O estado da arte no tratamento da doença de Dupuytren

Abstract

A doença de Dupuytren é uma doença da mão, conhecida há muitos anos, benigna e progressiva, que pode limitar a sua função e diminuir a qualidade de vida. Com o aumento da expectativa de vida, a prevalência parece estar a aumentar. O tratamento cirúrgico tem sido o gold standard, nas suas mais diversas formas, sendo o que tem os resultados mais estudados, mais previsíveis e mais duráveis. A opção corrente mais comum é a fasciectomia selectiva aberta. Os tratamentos não cirúrgicos têm sido largamente ineficazes até à introdução da colagenase do clostridium histolyticum, cujos estudos têm demonstrado a sua eficácia e segurança, apesar de faltarem resultados a longo prazo, nomeadamente em relação à recidiva e efeitos secundários.

Dupuytren’s disease is a non-malignant, progressive disorder of the hand that can severely limit hand function and diminish overall quality of life. With global life expectancy increasing, the prevalence of this disease appears to be increasing. Surgical treatment has been the gold standard, in its various forms, and it has the most studied, predictable and durable results. The most current option is selective open fasciectomy. Nonsurgical treatment has been widely ineffective until the introduction of collagenase clostridium histolyticum, which trials have demonstrated to be effective and safe, although we still have to wait for long-term follow-ups to know more on the rate of recurrence and adverse effects.
