Abstract: A Síndrome do Ovário Poliquístico (SOP) é uma das endocrinopatias mais frequentes no sexo feminino, com uma prevalência mundial estimada entre os 5 e 20% das mulheres em idade reprodutiva. Atualmente o seu diagnóstico é sobretudo de exclusão. Para tal existem, presentemente, os critérios de diagnóstico da Androgen Excess and Polycystic Ovary Syndrome Society (AE-PCOS), National Institutes of Health (NIH) e os critérios de Rotterdam. Estes critérios utilizam diferentes combinações de marcadores clínicos e bioquímicos, condicionando uma grande variedade de fenótipos de SOP distintos entre si. Estes factos motivam uma grande discussão e discórdia constante sobre o diagnóstico de SOP. A patogénese da SOP continua a ser desconhecida, existindo muitas teorias e poucas certezas. Pouco se sabe acerca das bases e vias moleculares envolvidas. No entanto, a insulinorresistência e o estado de hiperandrogenismo por ela causado, são atualmente considerados bases fisiopatológicas primordiais, tendo constituído um grande avanço no conhecimento da SOP. Atualmente a SOP e insulinorresistência são duas entidades em estreita relação. Também a obesidade e alterações metabólicas começam a ganhar um papel de destaque em torno desta tão complexa patologia.À luz dos conhecimentos atuais, a terapêutica a instituir deve ser individualizada tendo em conta as manifestações clínicas de cada doente. Assentando numa terapêutica essencialmente sintomática e dirigida aos sintomas apresentados, esta é baseada na administração de contraceção hormonal combinada e metformina. Começam a surgir novas opções, muito promissoras, que podem revolucionar a abordagem terapêutica desta entidade nosológica.
Polycystic ovary syndrome is one of the most frequent endocrinopathies in females, with an estimated worldwide prevalence between 5 and 20% of women of reproductive age.Currently the diagnosis is an exclusion one. Nowadays, the diagnostic criteria of the Androgen Excess and Polycystic Ovary Syndrome Society (AE-PCOS), National Institutes of Health (NIH) and the Rotterdam are the existing criteria. These criteria use different combinations of clinical and biochemical markers, wich generate a wide variety of polycystic ovary syndrome phenotypes that are distinct from each other. These facts motivate a great deal of discussion and constant disagreement about the diagnosis of polycystic ovary syndrome.The pathogenesis of polycystic ovary syndrome remains unknown, with many theories and few certainties. Little is known about the molecular bases and pathways involved. Despite this, insulin resistance and the inherent hyperandrogynism are currently considered one of the main pathological bases, having been a great advance in the knowledge about polycystic ovary syndrome. Currently, polycystic ovary syndrome and insulin resistance are two closely related entities. Obesity and metabolic disorders are also paving the way to play a fundamental role in this complex pathology.Currently, the therapy to be instituted must be individualized, as each patient is unique in its manifestations. Based on essentially symptomatic therapy and directed to the presented symptoms, it is based on the administration of combined hormonal contraception and metformin. New, very promising options begin to emerge, which may revolutionize the therapeutic approach to this pathology.
