentária com sistema o Benex Extractor®

Abstract: Introdução: A reabsorção óssea ocorre, inevitavelmente, após a extração dentária. Este fenómeno provoca alterações tanto a nível ósseo como a nível gengival. A importância do conhecimento destas alterações reflete-se no planeamento de um bom tratamento e na prevenção de eventuais problemas que possam surgir durante a reabilitação oral. Com o intuito de reduzir o trauma resultante da técnica de extração dentária convencional, tem vindo a ser proposta uma nova abordagem. Tem como princípio a mínima manipulação dos tecidos envolventes através da aplicação de esforços axiais (extração vertical), e não pela expansão do alvéolo dentário. Objectivos: O objectivo deste trabalho é fazer uma revisão sobre as alterações dimensionais que ocorrem no rebordo alveolar após a extração dentária convencional, através de uma metodologia tipo revisão sistemática; e sugerir uma metodologia de avaliação destas alterações após a extração dentária na zona anterior do maxilar, com um sistema extractor atraumático, Benex Extractor® (Hager & Meisinger GmbH, Germany and Helmut Zepf medizintechnik, GmbH Tuttlingen, Germany). Pretende-se ainda, comparar os resultados da literatura científica actual com os benefícios e limitações da técnica clinicamente avaliada. Materiais e Métodos: Para a revisão da literatura foi feita uma pesquisa na base de dados da MEDLINE, utlizando o motor de busca PUBMED. Os casos clínicos foram realizados segundo o mesmo protocolo. Foram feitas medições pré-extracionais e no controlo pós-operatório final, através da avaliação radiográfica com CBCT, avaliação clínica com instrumentos milimetrados e avaliação de modelos de estudo. A técnica de extração utilizada foi a técnica atraumática com um sistema extrator, Benex-Extractor®, e foram realizadas consultas de controlo pós-operatório aos 8, 15, 30 dias e aos 4 meses. Resultados: Da pesquisa foram selecionados 31 estudos. Estes reportam alterações significativas dos tecidos do rebordo alveolar após a extração dentária. Para os casos clínicos, foram selecionados 3 pacientes com indicação para extração incisivos superiores. Após a extração dentária ocorreu redução da lagura e da altura do rebordo, e espessamento dos tecidos moles em todos os pacientes, tal como verificado na literatura. Conclusão: O sistema Benex Extractor® permitiu a extração dentária com a mínima manipulação dos tecidos. Apesar de não existirem diferenças significativas entre os resultados obtidos nos casos clínicos e os resultados apresentados na literatura, não houve sinais de dilaceração dos tecidos ou hemorragia evidente após a extração dentária. Contudo mais estudos devem ser feitos neste sentido para avaliar a eficácia e o impacto das técnicas e dos sistemas de extração atraumáticos na redução da taxa de reabsorção do rebordo. Introduction: Bone resorption occurs inevitably after tooth extraction. This phenomenon causes changes both to a bone level and the gingival level. The importance of the understanding of these changes is reflected in the planning of a good treatment and prevention of any problems that may arise during the oral rehabilitation. In order to reduce the trauma resulting from the conventional dental extraction technique, a new approach has been proposed. Its principle is that of the minimum manipulation of the surrounding tissues by applying axial forces (vertical extraction), and not by the expansion of the tooth socket. Objectives: The aim of this work is to review the dimensional changes that occur in the alveolar ridge after conventional tooth extraction, through a methodology type systematic review; and suggest a methodology of evaluation of these changes after tooth extraction in the anterior region of the jaw, with an atraumatic extractor system, Benex Extractor® (Hager & Meisinger GmbH, Germany and Helmut Zepf medizintechnik, GmbH Tuttlingen, Germany). The aim is also to compare the results of the current scientific literature on the benefits and limitations of the technique clinically evaluated. Methods and Materials: To review the available literature was performed a search in the MEDLINE database, using the PUBMED search engine. The clinical cases were conducted according to the same protocol. Pre-extraction measurements and end postoperative control were made by radiographic evaluation with CBCT, clinical evaluation with millimeter evaluation instruments and study models. The extraction technique used was the atraumatic technique with an extractor system Benex- Extractor®, and were performed postoperative monitoring queries to 8, 15, 30 days and after 4 months. Results: From the research were selected 31 studies. They report significant changes in the alveolar tissues after tooth extraction. For clinical cases were selected 3 patients referred for upper incisor extraction. After tooth extraction it was seen a reduced width and bead height, as well as thickness of the soft tissue in all patients, as found in the literature. Conclusion: : The Benex Extractor® system allowed for a successful tooth extraction with minimal tissue manipulation. Although there are no significant differences between the results obtained in clinical cases and the results reported in the literature, there were no signs of evident disruption of tissue or bleeding after tooth extraction. However further studies must be made in this regard to evaluate the effectiveness and impact of the techniques and atraumatic extraction systems to reduce the ridge resorption rate.
