A Sobrecarga e a Qualidade de Vida dos Cuidadores Informais: Um estudo no Município de Cantanhede.

Abstract

Assente num desenho metodológico de caráter quantitativo do tipo exploratório, descritivo, correlacional, observacional e transversal o presente estudo, desenvolvido no Município de Cantanhede, pretendeu determinar o impacto que o ato de cuidar exerce na vida do cuidador informal, por meio da avaliação do nível de sobrecarga e de qualidade de vida (QdV). Para este fim, e partindo de uma entrevista estruturada, foi aplicado um questionário sociodemográfico conjuntamente com três instrumentos de medição, nomeadamente a Escala de Sobrecarga do Cuidador (ESC), o SF-36 v2 e o EQ-5D-5L. Selecionados a partir de um método de amostragem não probabilístico por conveniência, foram recolhidos dados de 96 cuidadores informais de pessoas dependentes. Dos resultados obtidos, verifica-se uma semelhança do perfil deste cuidador com aquele que se encontra retratado na literatura. Para além da supremacia feminina, observa-se que este papel é assumido maioritariamente pela descendência do doente (filha), com uma idade média de 63 anos, casadas e com um baixo nível de escolaridade. Apesar de também presente enquanto cuidador principal, a figura masculina surge neste estudo essencialmente numa posição de apoio secundária. Complementarmente a estes dados, e no que respeita àquele que foi o ponto central desta investigação, 58,3% dos cuidadores não apresenta nenhum índice de sobrecarga, resultado ao qual se junta a boa perceção que na generalidade possuem acerca da sua qualidade de vida (0,88), considerando ainda reunir melhores condições a nível da componente mental (52,6) face à dimensão física (40,4). Por sua vez, ao correlacionar as variáveis dependentes e independentes, não foi encontrada uma diferença significativa entre a sobrecarga e a natureza da dependência do doente, bem como entre a sobrecarga e o tempo de prestação de cuidados. Contrariamente, constatou-se que quanto mais velho é o cuidador, maior é a sobrecarga nas dimensões relação interpessoal (p=0,007) e perceção de autoeficácia (p=0,038). Ademais, foi identificada uma relação altamente significativa (p<0,01) entre a escala global de sobrecarga e a saúde mental do cuidador (p<0,001), ao passo que na dimensão física se revelou apenas significativa (p=0,028). Por fim, verificou-se que os cuidadores que de modo mais frequente se sentiram sozinhos e/ou sem o apoio de terceiros, revelaram níveis mais baixos de qualidade de vida, facto que evidencia a importância que o suporte, seja ele informal ou formal, assume na vida de quem cuida.

Based on a quantitative methodological design of the type exploratory, descriptive, correlational, observational and transversal the present study, developed in the Municipality of Cantanhede, intended to determine the impact of the act of taking care of someone exerts in the life of the informal caregiver, by assessing the level of the overload and the quality of life (QoL). For this purpose, and starting from a structured interview, a sociodemographic questionnaire was applied together with three instruments of measurement, especially the Burden Interview Scale, the SF-36 v2 and the EQ-5D-5L.Selected from a non-probabilistic method of sampling by convenience, data was collected from 96 informal caregivers of dependent people. From the results obtained, there is a similarity in the profile of this caregiver with the one that is portrayed in literature. Despite the feminine supremacy, it is observed that this role is predominantly undertaken by the patient’s descendants (daughter), with an average age of 63 years old, married and with a low level of education. Although the male figure is also present as a main caregiver, it appears in this study essentially as a supportive secondary position. Complementarily to this data, and with regards to what was the central part to this investigation, 58,3% of the caregivers didn’t show any rate of overload, a result that joins with their general perception about their quality of life (0,88), considering still that they have better conditions in terms of the mental component (52,6) in relation to the physical dimension (40,4). In turn, when correlating the dependent and independent variables, a significant difference was not found between the overload and the nature of the patient’s dependence, as well as the difference between the overload and the time of caring. Contrarily, it was found that the older the caregiver, the heavier the overload is in the interpersonal relationships (p=0,007) and the perception of self-efficiency (p=0,038). Furthermore, a highly significant relationship was identified (p<0,01) between the global scale of overcharge and the caregiver’s mental health (p<0,001), whereas in the physical dimension it was only significant (p=0,028). Finally, it was verified that the caregivers that more often felt lonely and /or without support from a third party, presented lower levels of quality of life, a fact that demonstrates the importance of support, whether informal or formal, in the life of the person who is caring.
