Abstract: Introdução: As infecções do tracto urinário estão entre as doenças infecciosas bacterianas mais comuns e afectam sobretudo a população feminina. Estima-se que 25% das mulheres que já tiveram um episódio de cistite aguda venha a ter, pelo menos, uma recidiva ao longo da sua vida. Objectivo: Pretende-se rever o estado da arte acerca da Infecção Urinária de Repetição na mulher adulta saudável, incluindo definição, epidemiologia, etiologia, fisiopatologia, factores de risco, diagnóstico, tratamento, profilaxia e custos associados. Materiais e métodos: Realizou-se uma pesquisa nas bases de dados da MEDLINE e PubMed, de artigos originais, revisões, meta-análises, revisões sistemáticas, normas de orientação clínica e ensaios clínicos publicados nos últimos anos, nas línguas inglesa e portuguesa. Foram utilizados principalmente os termos MeSH Urinary Tract Infections e recurrence ou Prevention and Control. Foram também consultadas publicações da Associação Portuguesa de Urologia, nomeadamente Acta Urológica, Separatas e Guias de Prática Clínica. Resultados: A cistite aguda ocorre principalmente em mulheres e raramente está associada a doenças de base e/ou anomalias estruturais ou funcionais do tracto urinário. A Echerichia coli é o agente etiológico isolado com maior frequência, sendo responsável por 70-95% do total dos casos. Este microrganismo possui determinantes genéticos de virulência e de resistência aos antibióticos, que desempenham um papel importante em infecções urinárias de repetição, definidas como a existência de, pelo menos, duas infecções durante seis meses ou três episódios durante um ano. A identificação de factores de risco e/ou situações predisponentes à infecção constitui uma etapa fundamental na abordagem das infecções urinárias de repetição, cujo diagnóstico deve incluir pelo menos uma urocultura. A antibioterapia é crucial no tratamento da cistite aguda não complicada, mas enquanto estratégia profilática não é VIII obrigatória. São possíveis várias abordagens, sendo que a profilaxia contínua apresenta uma eficácia de 95%, embora a profilaxia pós-coito seja preferível nos casos em que os episódios sucessivos de cistite estejam relacionados com a actividade sexual. A terapêutica auto-instituída está reservada para mulheres diferenciadas, com história de recorrências bem documentada, capazes de reconhecer a sintomatologia e de cumprir criteriosamente as instruções médicas. Além dos efeitos adversos associados à antibioterapia, a emergente resistência aos antibióticos acentua a necessidade de terapêuticas profiláticas alternativas, com destaque para a imunoterapia, que é hoje considerada a opção mais eficaz e mais bem documentada. O consumo de arando, a administração intravaginal de lactobacilos, a aplicação vaginal de estradiol/estriol e a acupuntura são outras das alternativas disponíveis. Finalmente, as infecções urinárias recorrentes são muito comuns e constituem um real problema sócio-económico, tendo em conta o grau de morbilidade e os custos que acarretam. Conclusões: Pela sua elevada frequência, as infecções urinárias de repetição têm grandes repercussões em termos de morbilidade e custos em saúde. De modo a minorar esta situação, é fundamental que os episódios de cistite aguda sejam correctamente diagnosticados, tratados e prevenidos, de acordo com as normas de orientação clínica actualmente em vigor, que já contemplam o crescente problema mundial da resistência aos antibióticos. Para esse efeito, não deve ser esquecida a variabilidade existente entre as várias regiões geográficas, relativamente não só aos agentes etiológicos mais frequentemente isolados, mas também ao perfil de susceptibilidade local desses microrganismos aos antibióticos.
Introduction: Urinary tract infections are among the most common bacterial infectious diseases, and mainly affect the female population. It is estimated that 25% of women who have already had an episode of acute cystitis will have at least one recurrence throughout their lives. Objective: The aim of this review is to present the state of the art on recurrent urinary tract infection on healthy adult woman. This article will review the current definition, epidemiology, etiology, pathogenesis, risk factors, diagnosis, treatment, prevention and costs of recurrent urinary tract infections. Materials and methods: A search was conducted through the databases of MEDLINE and PubMed and based in original articles, reviews, meta-analyzes, systematic reviews, clinical practice guidelines and standards of clinical trials published in recent years, both in English and Portuguese languages. Results: Acute cystitis occurs mostly in women and is rarely associated with underlying diseases and / or structural or functional abnormalities of the urinary tract. Escherichia coli is the etiologic agent isolated most frequently, accounting for 70-95% of the total cases. This organism has genetic determinants of virulence and antibiotic resistance, which play an important role in recurrent urinary infections, defined as the existence of at least two infections during six months or three episodes in a year. Identification of risk factors and / or conditions predisposing to infection is a key step when addressing recurrent urinary infections. The diagnosis should include at least one urine culture. Antibiotic treatment is crucial in the treatment of acute uncomplicated cystitis, but as a prophylactic strategy is not mandatory. Various approaches are possible. A continuous prophylaxis has an effectiveness of 95%, although post-coital prophylaxis is preferable in cases where the successive episodes X of cystitis are related with sexual activity. An acute self-treatment is reserved for differentiated women with a history of well-documented recurrences, able to recognize the symptoms and comply carefully with medical instructions. In addition to the adverse effects associated with antibiotic therapy, the emerging antibiotic resistance emphasizes the need for alternative prophylactic therapies, particularly immunotherapy, which is now considered the most effective and well-documented option. Consumption of cranberry, intravaginal administration of lactobacilli, vaginal application of estradiol / estriol and acupuncture are other alternatives available. Finally, recurrent urinary tract infections are very common and are a real socio-economic problem, taking into account the degree of morbidity and the costs they entail. Conclusions: Due to its high frequency, recurrent urinary infections have major repercussions in terms of morbidity and health costs. Considering these reasons it is essential that the episodes of acute cystitis are correctly diagnosed, treated and prevented, according to the standards of clinical practice guidelines currently in use, which already include the growing worldwide problem of antibiotic resistance. To this end, the variability between different geographical regions must not be forgotten, not only the most common etiological agents isolated, but also to the local susceptibility of these microorganisms to antibiotics profile
