TERAPÊUTICA DA DOR EM DOENTES PALIATIVOS - PROPOSTA DE PROTOCOLO CLÍNICO

Abstract

INTRODUCTION: The progressive population aging is the common denominator worldwide and encloses a relevant duality. It represents, on the one hand, a frankly positive figure revealing the increase in life expectancy due to biotechnological and medical advances, but on the other hand, it raises a matter of great importance due to the alarming inherent consequences. Since the emergence of chronic, advanced, and incurable pathologies is a reality in this context, the progression of age is thus an authentic bio-socio-economic challenge. In this sense, the growing need for specialized health care becomes inevitable, prioritizing the reduction of suffering and the preservation of quality of life (Qol) during the course of the disease until the final moments. Effective pain treatment and control of other associated symptoms are listed as key aspects for maintaining human dignity. For this, it is necessary to create and improve tools that promote the search for concrete answers in many areas, but primarily in pain therapy and in the minimization of suffering. These solutions must emerge, in a timely manner, for a population whose fragility and finitude are assumed to be a constant reality. OBJECTIVES: To define a clinical guidance protocol in pain therapy for patients in need of palliative care, structurally extended, practical, adaptable to the various realities, in a conventional format and in a digital platform. Secondly, define specific fundamental aspects related to the symptomatic control of Total Pain in search of an overall improvement in the Qol of the patient as a person. METHODOLOGY: Exploratory and descriptive bibliographic research based on the review of the scientific literature and analysis of the selected articles. The specific search method was cumulative through Medline databases with PubMed search interface, the repository of the University of Coimbra, Portuguese Medical Journals, Cochrane Library, Ebsco, Google Scholar, and B-on online library. The research was restricted to complete articles in Portuguese and English with extended research dates and search-based MeSH terms (Medical Subjects Headings): “total pain management, palliative care, symptomatic control of palliative patients, analgesics, pain monitorization, pain pathophysiology". This research resulted in the description of Theoretical and Clinical Contextualization and it constituted the basis for consolidating the structuring of protocols and description of the pharmacodynamic and pharmacokinetic characteristics of the drugs listed based on data from the National Authority for Medicines and Health Products (INFARMED), Online DrugBank and guidelines from General Directorate of Health (DGS), complemented by clinical experience and individual and group personal practice. CONCLUSIONS: With the renewed awareness that the growing population aging is accompanied by incurable chronic pathologies and specialized care needs, it is imperative to improve therapies in the various institutional and community realities where care is needed for end-of-life patients. As such, it is central to minimizing suffering through an effective and appropriate therapeutic plan of pain and symptomatic control. Therefore, this protocol intended to bring a valid contribution in this context through a practical and objective view in the search for the improvement of the Qol of palliative patients in order to maintain their dignity until the final moments of life.

INTRODUÇÃO: O progressivo envelhecimento populacional é o denominador comum a nível mundial e encerra em si uma relevante dualidade. Representa, por um lado, um dado francamente positivo, revelador do aumento da esperança de vida graças ao desenvolvimento biotecnológico e da medicina, mas por outro lado, faz emergir uma matéria de grande importância devido às alarmantes consequências inerentes. Sendo o aparecimento de patologias crónicas, avançadas e incuráveis uma realidade neste contexto, a progressão da idade constitui, assim, um autêntico desafio biossocioeconómico. Neste sentido, torna-se inevitável a necessidade crescente de cuidados de saúde especializados, priorizando a redução do sofrimento e a preservação da qualidade de vida (QV) durante o curso da doença até aos momentos finais. O eficaz tratamento da dor e o controlo dos demais sintomas associados são elencados como aspetos primordiais para manutenção da dignidade humana. Para tal, torna-se necessária a criação e o aperfeiçoamento de ferramentas que promovam a busca de respostas concretas em muitas áreas, mas primordialmente na terapêutica da dor e na minimização do sofrimento. Estas soluções urgem emergir, em tempo útil, para uma população cuja fragilidade e finitude se assumem como uma realidade constante. OBJETIVOS: Definir um protocolo clínico de orientação em terapêutica da dor para doentes com necessidade de cuidados paliativos, estruturalmente alargado, prático, adaptável às diversas realidades, em formato convencional e em plataforma digital. Secundariamente, definir aspetos específicos fundamentais relativos ao controlo sintomático da Dor Total em busca da melhoria global da QV do doente como pessoa. METODOLOGIA: Pesquisa bibliográfica do tipo exploratório e descritivo baseado na revisão da literatura científica e análise dos artigos selecionados. O método de pesquisa específico foi cumulativo através das bases de dados Medline com interface de pesquisa PubMed, do repositório da Universidade de Coimbra, Revistas Médicas Portuguesas, Cochrane Library, Ebesco, Google Académico e biblioteca online B-on. A pesquisa foi restringida para artigos completos em língua portuguesa e inglesa com datas de pesquisa alargadas e com base de busca nos termos MeSH (Medical Subjects Headings): total pain management, palliative care, symptomatic control of palliative patients, analgesics, pain monitorization, pain pathophisiology. Dessa pesquisa resultou a descrição da Contextualização Teórica e Clínica e, posteriormente, constituiu a base para consolidar a estruturação dos protocolos e descrição das características farmacodinâmicas e farmacocinéticas dos medicamentos enumerados com base nos dados da Autoridade Nacional do Medicamento e Produtos de Saúde (INFARMED), DrugBank online e diretrizes da Direção Geral da Saúde (DGS), complementadas pela experiência clínica e prática pessoal individual e de grupo. CONCLUSÕES: Com a renovada tomada de consciência de que o crescente envelhecimento populacional se faz acompanhar de patologias crónicas incuráveis e com necessidades de cuidados especializados, torna-se imperativo o aperfeiçoamento das terapêuticas nas diversas realidades institucionais e comunitárias onde seja necessário prestar cuidados aos doentes em fim de vida. Como tal, é fulcral a minimização do sofrimento através de um eficaz e adequado plano terapêutico da dor e controlo sintomático. Assim sendo, este protocolo pretendeu trazer um contributo válido nesse âmbito através de uma visão prática e objetiva, na busca da melhoria da QV dos doentes paliativos, de forma a manter a sua dignidade até aos momentos finais da vida.
