negativas para a saúde da mãe e para o desenvolvimento do bebé. Tem sido associada a vários
fatores psicossociais, entre eles o temperamento difícil do bebé. O objetivo do presente
trabalho é avaliar se o temperamento difícil do bebé contribui para a angústia da mãe durante
o pós-parto, considerando o efeito de outros fatores preditivos. Métodos: Participaram no
estudo um total de 100 mulheres com idade média de 31.9 anos (dp = 4.09), maioritariamente
casadas ou vivendo com um companheiro (87.5%) e nulíparas (71.4%). Foram avaliadas na
gravidez e no 3º mês do período pós-parto, através do preenchimento de um inquérito que
incluía as versões portuguesas do Beck Depression Inventory-II (BDI-II) e da Postpartum
Depression Screening Scale (PDSS), que permitiram avaliar os sintomas depressivos; a
versão portuguesa reduzida do Profile of Mood States, que permitiu estudar os afetos positivo
e negativo; questões sobre história prévia de depressão, perceção de stress e apoio social, um
questionário sobre sintomas de insónia; e o Questionário de Temperamento Difícil do bebé,
que foi aplicado para avaliar as dificuldades de regulação emocional e problemas de sono do
bebé. Foi ainda realizada atigrafia num total de 25 bebés. A análise estatística consistiu em
análises descritivas, correlações de Pearson e de Spearman, análises de regressão linear e
análises de regressão múltipla. Resultados: Verificou-se que as variáveis da gravidez que
mais contribuíram para sintomatologia depressiva no pós-parto foram o Afeto Negativo
(33.1%, p< .001, BDI-II; 38.9%, p< .001, PDSS) e a História Prévia de Depressão (3.9%, p=
.023, BDI-II; 5.2%, p= .006, PDSS), sendo que o Afeto Positivo também se revelou
significativo para a PDSS (2.6%, p= .046). As variáveis do pós-parto que contribuíram
consistentemente para a explicação da variância do BDI-II e da PDSS foram o Afeto Negativo (38.2%, p< .001, BDI-II; 57.1%, p< .001, PDSS) e o Temperamento de Sono do bebé (3.4%,
p = .016, BDI-II; 3.6%, p= .005, PDSS). O Afeto Positivo mostrou-se significativo para a
BDI-II (8.6%, p< 0.001) e a Insónia para a PDSS (2.2%, p= .024). Conclusão: Os resultados
do presente estudo sugerem que o temperamento difícil e em particular as dificuldades em
dormir do bebé contribuem para a angústia/sintomatologia depressiva da mãe no pós-parto. Introduction: Postpartum depression is a mental health problem with negative consequences for the mother and the baby. It has been associated with several psychosocial factors including infant difficult temperament. The aim of the study was to evaluate whether a difficult infant temperament contributes to mother´s emotional distress in postpartum, considering the effect of other well-known predictive factors. Methods: A total of hundred women with an average age of 31.9 years old (dp = 4.09) participated in the study. Most women were married or living with a partner (87.5%) and the vast majority was nulliparous (71.4%). Mothers were evaluated during pregnancy and in the third month postpartum through a survey which included the Portuguese versions of the Beck Depression Inventory-II (BDI-II) and the Postpartum Depression Screening Scale (PDSS) to evaluate depressive symptoms; the Portuguese and reduced version of the Profile of Mood States to study positive and negative affect; questions about lifetime depression, perceived stress and social support, a questionnaire about insomnia symptoms; and the Difficult Infant Temperament Questionnaire which was applied to assess babies difficulties in emotional regulation and/or sleep. Actigraphy was performed in a total of 25 babies. Statistical analyses included descriptive analysis, Pearson and Spearman correlations, linear regression analysis and multiple regression analysis. Results: The variables that in pregnancy mostly contributed to mothers` depressive symptoms in postpartum were negative affect (33.1%, p< .001, BDI-II; 38.9%, p< .001, PDSS) and lifetime depression (3.9%, p= .023, BDI-II; 5.2%, p= .006, PDSS); positive affect was a significant factor for PDSS (2.6%, p= .046). In postpartum the variables that consistently contributed to BDI-II and PDSS variance were negative Affect (38.2%, p< .001, BDI-II; 57.1%, p< .001, PDSS) and infant sleep temperament (3.4%, p = .016, BDI-II; 3.6%, p= .005, PDSS). Positive affect predicted BDI-II (8.6%, p< .001) while Insomnia explained PDSS variance (2.2%, p= .024). Conclusion: Results of the present study suggest that infant’s difficult temperament, particularly his/her sleeping difficulties, contribute to mothers’ emotional distress/depressive symptoms in postpartum. |
