dc.description.abstractIntrodução: A relação entre personalidade e sintomas somáticos tem vindo a ser amplamente explorada considerando como preditores significativos destes últimos o elevado nível de neuroticismo, a alexitimia e as relações de vinculação. Em Portugal, poucos estudos existem sobre estas relações, nomeadamente em pacientes com Fibromialgia. Este estudo procura aprofundar a compreenção da relação entre as variáveis supra indicadas e a somatização por forma a identificar aspetos a ter em consideração, dum ponto de vista psicossomático, em futuras intervenções de cariz preventivo.Métodos: Participaram 264 sujeitos, 56 com Fibromia (FM), 155 sem doença crónica (GC) e 53 com outras doenças crónicas (ODC), que preencheram um protocolo online constituído por questionário clínico e sociodemográfico, bem como medidas especificas de personalidade (NEO-FFI), Alexitimia (TAS-20), Vinculação (EVA) e Somatização (R- SOMS-2 e BSI). Foi realizada análise de variância para variáveis contínuas, utilizando one-way ANOVA, seguida do teste post hoc de Tukey (ou alternativa não paramétrica Kruskal-Wallis, seguido do teste LSD de Fisher) e Qui-Quadrado (com simulação de Monte Carlo) para variáveis nominais. Efetuaram-se análises de correlação bivariada (r de Pearson) e regressões hierárquicas múltiplas (método enter, com correção de Bonferroni).Conclusões: Verificámos que, na maioria das variáveis, existem diferenças significativas entre os sujeitos com FM e os outros dois grupos e que as características do ODC se assemelham ao GC. Considerando o R-SOMS-2, na FM apuraram-se correlações significativas entre Neuroticismo, “conforto com proximidade” e queixas somáticas e, tanto neste grupo como no GC, o Neuroticismo foi o único preditor estatisticamente significativo de Somatização. No BSI, apesar de se verificarem associações significativas entre Somatização e a maioria das variáveis independentes, nenhuma delas manifestou ser um preditor estatisticamente significativo, em nenhum dos grupos, o que pode corroborar a hipótese de amplificação somatosensorial, reforçando os efeitos do Neuroticismo e confirmando os estudos empíricos a respeito. Nos sujeitos com FM confirmaram-se níveis mais altos de Alexitimia e estilos de vinculação mais inseguros que nos outros grupos, com associações à Somatização pouco significativas. Nos FM os sintomas psicopatológicos não se mostraram preditores de Somatização pelo R-SOMS-2. Pelo BSI a Ansiedade constitui-se como preditor significativo ainda que expicando apenas cerca de 11% da variância na Somatização.por
dc.description.abstractIntroduction: The relation between personality and other somatic symptoms has been widely explored and there is evidence that high neuroticism, alexithymia and insecure attachment may act as predisposing conditions for somatization. Notwithstanding research within Portuguese population about the relations between the variables mentioned above is scarce, mostly concerning to Fibromyalgia. This study pretends to identify aspects that arise from a psychosomatics’ point of view and that might be important to account for to early intervention and prevention purposes. Methods: There were 264 participants, 56 Fibromyalgia, 155 no chronical diseases identified (control group GC) and 53 indicating several chronical diseases (ODC), which were asked to complete the Big Five Inventory NEO-FFI, TAS-20, AAS-R, R- SOMS-2, BSI and a demographic and clinical online survey. The first three scales evaluate neuroticism, alexithymia and adult attachment and the former somatization and somatosensory amplification. Variance analysis (ANOVA with post-hoc comparisons, Kruskal-Wallis with LSD Fisher test and Qui-Squared Monte Carlo simulation) was performed. We also conducted correlation analysis (Pearson r coefficient) and hierarchical multiple regressions (enter method and Bonferroni correction). Conclusions: There are significant differences within the FM group and the other two groups concerning to most of the variables. Additionally, no significant differences were found between the GC and ODC. It was found a significant correlation between somatization symptoms (R-SOMS-2), and two of the independent variables (neuroticism and closeness), but neuroticism was the only predictor statistically relevant, for both groups FM and GC. Significant association was found between somatization (BSI) and the independent variables but none of them was found to be a statistically relevant predictor, which might confirm the somatosensory amplification hypothesis, consistent with the empirical research concerning to personality and somatization. Additionally, FM group participants had higher levels of alexithymia as well as insecure attachment styles than the GC and ODC participants, which might as well confirm our initial hypothesis, even though none of them was found to be a statistically significant predictor. Psychopathology was not found to be a statistically significant predictor for somatization, concerning to R-SOMS-2, though for BSI Anxiety was found to be a significant preditor nonetheless explaining just 11% of somatizatization variance.eng
