dc.description.abstractIntrodução: A síndrome de amplificação dolorosa (SAD) é causa de dor crónica na idade pediátrica, sendo caracterizada por dor desproporcional aos achados semiológicos. A SAD pode manifestar-se conjuntamente com sinais/sintomas de disautonomia, sugerindo o diagnóstico de Síndrome Doloroso Regional Complexo (SDRC), ou na ausência de manifestações autonómicas, sendo referido como síndrome doloroso idiopático difuso/localizado. O conhecimento científico sobre a SAD na população pediátrica é particularmente limitado, continuando a colocar um desafio diagnóstico e terapêutico ao clínico. Objetivo: O objetivo deste estudo visa perceber o papel do programa de reabilitação na evolução clínica da população pediátrica com SAD – SDRC e dor musculosquelética idiopática. Métodos: Realizou-se uma análise retrospetiva baseada em processos clínicos da consulta de Medicina Física e Reabilitação (MFR) do Hospital Pediátrico de Coimbra, tendo sido identificados 16 doentes (14 raparigas e 2 rapazes). Cinco doentes cumpriam os critérios modificados de Budapeste da International Association for the Study of Pain (IASP) para diagnóstico de SDRC. Onze doentes com dor musculosquelética crónica, sem características neuropáticas, foram incluídos no grupo com diagnóstico não presuntivo de SDRC. Procedeu-se à análise da informação demográfica, clínica, diagnóstica, terapêutica e resultado funcional em ambos os grupos. Resultados: No grupo com diagnóstico presuntivo de SDRC a totalidade das crianças era do sexo feminino, com uma idade média de 13,6 anos ( 0,75). Trauma minor precedente foi identificado em 80% dos doentes. A totalidade dos doentes manifestou sinais de disautonomia à apresentação. Registou-se uma média de 0,8 ( 0,2) exames complementares de diagnóstico (ECD) realizados (radiografia simples correspondeu a 50%). À totalidade destes doentes foi prescrito um programa de reabilitação, sendo objetivada retoma funcional em 100% dos doentes. No grupo com diagnóstico não presuntivo de SDRC, observou-se predomínio do sexo feminino (81,8%) e idade média de 15,55 anos ( 0,31). Ausência de fator precipitante e ausência de manifestações disautonómicas foram achados prevalentes, em 72,7% e 100%, respetivamente. Foi prescrito programa de reabilitação isolado à maioria dos doentes (72,7%), em 18,2% associou-se farmacoterapia contínua e em 9,1% procedimento invasivo. Verificou-se retoma funcional em 54,5% dos doentes, com 45,5% a manterem algum grau de limitação. Conclusão: Os resultados do presente estudo sugerem o programa de reabilitação, consistindo em agentes físicos (termoterapia e eletroterapia), técnicas cinesiológicas, hidroterapia e exercício terapêutico, em conjunto com acompanhamento psicológico, como uma medida eficaz na abordagem terapêutica da SAD. Estudos adicionais são necessários para esclarecer a duração e intensidade ótimas do programa de reabilitação.Background: Amplified musculoskeletal pain syndrome is the cause of chronic pain in children and is characterized by pain disproportional to semiological findings. This syndrome can manifest itself in conjunction with signs/symptoms of dysautonomia, suggesting the diagnosis of Complex Regional Pain Syndrome (CRPS), or in the absence of autonomic manifestations, being referred to as idiopathic diffuse/localized pain syndrome. Scientific knowledge about amplified pain syndromes in the pediatric population is particularly limited, therefore they remain a diagnostic and therapeutic challenge to the physician. Objective: The objective of this study is to understand the role of a rehabilitation program on the clinical evolution of the pediatric population with amplified musculoskeletal pain syndrome – CRPS and idiopathic musculoskeletal pain. Methods: A retrospective analysis was performed based on chart review from the Physical Medicine and Rehabilitation consultation at Children’s Hospital in Coimbra. Sixteen patients were identified (14 girls and 2 boys). Five patients met the modified International Association for the study of Pain (IASP) Budapest criteria for the diagnosis of CRPS. Eleven patients with chronic musculoskeletal pain, without neuropathic characteristics, were included in the group with non-presumptive CRPS diagnosis. Information regarding demographic characteristics, clinical presentation, diagnostic and therapeutic approach and functional outcome were analyzed in both groups. Results: In the group with presumptive CRPS diagnosis all the children were female (mean age of 13.6 years). A minor incinting event preceded pain in 80% of patients. Signs of dysautonomia were present in every patient at presentation. An average of 0.8 diagnostic imaging studies were conducted (plain radiography in 50% of patients). Rehabilitation program was the sole prescribed treatment in the totality of patients. Functional recovery was documented in 100% of patients. Among the group with non-presumptive CRPS diagnosis a female predominance (81.8%) was observed (mean age of 15.55 years). In 72.7% of patients no inciting event was identified. None of the patients presented with autonomic dysfunction. Regarding treatment, a rehabilitation program was initiated as the sole treatment in 72.7% of patients. Pharmacologic and rehabilitation therapy were prescribed simultaneously in 18.2%. One patient (9.1%) underwent an invasive procedure. Functional recovery was documented in 54.5% of patients. 45.5% maintained some degree of limitation. Conclusion: The results of this study suggest that a rehabilitation program, consisting of physical agents (thermotherapy and electrotherapy), kinesiological techniques, hydrotherapy and therapeutic exercise, combined with psychotherapy is an effective approach to the treatment of amplified musculoskeletal pain syndromes. Additional studies are needed to clarify the optimal duration and intensity of the rehabilitation program.por
