Abstract: CONTEXTO: A prostatite é uma das patologias urológicas mais frequentes. Foram definidas quatro categorias, sendo que a categoria III refere-se a prostatite crónica/síndrome de dor pélvica crónica. A apresentação clínica desta síndrome é heterogénea e complexa. OBJECTIVO: Rever e actualizar os conceitos sobre a prostatite crónica/síndrome de dor pélvica crónica, salientando os aspectos principais da fisiopatologia, apresentação clínica, diagnóstico e tratamento. AQUISIÇÃO DE EVIDÊNCIA: Pesquisas em livros de texto e em bases de dados bibliográficas – PubMed e UpToDate. Dos artigos encontrados foram citados 79. SÍNTESE DA EVIDÊNCIA: Através da análise dos resultados da pesquisa, verificou-se que existem diferentes teorias, baseadas em estudos, para explicar a fisiopatologia. Diferentes sinais e sintomas podem estar presentes em diferentes indivíduos. Relativamente ao tratamento, muitos fármacos têm sido testados e utilizados, com eficácia em alguns doentes e fracasso noutros. CONCLUSÃO: A prostatite crónica/síndrome de dor pélvica crónica é uma síndrome multifactorial e complexa, pelo que, o doente deve ser enquadrado no seu contexto biopsicossocial, para melhor entendimento e maior sucesso no tratamento.
CONTEXT: Prostatitis is one of the most common urological pathologies. Four categories were defined, for which class III refers to chronic prostatitis/chronic pelvic pain syndrome. The clinical presentation of this syndrome is heterogeneous and complex. OBJECTIVE: Review and update the concepts of chronic prostatitis/chronic pelvic pain syndrome, highlighting the most important points about pathophysiology, clinical presentation, diagnosis and treatment. EVIDENCE ACQUISITION: Research on textbooks and bibliographic databases (PubMed and UpToDate) were made. Of all the articles 79 were cited. EVIDENCE SYNTHESIS: Through the analysis of the survey results, it was found that there are different hypothesis, based on studies, to explain pathophysiology. Different symptoms may appear in different individuals. Regarding treatment, many drugs have been tested and used effectively in some patients and failed in others. CONCLUSION: Chronic prostatitis/chronic pelvic pain syndrome is a complex and multifactorial syndrome, therefore, the biopsychosocial context of the patient should be considered for better understanding and more successful treatment.
