Abstract: Introdução: A Diabetes Mellitus, entidade patológica que compreende diversas condições que partilham o fenótipo da hiperglicemia, caracteriza-se pelo seu caráter crónico, multiplicidade de dimensões e prevalência crescente. Existem cerca de 425 milhões de pessoas com a patologia em todo o mundo, pelo menos 1 milhão das quais sendo portuguesas. Os custos que uma doença com estas características acarreta são, como expectável, elevados, sendo que as projeções apontam para encargos na ordem dos 2,5 triliões de dólares já em 2030. Em Portugal, os Cuidados de Saúde Primários desempenham um papel primordial na gestão da Diabetes Mellitus - desde o diagnóstico e vigilância até ao tratamento. A nível nacional, e apesar da existência dos indicadores do Bilhete de Identidade dos Cuidados de Saúde Primários, que surgiram com o objetivo de monitorização de diversos parâmetros, é notória a ausência da consideração dos custos indiretos e intangíveis, e mesmo os diretos são apenas parcialmente abordados. Como tal, e considerando o contexto de escassez de recursos que torna indispensável o estudo do custo oportunidade das diferentes intervenções em saúde, a relevância deste estudo prende-se com a necessidade de melhor entender o acompanhamento dos doentes e custos associados à Diabetes Mellitus em contexto português. Métodos: Estudo observacional transversal pela coleta dos dados de cinco indicadores do Bilhete de Identidade dos Cuidados de Saúde Primários, para o triénio 2017 a 2019: 261 – Proporção de utentes com diabetes, com registo de risco de ulceração do pé no último ano; 262 – Proporção de utentes com determinação de risco diabetes tipo 2 registado nos últimos 3 anos; 271 – Índice de acompanhamento adequado em utentes com Diabetes Mellitus; 350 – Custo com terapêutica do doente com Diabetes Mellitus; 351 – Custo com terapêutica do doente com Diabetes Mellitus controlado. Estes indicadores foram estudados a nível nacional e regional e neste ainda em função da média em Agrupamentos de Centros de Saúde sorteados numa proporção de metade mais um, ordenados alfabeticamente Foi realizada estatística descritiva e inferencial. Uma vez que não existe normalidade da distribuição numérica transversal a todos os indicadores, realizou-se estatística não paramétrica: Teste de Kolmogorov-Smirnov. Para diferenças entre os indicadores na distribuição por mais que dois grupos utilizou-se também estatística não-paramétrica: teste de Kruskal-Wallis. Calcularam-se dinâmicas de crescimento.Resultados: De um modo geral, existe um aumento do valor médio nacional no triénio considerado, para todos os indicadores estudados. Para a análise inferencial, considerou-se para diferença significativa o p<0,001. A realização do Teste de Kruskal-Wallis permitiu demonstrar não existirem diferenças significativas nos valores médios dos indicadores por região por ano, com exceção do indicador 261 no ano 2019. Já a realização do mesmo teste para os ACES incluídos no estudo por anos, demonstrou diferenças significativas para os indicadores 262 e 350. Verificaram-se dinâmicas de crescimento positivas entre os anos considerados.Discussão e Conclusão: Os resultados, obtidos em contexto de escassa informação nacional sobre o tema, permitem melhor entender os encargos associados à Diabetes Mellitus em Portugal. Foi possível constatar uma disparidade de valores médios entre ARS e ACES para alguns dos indicadores analisados. O indicador 261 (Proporção de utentes com DM, com registo de risco de ulceração do pé no último ano) apresenta diferenças significativas entre regiões para o ano 2019 (estando a ARS Norte nos tercis mais elevados, as ARS Centro e LVT nos tercis mais baixos, e as ARS Alentejo e o Algarve distribuídas entre os diferentes tercis de forma mais ao menos homogénea). Constatam-se ainda existirem para os indicadores 262 (Proporção de utentes com determinação de risco DMT2 registado nos últimos 3 anos) e 350 (Custo da terapêutica do doente com DM) diferenças entre ACES. Estas diferenças assentam provavelmente numa multiplicidade de fatores, que devem ser alvo de estudos subsequentes. Algumas recomendações futuras passam pela possível reformulação dos indicadores pré-existentes, no sentido de permitir uma melhor constituição destes; a construção de novos indicadores, que permitam a estimativa de novas dimensões relativas à patologia ou a correlação entre dimensões que já são estimadas; a implementação de novas de medidas de acompanhamento dos doentes; e ainda a implementação de medidas preventivas da doença.
Introduction: Diabetes Mellitus is a pathological entity that includes several conditions that share the phenotype of hyperglycemia. It is a chronic disease of multiple dimensions and increasing prevalence. There are about 425 million people with the disease worldwide, at least 1 million being Portuguese. Its costs are considerable, with projections pointing to charges around 2.5 trillion dollars as early as 2030. In Portugal, Primary Health Care plays a major role in the management of Diabetes Mellitus - from diagnosis and surveillance to treatment. In spite of the existence of Primary Health Care Identity Card indicators, that aim to monitor several parameters of this disease, it is notorious the exclusion of indirect and intangible costs, and even direct ones are only partially addressed. Considering the context of resources scarcity that makes the study of the opportunity cost of different health interventions necessary, this study aimed to better understand the monitoring of Diabetes patients and its costs in the portuguese context.Material and Methods: Observational cross-section study by collecting data from five indicators of the Primary Health Care Identity Card: 261 - Proportion of users with diabetes, with a risk of foot ulceration registered in the last year; 262 - Proportion of users with type 2 diabetes risk registered in the last 3 years; 271 - Adequate follow-up index in users with Diabetes Mellitus; 350 - Cost of therapy for patients with Diabetes Mellitus; 351 - Cost of therapy for patients with controlled Diabetes Mellitus. These indicators were studied on a national level and also by heath regions on the Portuguese National Health Service. At the regional level, the indicators were studied for randomized Health Centre Groupings, representing 50% plus one by region alphabetically ordered. Descriptive and inferential statistics were performed. Since there is no normal distribution across all indicators, non-parametric statistics were performed: Kolmogorov-Smirnov test. For differences between indicators in the distribution by more than two groups, non-parametric statistics were also used: Kruskal-Wallis test. Trends were calculated.Results: In general, there is an increase in the national mean value in the three years considered, for all the indicators studied. For inferential analysis, p <0.001 was considered for significant difference. The Kruskal-Wallis Test assessed that there are no statistically significant differences in the mean values of the indicators by region per year, with the exception of indicator 261 in the year 2019. The same test for the Health Centers Groupings included in the study by years considered, showed statistically significant differences for indicators 262 and 350. Positive trends of costs were observed for the studied years. Discussion and Conclusion: The results, attending the limited national information on the subject, allow a better understanding of the costs associated with Diabetes Mellitus in Portugal. It was possible to describe a disparity in mean values between Regional Health Administrations and Health Centers Groupings for some of the analyzed indicators. Indicator 261 (Proportion of users with DM, with risk of foot ulceration registered in the last year) shows significant differences between regions for the year 2019 (with the Norte Regional Heatlh Administration at the highest tertiles, Centre and Lisbon and Tagus Valley Regional Heatlh Administrations at the lowest tertiles, and Alentejo and Algarve Regional Heatlh Administrations distributed among the different tertiles in a relatively homogeneous way). There are also differences between Health Centers Groupings for indicators 262 (Proportion of users with DMT2 risk determination registered in the last 3 years) and 350 (Cost of therapy for patients with DM). These differences are probably explained by a multiplicity of factors, which should be the subject of subsequent studies. Some future recommendations include the possible reformulation of pre-existing indicators, in order to allow a better constitution of these; the creation of new indicators, which will allow the estimation of new dimensions related to the disease or the correlation between dimensions that are already estimated; the implementation of new patient follow-up measures; and the implementation of preventive measures for the disease.
