O PAPEL DO MÉDICO DE FAMÍLIA NA ABORDAGEM E SEGUIMENTO DO CASAL INFÉRTIL

Abstract

A infertilidade é um problema importante de saúde pública que causa sofrimento pessoal significativo. O médico de família tem um papel inestimável na investigação inicial, referenciação e suporte psicológico durante o tratamento. Este trabalho tem como objetivo analisar a literatura disponível sobre o tema infertilidade enquadrado no contexto de cuidados de saúde primários, documentando os vários passos da abordagem e seguimento, com o intuito de esclarecer o papel do médico de família na atuação e resposta a este problema, dentro do que é praticável e sensato no âmbito da medicina geral e familiar. Foi reformulada recentemente uma definição de infertilidade mais dirigida á prática e às características clínicas do casal infértil. Em Portugal, constatou-se uma prevalência da infertilidade ao longo da vida situada entre os 9 e 10%. A procura de serviços relacionados com a infertilidade tem vindo a crescer nos últimos anos, este aumento está relacionado com a tendência social atual em adiar a gravidez, maior visibilidade e conhecimento das opções terapêuticas e maior aceitação em sociedade do reconhecimento da infertilidade como condição médica. Compete ao médico de família ouvir, informar, investigar e orientar da melhor forma o casal que surge na consulta por infertilidade. A referenciação deve ser feita sempre que o tratamento ou investigação não estejam ao alcance dos cuidados de saúde primários. O médico de família ao desempenhar um papel inaugural na abordagem inicial do casal infértil, seria sensato sustentar uma formação mais dirigida á infertilidade e ao suporte do próprio casal no âmbito dos cuidados de saúde primários.

Infertility is an important public health issue that leads to a great personal suffering. Family physicians have a key role in the initial investigation, referral and physiological support during the treatment. This review aims to provide an analysis on the current literature on infertility in primary care settings, detailing the several steps on approach and pursuance, with the intent to clarify the purpose of family doctors in practice and response to this problem, inside the scope of what’s desirable and reasonable in primary care. Updated redefine definition of infertility takes into account the clinical features of the couple as well as being more focused on medical practice. In Portugal, the prevalence of infertility is between 9 to 10% in a lifelong. The search for services related to infertility has been rising for the past years, being this partly due to social tendencies to delay pregnancy, greater visibility and knowledge of option treatments and a bigger acceptance in society of infertility as a medical condition. Family physicians have an important role in making sure the couple is heard and informed along with starting investigation and guidance. Referral must be done whenever treatment and investigation cannot be performed in primary care settings. By performing an initiating contact on the approach to infertile couples, a more focused training on infertility and support for this couples in primary care should be strengthened.
