exagero de sintomas é uma questão preponderante na prática e investigação
no contexto da avaliação psicológica em contexto forense. O presente estudo
analisa a deteção de comportamentos de esforço reduzido e exagero de
sintomas numa amostra de reclusos do Estabelecimento Prisional de
Coimbra, tendo como objetivos a análise da relação entre as medidas de
validade de sintomas utilizadas e variáveis demográficas (idade e
escolaridade) e o funcionamento cognitivo; a análise de potenciais
vantagens/limites das medidas de validade de sintomas utilizadas na presente
amostra; a análise da relação entre as várias medidas de validade de sintomas
utilizadas. Foram utilizados dois instrumentos dirigidos à deteção de esforço
reduzido ou exagero de sintomas de dificuldades cognitivas: Test of Memory
Malingering (TOMM; Tombaugh, 1996; Mota et al., 2008) e Rey 15-Item
Memory Test (FIT; Rey, 1964); dois índices de esforço reduzido derivados
de testes oriundos de um instrumento clássico de avaliação neuropsicológica
(Vocabulário e Memória de Dígitos; WAIS-III); um instrumento orientado
para a deteção de exagero de sintomas psicopatológicos (Structured
Inventory of the Malingered Symptomatology; SIMS; Smith & Burger, 1997;
Widows & Smith, 2005; Simões, 2010); e um instrumento de avaliação do
funcionamento cognitivo (Montreal Cognitive Assessment; MoCA;
Nasreddine et al., 2005; Simões, et al., 2008).
Não foram encontradas evidências de relação entre nenhuma das
variáveis sociodemográficas (idade e escolaridade) e os resultados no
TOMM e SIMS, justificando o recurso a estas medidas na avaliação da
validade de sintomas. Com exceção do índice V-MD, verificou-se que todas
as medidas utilizadas revelaram relações moderadas com o resultado no
MoCA. Os resultados obtidos apoiam a utilização do TOMM nesta amostra,
comparativamente às restantes medidas. Por fim, são apresentadas as
pontuações obtidas pelos sujeitos no MoCA, um instrumento recente de
rastreio cognitivo relativamente ao qual não são conhecidas investigações
em contexto prisional. The assessment of malingering, reduced effort and symptom exaggeration is a prevalent issue for the practice and research in the psychological assessment, especially in the forensic context. This study assesses the detection of reduced effort and symptom exaggeration behaviors in a sample of inmates from the prison of Coimbra. The main goals are the assessment of the relationship between the symptom validity measures used and the demographic variables (age and education) and cognitive functioning; the analysis of potential advantages/limits of the symptom validity measures applied in the present sample; the analysis of the relationship between the different symptom validity measures used. The instruments applied were: the Test of Memory Malingering (TOMM, Tombaugh, 1996; Mota et al., 2008) and the Rey 15-Item Memory Test (FIT; Rey, 1964), two instruments specifically designed for the detection of reduced effort or exaggeration of symptoms of cognitive difficulties; two indicators of reduced effort derived from a classic test of neuropsychological assessment (Vocabulary and Digit Span, WAIS-III); a tool oriented towards the detection of exaggeration of psychopathological symptoms (Structured Inventory of the Malingered Symptomatology; SIMS, Smith & Burger, 1997; Widows & Smith, 2005; Simões, 2010); and an instrument for assessing cognitive functioning (Montreal Cognitive Assessment; MoCA; Nasreddine et al., 2005; Simões, et al., 2008). There was no evidence found that suggests the presence of a relationship between either of the sociodemographic variables (age and education) and the results on the TOMM and the SIMS, which supports their use on symptom validity assessment. Except for the V-DS index, it was found that all measures applied revealed moderate relationships with the results in MoCA. The results support the use of the TOMM this sample, as compared to the other measures. Lastly, the scores obtained by the subjects in MoCA, a recent cognitive screening instrument for which no investigations are known in the prison context, are presented. |
