Abstract: Introdução: A prevalência das doenças alérgicas tem vindo a aumentar nas últimasdécadas, e estas passaram a ser responsáveis por grandes gastos sociais eeconômicos. A partir da “Hipótese da Higiene” e da Hipótese “OrigensDesenvolvimentistas da Saúde e Doença” (DOHaD), é possível inferir que há uma janelade oportunidade para a prevenção das doenças alérgicas nos primeiros meses de vida.O leite humano é a principal fonte de nutrientes neste período crítico de programaçãoimunológica e tem grande importância no desenvolvimento da criança. Contudo, aliteratura não é consensual quanto ao seu papel na prevenção de doenças alérgicas.Objetivos: Esta revisão descritiva tem como objetivo a compilação da literatura recente,acerca da relação entre o aleitamento materno (AM) e o desenvolvimento da doençaalérgica nas crianças, especificamente na Asma, na Hipersensibilidade Alimentar, noEczema Atópico e na Rinite Alérgica.Métodos: A pesquisa bibliográfica para foi realizada nas seguintes bases de dados:Pubmed, UpToDate, Elsevier, Scielo e Google Académico. Foram selecionados 78artigos publicados entre 2005 e 2020.Resultados: A duração ótima do AM exclusivo e o período ideal para a diversificaçãoalimentar não são consensuais entre os estudos. Em relação à asma e à rinite alérgica,os resultados são contraditórios, alguns demostrando um papel protetor do AM e outrosem que esse benefício não é evidenciado. Porém, é evidente que a deficiência de certassubstâncias nas mães (ácidos gordos poliinsaturados n-3 e vitamina D) contribuem parao aumento do risco de asma nas crianças. No caso da hipersensibilidade alimentar,atualmente, não há evidência suficiente que permita tirar conclusões sobre o verdadeiropapel do AM no seu desenvolvimento. Contudo, a introdução de determinados alimentosprecocemente, enquanto se mantém o AM, pode ser protetora para o desenvolvimentofuturo de alergia a esses alimentos. No que se refere ao eczema atópico, há um númerosimilar de estudos relatando efeitos benéficos e negativos. Há também estudos quedemonstram não haver correlação.Conclusão: Os resultados dos estudos efetuados acerca do papel do aleitamento nodesenvolvimento da doença alérgica são controversos. Isso se deve a diversos fatores,como por exemplo: a dificuldade em estabelecer uma definição globalmente utilizadados diferentes tipos de doenças alérgicas e a dificuldade em estabelecer um diagnósticode certeza das mesmas. Apesar da controvérsia existente e termos muitas lacunas apreencher nesse campo, é indiscutível o papel fundamental do leite humano (LH) nodesenvolvimento da criança, faltando definir a idade ótima para iniciar a diversificaçãoalimentar, no que concerne às patologias alérgicas. A recomendação atual, com o inícioda diversificação alimentar entre os quatro e seis meses, mantendo o AM, parece a maisconsensual entre as várias sociedades científicas.
Introduction: The prevalence of allergic diseases has been increasing in recentdecades, and these have become responsible for large social and economicexpenditures. From the “Hygiene Hypothesis” and the DOHaD Hypothesis, it is possibleto infer that there is a window of opportunity for the prevention of allergic diseases in thefirst months of life. Human milk is the main source of nutrients in this critical period ofimmunological programming and is of great importance in the child's development.However, the literature is not consensual as to its role in the prevention of allergicdiseases.Objective: This review article aims to compile recent literature on the relationshipbetween breastfeeding (BF) and the development of allergic disease in children,specifically in Asthma, Food Hypersensitivity, Atopic Eczema and Allergic Rhinitis.Methods: The bibliographic data was obtained from the following databases: Pubmed,UpToDate, Elsevier, Scielo and Academic Google. Within the preceding databases, 78articles published from 2005 to 2020 were selected.Results: The optimal duration of exclusive breastfeeding and the ideal period for fooddiversification are not consensual between studies. Regarding asthma and allergicrhinitis, the results are contradictory, some demonstrating a protective role for BF andothers in which there is no correlation. However, it is evident that the deficiency of certainsubstances in mothers (polyunsaturated fatty acids n-3 and vitamin D) contributes to theincreased risk of asthma in children. With regards to food hypersensitivity, currently,there is not enough evidence to draw conclusions about the true role of BF in itsdevelopment. However, the early introduction of certain foods, while maintaining BF, canbe protective for the future development of allergy to these foods. Regarding atopiceczema, there is a similar number of studies reporting a beneficial and negative effect,apart from those indicating no correlation.Conclusion: The results of studies carried out on the role of breastfeeding in thedevelopment of allergic disease are controversial due to several factors, for instance, thedifficulty in establishing a globally used definition of the different types of allergicdiseases and the difficulty in establish an assertive diagnosis of them. Despite theexisting controversy and the fact that we have many gaps to fill in this field, thefundamental role of human milk (HM) in the development of the child is unquestionable,for initiating food diversification remains to be defined, with regard toallergic pathologies. The current recommendation, with the beginning of fooddiversification between four and six months, maintaining BF, seems the most consensualamong the various scientific societies.
