Abstract: Introdução: A abordagem de pequenas massas testiculares diagnosticadas incidentalmente é controversa, pois é complexo distinguir pré-operatoriamente se estas lesões são benignas ou malignas. Enquanto o risco de malignidade é real, pode ser possível preservar testículos com lesões benignas em segurança. Objetivos: Avaliar sistematicamente a evidência sobre a prevalência de pequenas massas testiculares, se estas apresentam histologia benigna ou maligna e a sua abordagem.Métodos: Após registo do protocolo de revisão no PROSPERO, levámos a cabo uma pesquisa sistemática da literatura em setembro de 2020, utilizando bases de dados eletrónicas bem estabelecidas: PubMed, EMBASE, Cochrane Central Register of Controlled Trials, Cochrane Library, Web of Science, ClinicalTrials.gov e MedRxiv, procurando estudos que reportassem massas testiculares pequenas ou incidentais e a sua abordagem através de orquiectomia radical, orquiectomia parcial, ou vigilância ecográfica. Os artigos foram selecionados de acordo com o documento Preferred Reporting Items for Systematic Review and Meta-analysis (PRISMA). Resultados: 2126 abstracts foram avaliados e 112 artigos completos foram selecionados. Destes, 57 cumpriam os critérios de inclusão. Verificámos que massas testiculares incidentais foram detetadas em 1,74% dos pacientes que realizaram ecografia escrotal. 41,12% das massas testiculares cirurgicamente removidas eram benignas. O exame extemporâneo peroperatório (cortes de congelação), é uma ferramenta fiável para distinguir massas testiculares malignas de benignas (média de 93,05% precisão), apoiando o uso de orquiectomia parcial. Lesões benignas estavam associadas com diâmetros menores (< 1cm 68,78% benignas), eram frequentemente hipoecogénicas e com margens regulares à ecografia. Conclusões: As pequenas massas testiculares são frequentemente benignas. Padrões clínicos e ecográficos não são suficientemente fiáveis para incluir pacientes em protocolos de vigilância ecográfica e a cirurgia exploratória com exame extemporâneo é crucial para evitar a remoção de testículos com lesões benignas. Pesquisas futuras poderão revelar novas ferramentas imagiológicas ou biomarcadores para apoiar a decisão clínica.
Background: Controversy exists regarding the management of small testicular masses (STMs) incidentally diagnosed, as it is difficult to preoperatively ascertain the malignant or benign histology of these lesions. Although there is the risk of malignancy, an effort might be realistic in order to safely preserve testicles bearing benign masses. Objectives: To systematically evaluate the evidence regarding the prevalence of STMs, their benign or malignant histology and their management.Methods: After registration of the review protocol on PROSPERO, we conducted a systematic literature search in September 2020 using well-established electronic databases: PubMed, EMBASE, Cochrane Central Register of Controlled Trials, Cochrane Library, Web of Science, ClinicalTrials.gov and MedRxiv for studies reporting small or incidental testicular masses and their management by radical orchiectomy (RO), testis sparing surgery (TSS) or ultrasound (US) surveillance. Manuscripts were selected according to the Preferred Reporting Items for Systematic Review and Meta-analysis (PRISMA) statement. Results: 2126 abstracts were initially screened, and 112 full-text articles were retrieved. From these, 57 studies met the inclusion criteria. We verified that testicular masses were detected in 1,74% of patients undergoing US examination. 41,12% of all STMs removed by surgery were benign. Intraoperative frozen section examination (FSE) is a reliable tool to discriminate between benign and malignant testicular masses (average 93,05% accuracy), supporting the use of TSS. Benign lesions were associated with smaller diameter (< 1cm 68,78% benign), were often hypoechoic and exhibited regular margins on US. Conclusions: Small testicular masses are often benign. Clinical and US patterns are not accurate for including patients in surveillance protocols and TSS paired with FSE is pivotal for precluding the removal of testicles bearing benign lesions. Future research might unveil new imaging tools or biomarkers to support clinical management.
