globalmente é a segunda maior causa de morte por cancro, com um número
total de óbitos de 738 000 por ano. Atualmente, a infecção por Helicobacter
pylori é um conhecido fator de risco para o desenvolvimento deste tumor.
Mas, ulteriormente, o vírus Epstein-Barr (EBV) também foi considerado um
agente infeccioso causador de CG (quase 10% da totalidade dos CG são
positivos para o EBV). Devido à falta de biomarcadores apropriados e
específicos para uma deteção precoce, a maioria dos casos são
diagnosticados em fases tardias e a taxa de sobrevivência é baixa.
Neste estudo foram investigados 82 doentes com carcinoma gástrico e 33
indivíduos controlos. A presença de EBV foi avaliada por PCR em tempo
real e por hibridização in situ. A deteção e genotipagem do H. pylori foram
realizadas por PCR em tempo real, utilizando primers específicos para o
gene flagelar e para os genes cagA e vacA. A identificação de células
tumorais circulantes no sangue periférico dos doentes com GC foi efetuada
por citometria de fluxo, utilizando anticorpos anti-queratina (K) (anti-K18,
K20 e anti-KPan) e, paralelamente, foi também determinado o perfil de
expressão de Ks no tumor. Na avaliação da resposta imune antitumoral
foram analisados os linfócitos infiltrantes do tumor (TIL) e os neutrófilos
infiltrantes do tumor (TIN), bem como, a população leucocitária do sangue
periférico. Análises multivariadas foram realizadas para avaliar o efeito dos
fatores prognósticos estudados, conjuntamente com o sexo, idade, tipo
histológico, localização tumoral e estágio TNM, na sobrevivência.
O EBV, por PCR, foi detetado em 96,3% dos casos, enquanto que, por
hibridização in situ foi positivo em apenas nove casos (11%). O H. pylori foi
encontrado em 51 (62,2%) doentes com CG; 60,5% e das estirpes de H.

pylori foram cagA positivo. As células tumorais circulantes foram detetadas
em 31,9% dos doentes e, a K18 revelou-se o marcador mais sensível, pois foi
encontrada em 23,6% dos casos. Nos tumores o padrão de expressão de
queratinas permitiu diferenciar 5 fenótipos distintos, em que o mais
prevalente foi o fenótipo 1 (K18+/k20-/kPan-) com 43,1% dos doentes,
seguido do fenótipo 4 (K18+/ K20-/KPan+) com 25% dos casos. O perfil de
expressão de queratinas nos tumores tem impacto significativo na
sobrevivência dos doentes: o fenótipo 4 apresenta a melhor sobrevivência e o
fenótipo 2 a pior. Em nenhum dos indivíduos saudáveis foi observado a
presença de queratinas, quer no tecido gástrico quer no sangue periférico.
Verificamos que os neutrófilos e os linfócitos B infiltrantes do tumor
exercem atividade pró-tumoral, enquanto que, às células NK e às células T,
nomeadamente as CD4+, as CD8+ e as gd estão envolvidas na resposta
imune antitumoral.
Quanto a parâmetros com influência significativa no prognóstico dos doentes
identificamos o sexo, a profundidade de invasão tumoral, a presença de
metástases linfonodais, a classificação TNM e o fenótipo de expressão das
queratinas no tecido tumoral. Obteve-se um modelo preditivo de existência
de tumor gástrico que se perspectiva ser utilizado no rastreio de indivíduos
com clínica sugestiva de cancro do estômago. Gastric cancer (GC) is one of the most common neoplasms in Portugal, and also the second leading cause of cancer-related deaths worldwide with 738 000 deaths per year. Nowadays, Helicobacter pylori infection is a well known risk factor for the development of gastric cancer. Epstein–Barr virus (EBV) has also been regarded as a GC-causing infective agent (nearly 10% of all cases of GC are positive to EBV). Due to the lack of suitable and specific biomarkers for early detection, and to the fact that most cases of the disease are diagnosed at late stages, survival rate is low. In this work it were investigated 82 patients with gastric carcinoma and 33 healthy subjects. The presence of EBV was assessed by real time PCR and in situ hybridization. Detection of H. pylori and genotyping were performed by real time PCR, using specific primers for the flagella gene and cagA and vacA genes. In order to identify circulating cancer cells in patients with GC, peripheral blood samples were investigated by flow cytometry using antikeratin antibodies (anti-K18, anti-K20 and anti–KPan). Comparison was made with the keratin expression profile in malignant gastric tissue. To characterize the antitumour immune response in GC patients we analyzed tumour-infiltrating lymphocytes (TIL) and tumour-infiltrating neutrophils (TIN) as well as the leukocyte population in peripheral blood. Multivariate analyses were performed to assess the value of the prognostic factors listed aboved together with sex, age, histological classification, location of neoplasia and TNM stage, on survival. EBV was detected by PCR in 96% of cases, whereas by in situ hybridization was detected in the tumour cells in only nine cases (11%). H. pylori was found in 51 (62.2%) GC patients and 60.5% of the strains of H. pylori were cagA positive. Circulating tumour cells were detected in 31.9% of the GC Abstract xlviii patients, and the K18 proved to be the most sensitive marker, as was found in 23.6% of cases. In tumours, the expression pattern of keratins enable the identification of five distinct phenotypes in which the most prevalent was phenotype 1 (K18+/k20-/kPan-) with 43.1% of patients, followed by the phenotype 4 (K18+/K20-/KPan+) with 25% of cases. The expression profile of keratins in tumours has a significant impact on patient survival: phenotype 4 presents the higher survival and phenotype 2 the lowest. None of the gastric or blood specimens from healthy subjects had keratin expression. We found that neutrophils and B lymphocytes infiltrating the tumour exert pro- tumour activity, whereas NK cells and T cells, including CD4 +, CD8 + and g d, are involved in the anti-tumour immune response. As parameters with significant influence on the prognosis of patients we identified gender, depth of tumour invasion, lymph node metastasis, TNM classification and the phenotype expression of keratins in tumour tissue. A predictive model for gastric cancer was obtained and the perspective is that it represents a valuable tool to be used in the screening of patients with suggestive symptoms of gastric cancer. |
