Title: Evolução de doentes infetados com hepatite C após transplante hepático

Abstract: Introdução: A infeção pelo vírus da hepatite C (VHC) é atualmente a principal indicação para transplante hepático nos países desenvolvidos. A recorrência da infeção é universal nos doentes que são RNA VHC positivos no momento do transplante. O objetivo deste estudo é avaliar retrospetivamente a evolução destes doentes e os fatores que influenciam o curso da doença. Materiais e Métodos: Foram incluídos os 62 doentes com infeção VHC submetidos a transplante hepático desde Outubro de 1994 até Outubro de 2013. Foram realizados 12 retransplantes para 11 doentes retransplantados. 31 doentes são seguidos regularmente no Centro Hospitalar e Universitário de Coimbra (CHUC). Foi feita uma análise descritiva dos dados, e comparação de sobrevivências, recorrendo a uma análise Kaplan-Meier. Resultados: Os principais motivos para transplante foram cirrose hepática por VHC, em 29 doentes (46,8%), e carcinoma hepatocelular (CHC) enxertado num fígado cirrótico por VHC, em 25 doentes (40,3%). A idade média dos doentes era de 49 anos. O genótipo VHC mais frequentemente encontrado foi o genótipo 1 (n=27). 4 doentes apresentavam valores de RNA VHC pré-transplante indetetáveis. 43 doentes apresentaram recorrência da infeção com valores detetáveis de RNA VHC. 14 negativaram durante o follow-up, 6 sob terapêutica antiviral com peginterferon alfa (pegIFN) e ribavirina. 39 apresentaram evidência histológica de recorrência da infeção. 24 doentes sofreram rejeição aguda e 2 rejeição crónica. 43 doentes apresentaram complicações, 16 (37,2%) vasculares, 14 (32,6%) biliares e 27 (62,8%) infeções bacterianas. Considerando todos os doentes, 41,9% (n=26) faleceram, 12 no primeiro ano, e a sobrevivência ao fim de 5 anos foi de 67,8%. Dos 11 doentes retransplantados, 72,7% (n=8) faleceram e a sobrevivência média após o retransplante foi de 17 anos. Conclusão: A recorrência da infeção ocorreu em mais de 95% dos doentes vivos após D30, sendo quase universal nesta série. Os doentes com infeções bacterianas apresentaram a mais baixa taxa de sobrevivência aos 5 anos, em comparação com as outras complicações. Observou-se uma taxa de negativação relativamente baixa, o que mostra a necessidade de se aplicarem regimes terapêuticos com melhores taxas de resposta virológica sustentada e sem recurso ao interferon, como é o caso do sofosbuvir.
Introduction: Hepatitis C virus (HCV) infection is currently the most important indication for liver transplant in the developed countries. The recurrence of the infection is universal in patients who have positive HCV RNA levels at the time of the transplant. The purpose of this study is to analyse retrospectively the long-term outcome of these patients and the factors that influence the course of the disease. Patients and Methods: The authors included the 62 patients with HCV infection who underwent liver transplantation from October 1994 to October 2013. 11 patients were retransplanted, one patient twice, making a total of 12 retransplants. 31 patients are regularly followed-up in Centro Hospitalar e Universitário de Coimbra (CHUC). The authors used a descriptive analysis and a Kaplan-Meier analysis, for survival comparison. Results: The main reasons for transplantation were HCV-related liver cirrhosis, in 29 patients (46,8%), and HCV-associated hepatocellular carcinoma (HCC), in 25 patients (40,3%). The mean age of the patients was 49 years. Genotype 1 was the most frequently found genotype (n=27). 4 patients presented undetectable HCV RNA levels prior to transplantation. Recurrence occurred in 43 patients, with positive HCV RNA levels. 14 patients achieved negative results for HCV RNA during follow-up, 6 of whom were under antiviral therapy with peginterferon alfa (pegIFN) and ribavirin. 39 showed evidence of histological recurrence of the infection. 24 developed acute cellular rejection and 2 chronic rejection. 43 patients developed complications, 16 (37,2%) vascular, 14 (32,6%) biliary and 27 (62,8%) bacterial infections. Considering all patients, 41,9% (n=26) died, 12 during the first year after transplant, and the 5-year survival rate was 67,8%. Of the 11 patients who were retransplanted, 72,7% died and the mean survival after retransplant was 17 years. Conclusion: The recurrence of the HCV infection occurred in more than 95% of the patients alive after D30, almost universal in this series. Patients with bacterial infections presented the lowest 5-year survival rate, in comparison with other complications. In this study, the sustained virological response (SVR) rate was low, which shows the importance of applying treatment regimens that provide better SVR rates, without the use of interferon, such as sofosbuvir.
