Abstract: Introduction: Patient Centered Medicine (PCM) approach is becoming ever more important because of its association with positive effects on doctor and patient. The present study aimed to know whether the approach to a clinical case, through the consultation’s problems classification of a fictitious case by doctors of General and Family Medicine, showed characteristics of a mechanistic approach or a systemic one.Methods: A cross-sectional observational study was carried out in the Central Region of Portugal and in the Autonomous Region of the Azores. Doctors were invited to participate, anonymously, by electronic invitation either on the official internship network or on a doctor’s social network, the MGFamiliar network. A self-fulfilling questionnaire was used to verify the classification of a clinical case in Subjective, Assessment and Plan (from the SOAP methodology) with the response options of the International Classification of Primary Health Care (ICPC2) in chapters P (Psychological) and Z (Social) possible for this case. Mechanicists were considered as those who only classified '' P '' and systemic those who selected options from chapters P + Z or only Z. The questionnaire also included questions for epidemiological characterization, such as sex, age, time of exercise of Medicine, being a specialist or intern, being a specialist advisor and type of work unit.Results: A sample of 227 doctors (70% female; 66% specialists, 34% interns) was studied. There were no significant differences in the studied variables. In Subjetive SOAP chapter classification 56% of the doctors were systemics and in Assessment such proportion was of 59%. In the Plan, there was a predominance of technicians with a frequency of 58%. Discussion: The predominance of systemic physicians in the Subjective and Assessment may be due to the inclusion of communication skills training programs in the university medical curriculum and in specific training in general practice / family medicine. The predominance of technicians in the Plan may be due to the fact that the therapy is still technically oriented towards medication and more thought about in the orbit of what the doctor thinks. The overload of work and negative models of senior doctors may justify the absence of a more frequent practice of MCP. The pandemic reality of the new coronavirus (SARS-CoV-2) may influence medical performance by increasing the workload, the tele-consultation model, the in-cabinet consultation with physical distancing and complex email reasons for encounter.Conclusion: General Practice/Family Medicine doctors of this sample, were mostly systemics for the Subjective and Assessment parts of consultation and more technicist in the Plan, requiring training actions, in undergraduate and postgraduate courses in Continuous Professional Development, to improve PCM.
Introdução: A abordagem em função da Medicina Centrada na Pessoa (MCP) tem vindo a adquirir relevo por associar-se a efeitos positivos em médico e paciente. O presente estudo visou saber se a abordagem a um caso clínico, através da classificação de problemas na consulta de um caso fictício por médicos de Medicina Geral e Familiar, mostrava características de abordagem mecanicista (apenas a visão médica da patologia) ou sistémica (a visão do médico e da causalidade no paciente).Materiais e Métodos: Realizou-se um estudo observacional transversal quantitativo na Região Centro de Portugal e na Região Autónoma dos Açores. Os médicos foram convidados a participar, em anonimato, por convite eletrónico quer na rede oficial dos Internatos, quer pela rede MGFamiliar. Utilizou-se um questionário de auto-resposta para verificar a classificação de um caso clínico em Subjetivo, Avaliação e Plano (da metodologia SOAP) com as opções de resposta da Classificação Internacional de Cuidados de Saúde Primários (ICPC2) nos capítulos P (Psicológico) e Z (Social) possíveis para esse caso. Consideraram-se como médicos mecanicistas os que apenas classificaram ‘’P’’ e sistémicos os que selecionaram opções dos capítulos P+Z ou apenas Z. O questionário incluía, ainda, questões para caracterização epidemiológica, tais como sexo, idade, tempo de exercício de Medicina, ser especialista ou interno, ser orientador de especialidade e tipo de unidade de trabalho.Resultados: Obtiveram-se dados de 227 médicos (70% género feminino; 66% especialistas, 34% internos). Sem diferenças significativas em função das variáveis utilizadas, foram considerados sistémicos em Subjetivo 56% dos médicos e em Avaliação 59%. Em Plano, houve um predomínio de tecnicistas (58%). Discussão: O predomínio de médicos sistémicos no Subjetivo e Avaliação pode advir da inclusão de programas de treino de competências comunicacionais no currículo universitário de Medicina e na formação específica em Medicina Geral e Familiar. O predomínio de tecnicistas em plano pode dever-se a que a terapêutica é ainda tecnicamente orientada para a medicação e mais pensada na órbita do que o médico pensa. A sobrecarga de trabalho e modelos negativos de médicos seniores podem justificar a ausência de uma prática mais frequente de MCP. A realidade pandémica do novo coronavírus (SARS-CoV-2) poderá vir influenciar a atuação médica pelo aumento da sobrecarga de trabalho e do modelo de trabalho em teleconsulta ou de consulta presencial com distanciamento, bem como de apresentação de problemas complexos via correio eletrónico. Conclusão: Os médicos em Medicina Geral e Familiar exerciam, nesta amostra, maioritariamente segundo uma abordagem sistémica no Subjetivo e Avaliação e mais tecnicista no Plano, sendo necessárias ações de formação, em pré-graduado e em pós graduado em Desenvolvimento Profissional Contínuo, para melhoria desta prática.
