Title: Perfeccionismo e eficácia da terapêutica dietética de emagrecimento

Abstract: Introdução: A eficácia da terapêutica dietética de emagrecimento pode ser influenciada por vários factores, nomeadamente ambientais, genéticos e psicológicos. No entanto, pouco se sabe acerca do papel destes últimos na eficácia da terapêutica dietética. O perfeccionismo, traço de personalidade caracterizado pela tendência para estabelecer padrões excessivamente elevados, tem sido associado às perturbações do comportamento alimentar, à auto-estima, à insatisfação com a imagem corporal, à percepção de stresse e à vivência de emoções negativas, aspectos que poderão também influenciar o envolvimento dos utentes nas prescições do seu terapeuta. Mas, o perfeccionismo pode também ter aspectos adaptativos, cujas eventuais repercussões positivas, ao nível da terapêutica dietética de emagrecimento, ainda não foram investigadas. Objectivos: Caracterizar a amostra de utentes da consulta de dietética e nutrição/exercício físico do Gabinete de Saúde Juvenil de Braga quanto aos comportamentos alimentares, satisfação com a imagem corporal, perfeccionismo, auto-estima, percepção de stresse e afecto positivo e negativo; e estudar o papel destas variaveis na eficácia da terapêutica dietética, avaliada através de antropometria e bioimpedância. Metodologia: Estudo prospectivo baseado na avaliação das participantes na primeira consulta e passados três (T3) e seis meses (T6) quanto à composição corporal através de antropometria e bioimpedância e quanto à avaliação psicológica através da aplicação de vários questionários de auto-resposta, como o Teste de Atitudes Alimentares; a Escala de Percepção de Stresse; o Perfil de Estados de Humor; a Escala Multidimensional de Perfeccionismo; a versão experimental da Almost Perfect Scale; a Escala de Silhuetas Corporais e a Escala de Auto-estima de Rosenberg. Foram avaliadas 78 jovens mulheres com excesso de peso na primeira consulta de nutrição do Gabinete de Saúde Juvenil de Braga (Idade média=21.18±3.37; IMC médio=29.70 Kg/m2 ±4.249). No T3, 74.4% destas (n=58) continuavam em seguimento e no T6 apenas 23.1% (n=18). Resultados: No T3 a percentagem de perda de peso correlacionou-se significativa e positivamente com o Afecto positivo/AP, Perfeccionismo Auto-orientado/PAO, Padrões Elevados/PE, Ordem e Auto-estima/AE e negativamente com Afecto Negativo/AN, Perfeccionismo Socialmente Prescrito/PSP, Discrepância e Insatisfação com a Imagem Corporal/IIC. No T6 a % perda de peso correlacionou-se significativa e positivamente com o AP e PE. A análise de regressão mostrou que no T3, considerando as variaveis psicológicas avaliadas no T0 como variáveis independentes, a Ordem (β=.307) revelou-se como o preditor significativo da % de peso perdido; já no T6, o preditor significativo foi a dimensão PE (β=.676). Discussão: Os resultados obtidos neste estudo mostram, pela primeira vez, que o perfeccionismo positivo está associado prospectivamente com a eficácia da terapêutica dietética de emagrecimento.
Introduction: The efficacy of dietary therapy in weight loss may be influenced by several factors, including environmental, genetic and psychological. However, little is known about the role of later in the efficacy of dietary therapy. Perfectionism, which is a personality trait characterized by the tendency to set excessively high standards, has been associated with eating disorders, self-esteem, body image dissatisfaction, stress perception and negative affect, may also influence the users adherence with their therapist advice. Nevertheless, perfectionism also has adaptive aspects, and their potential positive effects in the therapeutic dietary for weight loss have not yet been investigated. Objectives: To characterize a sample of outpatient users of the Dietetics, Nutrition and Exercise Consultation at the Braga Office of Youth Health regarding eating behaviors, body image satisfation, self-esteem, perfectionism, stress perception and positive and negative affect; and to study the role of these variables on the effectiveness of dietary therapy, assessed by anthropometry and bioimpedance. Methodology: Prospective study based on the evaluation of participants at the first visit in a nutritional intervention and after three (T3) and six (T6) months. Body composition was assessed by anthropometry and bioimpedance and the psychological evaluation included a set of validated self-reported questionnaires: Eating Attitudes Test-25; Perceived Stress Scale; Profile of Mood States; Multidimensional Perfectionism Scale; Contour Drawing Rating Scale; Rosenberg Self-Esteem Scale and an experimental version of the Almost Perfect Scale. Seventy-eight young overweight women were assessed in the first nutritional consultation (mean age=21:18±3:37, mean BMI=29.70 ± 4.249 kg/m2). From these, 74.4% (n=58) were assessed at T3 and 23.1% (n=18) at T6. Results: In the T3 the % of weight loss was significantly and positively correlated with the Positive Affect/PA, Self-Oriented Perfectionism, Personal high standards/PHS, Order and Self-Esteem and negatively correlated with Negative Affect, Social Prescribed Perfectionism, Discrepancy and Body Image Dissatisfaction. At the T6 the % of weight loss was significantly and positively correlated with PA and PHS. Regression analysis showed that at T3, and considering the psychological variables assessed at T0 as independent variables, the Order (β=.307) proved to be a significant predictor of the % of weight loss; at T6, the significant predictor was the PHS (β=.676). Discussion: The present work shows, for the first time that positive perfectionism is prospectively associated with the efficacy of dietary therapy for weight loss.
