Abstract: A criação do SNS em 1979 abriu portas ao acesso generalizado aos cuidados de saúde em Portugal. Com a missão de prestar cuidados preventivos, curativos e de reabilitação a toda a população, o SNS carateriza-se por ser universal, geral e tendencialmente gratuito. Na noção de universalidade cabe o conceito de equidade, que nunca foi totalmente atingido face à dificuldade em encontrar um equilíbrio entre a prestação de cuidados de saúde próximos e a eficiente gestão de recursos limitados. Assim, pretendeu-se com este estudo avaliar a existência de iniquidades na prestação de Cuidados de Saúde Primários entre territórios litorais (ACeS Baixo Mondego) e interiores (ACeS Beira Interior Sul). Para isso, através de estatística inferencial, comparámos os resultados obtidos pelos dois ACeS em termos de controlo e seguimento de doenças crónicas; acompanhamento de saúde materna e infantil; atividade preventiva e capacidade de resposta. Os resultados encontraram diferenças estatisticamente significativas em nove variáveis estudadas, oito das quais com o ACeS BIS a apresentar piores resultados (proporção de diabéticos com exame oftalmológico no último ano; proporção de grávidas com ecografia do 2º trimestre; índice de realização de exames laboratoriais do 1º, 2º e 3º trimestres; proporção de crianças com 1 ano de idade com acompanhamento adequado; proporção de crianças com 2 anos de idade com acompanhamento adequado e proporção de consultas médicas iniciadas pelo utente realizadas nos primeiros 15 dias) e uma (proporção de consultas médicas “do dia” efetuadas em UF diferente) com o ACeS BM a atingir valores aquém do esperado. Estas diferenças encontram explicação na menor disponibilidade de cuidados especializados no interior; no facto de os dados analisados resultarem de registos que muitas vezes estão incompletos e nas diferentes caraterísticas das populações de cada ACeS, com uma população envelhecida no interior, a precisar não só de maior número de contactos, mas também de contactos mais complexos. A identificação destas iniquidades é de extrema relevância para que se implementem políticas que as visem combater a fim de promover a saúde global das populações.
The creation of the SNS in 1979 opened the door to generalised access to healthcare in Portugal. With the mission of providing preventive, curative and rehabilitative care to the entire population, the SNS is characterized by being universal, general and tendentially free of charge. Within the notion of universality lies the concept of equity, which has never been fully achieved due to the difficulty in finding a balance between the provision of close health care and the efficient management of limited resources. Thus, this study aimed to assess the existence of inequalities in Primary Health Care provision between coastal (ACeS Baixo Mondego) and inland territories (ACeS Beira Interior Sul). To this end, we used inferential statistics to compare the results obtained by the two ACeS in terms of chronic disease control and follow-up; maternal and child health monitoring; preventive activity and responsiveness. The results found statistically significant differences in 9 variables studied, 8 of which with the ACeS BIS showing worse results (proportion of diabetics with ophthalmologic examination in the last year; proportion of pregnant women with 2nd trimester ultrasound; index of laboratory tests achievement in the 1st, 2nd and 3rd trimesters; proportion of 1 year old children with appropriate follow-up; proportion of 2 year old children with appropriate follow-up and proportion of user-initiated medical consultations made within the first 15 days) and 1 (proportion of "on the day" medical consultations performed in a different FU) with the ACeS BM achieving lower values than expected.These differences can be explained by the lower availability of specialised care in the inland territories; the fact that the data analysed results from records that are often incomplete; and the different characteristics of the populations of each ACeS, with an ageing population in the inland requiring not only a greater number of contacts, but also more complex contacts. The identification of these inequalities is extremely important in order to implement policies that aim to combat them in order to promote the overall health of the populations.
