Células estaminais na terapêutica das doenças hepáticas

Actualmente existem, nos Estados Unidos, cerca de 10.000 pessoas com doença hepática terminal para quem o transplante constitui o único tratamento eficaz. Contudo, o número de órgãos disponíveis não é suficiente para as necessidades existentes, vindo 7% destes doentes a falecer enquanto integram as listas de espera. O advento da medicina regenerativa trouxe consigo a esperança de que formas alternativas de tratamento permitam uma abordagem terapêutica mais simples e eficaz destas patologias. A utilização de células estaminais é o procedimento que mais expectativas tem criado e a regeneração hepática, devido às suas capacidades únicas, é um dos processos que se tenta mimetizar com intuitos terapêuticos. OBJECTIVOS: Com este trabalho pretendeu-se fazer uma revisão de bibliografia disponível sobre o recurso à terapia celular com células estaminais para o tratamento das doenças hepáticas. DESENVOLVIMENTO: Foram descritos os tipos de células mais estudados até ao momento (células progenitoras hepáticas, as células estaminais hematopoiéticas e as células estaminais embrionárias) e analisado o papel das células estaminais na fisiologia, patofisiologia e terapêutica de doenças hepáticas. Foram ainda abordadas questões como a segurança a longo prazo, a tolerabilidade e a eficácia do tratamento, problemas que permanecem por resolver. CONCLUSÕES: A falta de conhecimentos no que diz respeito à própria biologia da célula estaminal, mas também a escassez de ensaios clínicos realizados até ao momento, não Células Estaminais na Terapêutica das Doenças Hepáticas 4 permitiram ainda a obtenção de respostas adequadas. Há, por outro lado, que notar a complexidade das questões éticas que, subjacentes à temática das células estaminais, condicionam consideravelmente a pesquisa neste campo

INTRODUCTION: Liver failure is caused by chronic liver diseases, congenital metabolic diseases or acute liver failure. Currently, in the United States, there are about 10.000 people with terminal liver disease for whom a transplant is the only saving procedure. Nevertheless, the number of available organs is not sufficient for the existing needs resulting in the death of 7% of these patients while waiting for a transplant. With regenerative medicine came the hope that alternative courses of treatment may allow simple and efficient therapeutic approaches to take place. The use of stem cells is the procedure that has created the most prospects and hepatic regeneration, due to its unique capabilities, is one of the processes that has been tried to mimic with therapeutic purposes. OBJECTIVES: To make a review of the literature on the use of stem cell therapy in the treatment of liver diseases. In this work hepatic progenitor cells, hematopoietic stem cells and embryonic stem cells (the most studied cell types) are described and the role of stem cells in physiology, pathophysiology and in the therapy of liver diseases is reviewed. Remaining unsolved problems like long-term safety, tolerability and treatment efficiency are also addressed. DISCUSSION: The lack of knowledge about stem cell biology as well as the shortness of clinical trials in this area did not allow the achievement of appropriate answers yet. On the other hand, it has to be taken in consideration the complexity of ethical issues that underlie the stem cells’ utilization and thus greatly constrain the research in this field
