insuficiente ou exagero de sintomas (cognitivos e psicopatológicos) constitui
atualmente uma exigência básica em qualquer protocolo de avaliação em
contexto forense. O presente estudo enquadra um dos procedimentos
passíveis de avaliar esses comportamentos: os testes psicológicos,
especificamente a recente Escala de Validade de Sintomas – Versão
Experimental (EVS-VE; Simões, Nunes & Cunha, 2010). Ambicionando
conhecer melhor a validade e utilidade da EVS-VE, foram realizados dois
estudos: o Estudo I, de natureza normativa (Amostra da Comunidade:
N=180) e o Estudo II, de validação, com uma amotra heterogénea recolhida
em contexto médico-legal (Amostra Médico-Legal: N=53). Para além da
EVS-VE o protocolo de avaliação deste estudo inclui outras provas, como o
Inventário Estruturado de Simulação de Sintomas (SIMS; Smith & Burger,
1997; Widows & Smith, 2005; Simões, et al., 2010) e o Rey 15-Item
Memory Test (RMT; Rey, 1958, Boone at al., 2002; Simões, et al., 2010),
medidas de exagero de sintomas; o Inventário de Sintomas Psicopatológicos
(BSI; Derrogatis, 1982; Canavarro, 2007), medida de psicopatologia; a
Escala de Desejabilidade Social de Marlowe-Crowne (MCSDS; Marlowe &
Crowne, 1960; Almiro, Simões & Sousa, 2012), medida de desejabilidade
social e uma medida do nível inteletual global, as Matrizes Progressivas
Estandardizadas de Raven (MPR; Raven & Court, 1996; versão portuguesa
Infoteste, 1999).
Os resultados da EVS-VE foram promissores relativamente à
fiabilidade (consistência interna) na amostra médico-legal (α=.920), mas
questionáveis na amostra de comunidade (α=.519). No que diz respeito à
validade convergente dos resultados na EVS-VE é de referir a sua relação
com as pontuações no SIMS (relação entre Total das escalas: r=.610 na
amostra da comunidade; r=.779 na amostra médico-legal). Por sua vez, não
foi observada relação com significado estatístico entre pontuações na EVSVE
e a medida de desejabilidade social (MCSDS) ou a medida de exagero
de sintomas cognitivos (RMT). Já entre as pontuações na EVS-VE e a
medida de psicopatologia (BSI) foi observada uma relação positiva com
significado estatístico na amostra da comunidade, o que sugere alguma vulnerabilidade da EVS-VE como medida de simulação. Nessa mesma amostra não foi identificado qualquer impacto nos resultados da EVS-VE das variáveis sociodemográficas (idade, género e escolaridade). No que concerne ao contexto médico-legal, foram considerados dois subgrupos amostrais com base nos processos legais dos examinados (Responsabilidades Parentais e Outras Avaliações), onde foram observadas diferenças estatisticamente significativas entre subgrupos (pontuações mais altas no subgrupo Outras Avaliações). Foi ainda estudada a relação entre pontuações na EVS-VE e a "percepção de saúde" (física e psicológica) do indivíduo sobre si (estudados no trabalho de Nunes, 2011). Os resultados discriminaram significativamente os indivíduos, obtendo os sujeitos que não se consideravam saudáveis pontuações mais altas na EVS-VE, relativamente aos outros que se percecionam “saudáveis”. The exam of malingering, insufficient effort or exaggeration of symptoms (cognitive and psychopathological) is now a basic requirement in any evaluation protocol in the forensic context. This study involves a procedure which that can assess these behaviors: the psychological tests, specifically the recent Symptom Validity Scale – Experimental Version (EVS-VE; Simões Nunes & Cunha, 2010). Aspiring to better understanding the validity and usefulness of the EVS-VE, two studies were conducted: Study I is based on a normative nature (Community Sample: N=180) and Study II involves a heterogeneous sample collected in medical-legal context (Medical-Legal Sample: N=53). Apart from the EVS-VE, the protocol of this study includes other tests, such as Structured Inventory Malingering Symptomatology (SIMS, Smith & Burger, 1997; Widows & Smith, 2005; Simões, et al., 2010) and the Rey 15-Item Memory Test (RMT; Rey, 1958 Boone at al., 2002; Simões, et al., 2010), measures of exaggeration of symptoms, the Brief Symptom Inventory (BSI; Derrogatis, 1982; Canavarro, 2007), a measure of psychopathology, the Marlowe Crowne Social Desirability Scale (MCSDS; Crowne & Marlowe, 1960; Almiro, Simões & Sousa, 2012), measure of social desirability, and a measure of cognitive intellectual level, the Raven's Progressive Matrices Standardized (MPR; Raven & Court, 1996; Portuguese version Infoteste, 1999). The results of the EVS-VE were promising regarding the reliability (internal consistency) both in the community sample (α=.519) and in the forensic sample (α=.920). Regarding the validity of the results in the EVS- VE convergence is observed with scores on SIMS (ratio of Total scales: r=.610 in community sample, r=.779 in the forensic sample). On the other hand, there was no statistically significant relationship between scores on the EVS-VE and the measure of social desirability (MCSDS) or the measure of the extent of exaggeration of cognitive symptoms (RMT). Among the scores on the EVS-VE and the measure of psychopathology (BSI) was observed a statistically significant positive relationship, revealing vulnerabilities in the EVS-VE as a measure of simulation. In this sample it was not identified any impact on the results of the EVS-VE on the sociodemographic variables (age, gender and education). Regarding the medical-legal context, two sample subgroups were identified based on the legal processes of the ones examined (Child Custody and Other Assessments), where statistically significant differences were observed between subgroups (higher scores in the subgroup Other Assessments). It was also considered the relationship between scores on the EVS-VE and “perceived health” (physical and psychological) of the individual over himself (studied in the work of Nunes, 2011).
