Abstract: A neuropatia hereditária de Leber é uma doença genética mitocondrial rara que se carateriza por uma diminuição severa da acuidade visual, indolor e bilateral (em 25% dos casos), consequente da degeneração das células ganglionares da retina. Embora esta seja a manifestação mais comum, existem outras patologias associadas a esta neuropatia, tais como, arritmias cardíacas, neuropatias periféricas, distonia e mioclonia, podendo estar ainda associados alguns distúrbios neurológicos.A neuropatia de Leber afeta maioritariamente indivíduos do sexo masculino, com idades compreendidas entre os 15 e os 30 anos e é apenas transmitida pela herança materna. Embora a discrepância entre sexos seja ainda de causa desconhecida, atualmente são referidos diferentes aspetos como fatores hormonais, exposição ao tabaco, álcool ou até mesmo interferência de certos genes do cromossoma X.Efetivamente, esta neuropatia é causada por diversas mutações no DNA mitocondrial (mtDNA) que codifica subunidades do complexo I da cadeia respiratória mitocondrial, sendo as mais comuns a G3460A na subunidade ND1, a G11778A na subunidade ND4 e a T14484C na subunidade ND6. Estas mutações são responsáveis por defeitos na síntese de ATP, excesso de produção de espécies reativas de oxigénio (ROS) e apoptose das células ganglionares da retina.São diversas as abordagens terapêuticas para prevenir a diminuição da acuidade visual bilateral. A utilização de fármacos como a Idebenona, EPI – 743 e Brimonidina e a terapia génica têm demonstrado resultados encorajadores no tratamento desta neuropatia. Para além disto, é fundamental aliar a estas abordagens o aconselhamento ao doente para resultados mais promissores.
Leber's hereditary neuropathy is a rare mitochondrial genetic disease that is characterized by a severe decrease in visual acuity, painless and bilateral damage (in 25% of cases), resulting from the degeneration of the retinal ganglion cells. Although this is the most frequent manifestation, there are other pathologies associated with this neuropathy, such as cardiac arrhythmias, peripheral neuropathies, dystonia and myoclonus, which may also be associated with some neurological disorders. Leber's neuropathy mainly affects males, aged between 15 and 30 years and is only transmitted by maternal inheritance. Although the discrepancy between genders is still anunknown cause, actually there are other factors such as hormones, exposure to tobacco, alcohol or even interference from certain X chromosome genes being taken into account. Effectively, this neuropathy is caused by several mutations in mitochondrial DNA (mtDNA)that encode complex I subunits of the respiratory mitochondrial chain the most common beingthe G3460A in ND1 subunit, the G11778A in ND4 subunit and the T11488C in ND6 subunit. These mutations are responsible for defects in the ATP synthesis, overproduction of reactiveoxygen species (ROS) and apoptosis of retinal ganglion cells.There are diverse therapeutic approaches to prevent the decrease of the bilateral visualacuity. The use of drugs, such as Idebenone, EPI-743 and Brimonidine and the gene therapyhave demonstrated encouraging results in the treatment of this neuropathy. In addition, toobtain more promising results it is essential to combine these approaches with advice to thepatient.
