dc.description.abstractIntrodução Os Inibidores da Enzima Conversora da Angiotensina (IECAs) e os Antagonistas dos Receptores da Angiotensina II (ARAs) são fármacos efectivos na prevenção primária e secundária do Acidente Vascular Cerebral isquémico (AVCi). Postula-se que a protecção vascular resultará não apenas das propriedades anti-hipertensoras mas também de um potencial efeito vasoprotector e modulador do sistema fibrinolítico destas classes de fármacos. Este trabalho tem como objectivo avaliar o impacto da terapêutica prévia com IECAs vs ARAs no prognóstico funcional após AVCi agudo, e possível relação com a terapêutica fibrinolítica endovenosa. Métodos Estudo observacional, de coorte histórica, que incluiu os doentes admitidos consecutivamente na UAVC do serviço de Neurologia do Centro Hospitalar e Universitário de Coimbra (CHUC), no período compreendido entre 1 de Janeiro de 2011 a 31 de Dezembro de 2012, com o diagnóstico de AVCi e história prévia de hipertensão arterial medicada. Recolheram-se dados clínicos, laboratoriais e imagiológicos por consulta de instrumento de registo específico da UAVC dos CHUC. O diagnóstico final foi confirmado por consulta de carta de alta. O estado funcional do doente foi avaliado três meses após o AVCi utilizando a mRS (modified Rankin Scale). Consideraram-se dois grupos, um designado de “bom prognóstico”, (mRS ≤2) e outro de “mau prognóstico” (mRS >2). Resultados Foram incluídos 234 doentes, 87 (37.2%) sob terapêutica prévia com ARAs, 85 (36.3%) com IECAs e 62 (26.5%) com outros fármacos anti hipertensores, constituindo estes os nossos grupos de estudo. Nos doentes sob ARAs identificou-se menor frequência de diabetes mellitus e AVC prévio. A severidade inicial do AVCi foi menor nos doentes sob ARAs (severidade ligeira a moderada à admissão/ NIHSS < 14: ARAs = 65.1%; IECAs = 45.0%; e outros anti-hipertensores = 58.1%) e a evolução às 48 horas favorece o mesmo grupo de doentes (média da diferença entre o valor da escala NIHSS à admissão e às 48h: ARAs = 2.828; IECAs = 1.459; e outros anti-hipertensores, p=0.081). Nas restantes variáveis não encontramos diferenças significativas entre os três grupos, nomeadamente no que se refere á idade, forma de apresentação clínica, etiologia do AVCi e realização de terapêutica de fase aguda. Na análise multivariável verificámos que a terapêutica prévia com ARAs é um factor independente de bom prognóstico (OR ajustado = 3.145; 95% IC= 1.407 – 7.038). Ao estratificar a amostra, de acordo com a realização ou não de terapêutica fibrinolítica, apenas no grupo submetido a fibrinólise se manteve a relação entre a terapêutica prévia com ARAs e o bom prognóstico (OR ajustado = 3.974; 95% IC= 1.208 – 13.066). Na amostra de doentes submetidos a fibrinólise os três grupos são de uma forma geral semelhantes entre si, excepto na severidade à admissão, avaliada pela escala NIHSS, que favorece os doentes sob ARAs, comparativamente aos restantes grupos (NIHSS < 14 em 62.5% dos doentes sob ARAs, 28.0% sob IECAs e 36.7% sob outros antihipertensores). O grupo de doentes sob terapêutica prévia com ARAs foi o único a apresentar uma evolução favorável às 48h (p = 0.002; média da diferença entre o valor da escala NIHSS à admissão e às 48h = 3.646). Conclusão Os resultados deste estudo sugerem que a terapêutica prévia com ARAs associa-se a um melhor prognóstico do AVCi, mas este benefício parece estar presente apenas nos doentes submetidos a fibrinólise endovenosa e pode resultar de uma menor severidade inicial do evento vascular.por
dc.description.abstractIntroduction Angiotensin-converting enzyme inhibitors (ACEI) and angiotensin receptor blockers (ARBs) are effective drugs in primary and secondary prevention of ischemic stroke. The vascular protective effect may not result only of anti-hypertensive properties but also from a potential vasoprotective effect due to modulation of the fibrinolytic system by these classes of drugs. This study aims to assess the impact of prior therapy with ACE inhibitors vs ARBs in functional outcome after acute ischemic stroke, and possible links with intravenous fibrinolytic therapy. Methods This is an observational, historical cohort study, that included patients consecutively admitted to the Neurology Service of Centro Hospitalar e Universitário de Coimbra (CHUC), between January 1st of 2011 to December 31st of 2012, with ischemic stroke diagnosis and history of medicated hipertension. Clinical, laboratory, and imaging data were collected consulting a specific registry instrument used in the Neurology Service of CHUC, which was completed by a vascular neurologist during patient admission. The final diagnosis was confirmed by discharge notes. The functional status of the patient was evaluated 3 months after acute ischemic stroke episode, using the mRS (modified Rankin Scale). Two distinct groups were considered: "good outcome ", (mRS ≤ 2), and another called the “poor outcome ", (mRS > 2). Results The study included a total of 234 patients, 87 (37.2 %) with previous therapy with ARBs, 85 (36.3 %) with ACE inhibitors and 62 (26.5 %) with other anti-hypertensive agents, that constituted our study groups. Patients with a prior therapy with ARB had a lower incidence of diabetes mellitus and previous stroke. The initial severity of ischemic stroke was lower in patients on ARBs (mild to moderate in severity at admission/ NIHSS < 14: ARBs = 65.1 %, ACEI = 45.0 % and other antihypertensive = 58.1 %) and at 48 hours the evolution favored the same group of patients (mean difference between the value of the NIHSS scale at admission and at 48h: ARBs = 2.828; ACEI = 1.459; and other antihypertensives, p = 0.081). There were no significant differences among the three groups for other variables, such as age, specific clinical presentation and etiology of ischemic stroke and production of acute phase therapy. In multivariate analysis we found that prior therapy with ARBs is an independent factor of good prognosis (adjusted OR = 3.145; 95% CI = 1.407 - 7.038). After stratifying for fibrinolytic therapy, only the group undergoing fibrinolysis was associated with prior ARBs therapy and good prognosis (adjusted OR = 3.974; 95% CI = 1.208 - 13.066). In the sample of patients undergoing fibrinolysis the three groups were similar among themselves, except for severity at admission as assessed by NIHSS scale, that favored patients receiving ARBs compared to the other groups ( NIHSS < 14 in 62.5 % of patients under ARBs, 28.0 % under ACE inhibitors and 36.7 % in other antihypertensives). The group of patients receiving prior therapy with ARBs was the only with a favorable evolution at 48h (p = 0.002, mean difference between the value of the NIHSS scale at admission and at 48h = 3.646). Conclusion The results of this study suggest that previous therapy with ARBs is associated with a better prognosis of ischemic stroke, but this benefit appears to be present only in patients undergoing intravenous fibrinolysis and may result in a lower initial severity of vascular event.por
