dc.description.abstractO consentimento informado, um documento eticamente e legalmente obrigatório antes do início do estudo, expressa a decisão de participar num ensaio voluntariamente e demonstra que, quem o assina, representante legal ou o próprio sujeito do ensaio, possui as capacidades necessárias para o realizar. Este formulário especifica os direitos do participante e a que procedimentos será submetido. Contudo, faltam métodos que avaliem a perceção do consentimento informado por parte dos participantes e a qualidade do mesmo. Estudos anteriores revelam que os participantes não percebem o consentimento informado na totalidade e que, por consequência, é um dos motivos da desistência do ensaio clínico. O objetivo deste estudo foi avaliar a capacidade de entendimento do consentimento informado e os possíveis condicionantes que levam à melhor ou pior compreensão deste por parte dos participantes de um ensaio clínico. A metodologia passou pela aplicação do questionário Quality of Informed Consent (QuIC) a uma amostra de 100 participantes incluídos em diversos ensaios clínicos de fase III na área de cardiologia, depois de ser traduzido e adaptado para a língua portuguesa. Foram recolhidos dados sociodemográficos, como género, idade, situação familiar e profissional e nível de escolaridade, assim como a autoavaliação de cada participante relativa à participação no estudo e da sua saúde em geral. Nesta amostra, 85% dos doentes eram do sexo masculino e a média de idades foi de 67,32. A maioria dos participantes (70%) afirmaram ser aposentados e 49% referiram ter completado o 1ºciclo de escolaridade. Todos os doentes avaliaram a sua participação e a sua saúde de forma positiva. Todos conhecem a principal razão da realização de ensaios clínicos e 97% percebem que ajudarão no tratamento de futuros doentes. Quase todos os doentes (97%) perceberam que, ao assinar o consentimento informado, iriam ser inseridos num ensaio clínico. Pelo contrário, nenhum dos participantes percebeu que o tratamento experimental a que estão a ser submetidos não está provado como sendo o melhor possível para a sua patologia. A última questão que pede aos participantes do estudo, que avaliem o seu nível de compreensão, de um modo geral, do ensaio clínico quando assinou o consentimento, obteve uma pontuação de 70,8 (0-100). A partir destes dados foi possível concluir que o nível de escolaridade e a condição social não afeta diretamente a compreensão do consentimento, mas sim a crença de que o tratamento novo será a cura para a sua doença. É necessária uma maior consciencialização sobre a importância da leitura do consentimento informado, e eventual adaptação do mesmo ao tipo de estudo e às características dos doentes, para que os participantes percebam o melhor possível o protocolo do ensaio clínico, sobretudo, os riscos que advém do tratamento experimental, assim como esclarecer os seus direitos por participarem no estudo.por
dc.description.abstractInformed consent, an ethically and legally mandatory document before the start of the study, expresses the decision to participate in a trial voluntarily and demonstrates that whoever signs it, legal representative or the subject of the trial, has the necessary skills to perform it. This form specifies the participant's rights and the procedures that will be submitted. However, there is a lack of methods to assess the participants' perception of informed consent and its quality. Previous studies reveal that participants do not fully understand informed consent and that, as a result, it is one of the reasons for dropping out of the clinical trial. The aim of this study was to evaluate the ability of understanding informed consent and the possible conditions that lead to a better or worse understanding of it by the participants of a clinical trial. The methodology included the application of the Quality of Informed Consent (QuIC) questionnaire to a sample of 100 participants included in several phase III clinical trials in cardiology, after being translated and adapted into the Portuguese. Sociodemographic data were collected, such as gender, age, family and professional situation and education level, as well as the self-assessment of each participant regarding their participation in the study and their general health. 85% of the sample were male and the average age was 67.32. 70% said they were retired and 49% said they had completed the primary school. All patients evaluate their participation and their health positively. 100% know the main reason for conducting clinical trials and 97% understand that they will help in the treatment of future patients. 97% realize that by signing the informed consent, would be included in a clinical trial. In contrast, none of the participants (0%) realizes the experimental treatment being submitted is not proven to be the best for its pathology. The last question asks the study subjects to assess their level of understanding, in general, of the clinical trial when they signed the consent, obtained a score of 70.75 (0-100). With these results it was possible to conclude that the level of education and their social condition may not directly affect the understanding of consent, but the belief that the new treatment will be the only cure for their disease. It is necessary a greater awareness about the importance of reading informed consent, so that participants understand as much as possible about the protocol of the clinical trial, especially the risk that results from the experimental treatment, as well as clarify their rights to participate in the study.eng
