Abstract: O Treino Cognitivo (TC) inclui o conjunto de intervenções comportamentais, não farmacológicas, que têm o intuito de preservar e, se possível potenciar, as capacidades intelectuais de um indivíduo, quer através do treino de capacidades cognitivas específicas, quer pelo exercício de actividades que têm como objectivo produzir melhorias num conjunto de áreas, que no seu todo, produzem benefícios na realização de funções da sua vida quotidiana. Pelos declínios da cognição que marcam tanto o envelhecimento normal como certas condições patológicas associadas à idade avançada, tal como o Défice Cognitivo Ligeiro (DCL) ou estados de demência, a aplicação deste tipo de medidas na população idosa tem hoje uma crescente importância. Como tal, o objectivo da presente revisão foi sobretudo rever o actual estado da arte, descrevendo alguns dos principais marcos e descobertas feitos na investigação realizada até à data nesta área. Foi também tentado averiguar quais as melhores abordagens e estratégias de TC a seguir, tanto em idosos saudáveis, como em idosos com DCL e estados de demência, nomeadamente Doença de Alzheimer (DA). De forma mais pormenorizada, é salientada a importância das técnicas de Treino Nuclear (TN) e Prática Exaustiva (PE) sobre as de Treino Estratégico (TE), como forma de obter melhores resultados, tanto em termos directos como em termos de transferência em idosos saudáveis. Salienta-se também a importância da ênfase dada ao processo de codificação no TC da memória, cujas diferenças inter-individuais poderão estar na base da diferença de desempenho de muitos idosos. De reparar também que estudos de neuroimagem mostraram que indivíduos idosos apresentam sensivelmente a mesma plasticidade cerebral que indivíduos mais jovens, quando providenciados com as devidas estratégias no processo de aprendizagem de uma nova capacidade. Para além disto, não deverá ser esquecida a importância do Exercício Físico (EF) como complemento do TC na manutenção das capacidades cognitivas na idade avançada. É também aqui descrita a vantagem das técnicas reconstituintes de TC sobre as técnicas compensatórias nos doentes com DA.
The Cognitive Training includes the set of non-pharmacological behavioural interventions which aim at preserving, and if possible potentiate, the intellectual capabilities of an individual, either by training specific cognitive abilities or by performing activities that are supposed to improve a group of areas, that in its whole intend to produce benefits in the performance of activities of daily living. As the advanced aged is associated with some level of cognitive declines (both normal aging and pathological states, like Mild Cognitive Impairment or dementia), an increasing attention is now being paid to the application of this kind of measures in the elderly population. Thus, the main objective of the present review is to analyse the current state of the art, while describing some of the main achievements and findings obtained in this area so far. It was also tried to ascertain which are the best approaches and strategies of Cognitive Training that must be followed, both in healthy elderlies and in those suffering from Mild Cognitive Impairment or even demential states, namely Alzheimer’s Disease. Concretely, the importance of the techniques of Core Training and Extended Practice, over the techniques of Strategy Training, is underlined, as a way to get better results, either direct ones or in a transfer perspective, in healthy elderlies. It’s also pointed the importance of the emphasis given to the information encoding process in the cognitive training of memory, which might hold the explanation to the inter-individual differences in the performance levels in memory tasks of many older adults. It should also be noticed that neuroimaging studies have shown that older individuals presented almost the same brain plasticity as younger individuals, but only when given the right strategies during the learning process of a new ability. Besides this, it’s also shown the importance of maintaining a regular physical activity, as a complement to the cognitive training, in the preservation of the intellectual abilities in old age. It’s also described here the advantage of restorative technics over compensatory techniques in the older adults who suffer from Alzheimer’s Disease.
